The form should be completed by the relevant Head of Department. 2. Boxes 1, 2, 3, 6 and 7 in Section A and the whole of Section B, in consultation with the external examiner if necessary, should be completed in all cases. 3. Boxes 4 and 5 in Section A should be completed as required – information regarding current duties is only needed if the external examiner’s duties are being extended or modified. 4. The Head of Department should then forward the completed EE/Extension form to the Faculty Quality Assurance Officer for: i) ii) Faculty scrutiny and approval; and onward transmission to the Quality Assurance Officer (Collaborative Activity) for Institutional approval by the Deputy Vice-Chancellor (Academic) on behalf of the Senate. Please note that an External Examiner's normal period of appointment is four years and four months. An extension of one year is only allowed if there is a demonstrably good reason for so doing. 6.
