Sponsor, please prepare and attach the following:
1.
Submit a ten page (maximum) written, pictorial and graphic portrayal of the project, program or procedure where the
Value Methodology was used. A typed statement of the nominee’s credentials and accomplishments for the Outstanding
VM Projects & Programs award. This is not meant to be an individual award. The purpose of the award is to recognize
corporations, sole proprietors, consulting firms, or government agencies. Please identify participants.
2. Please address as a minimum Basic Criteria 1, 2, and 3 in the nomination form demonstrating the following
accomplishments:
a. Clearly defined goals and objectives
b. Effective use of teamwork in analyzing, evaluating, and recommending Value Methodology solutions
c. Documentation of the acceptance and implementation of the results of the Value Methodology
3. Please provide an effective presentation, communication, and sharing of the accomplishment for others to learn.
Description of project, program, or procedure including a digital slide program for the judges to better understand the
nomination. The slide program may be shown at the conference. Additional display posters could also be prepared for the
conference.
4. Illustrate the Value Methodology applications in the study or program
5. Show results as measured in performance improvements, documented savings, advancement of the mission of the
organization, program success, etc.
6. Address and attach the basic criteria that may apply to the nomination
Applications must be submitted three months prior to the annual conference.
Should this nomination be approved to receive the award, please provide complete contact information for up to four media outlets
suitable for sending press releases. One award may be given in each of the four categories. Honorable Mention certificates will be
given for all submittals.

group according to numerical and alphabetical chronology. Each criterion has a value of 10 points. The nominee may receive points
ranging from 0 to 10 for each criterion based upon substantiating data. Use additional sheets as needed. Asterisks designate
