A PAIR OF TEACHERS FROM DIFFERENT SCHOOLS
Note: Use this application if you are pairing with a teacher from another school. One of you must be
from mathematical sciences and one from biological sciences.
The DIMACS Bio-Math Connect Institute program, running July 17 to July 29, 2005, will be held at
Rutgers University in Piscataway, New Jersey. Participants must have e-mail access from May 2005
through August 2006.
Checklist:
1. Five page Application form
2. Statement of Understanding: signed by applicant and school principal.
3. Graduate Credit Requirements: signed by applicant.
4. Letters of Recommendation. One from each teacher’s school principal and one from each
teacher’s curriculum supervisor or department chair. (Each teacher includes their own.)
Although these items may be mailed separately, only complete application files will be reviewed.
Deadline for submitting all documents is April 1, 2005.
Early Acceptance deadline is January 24, 2005.
