Basic Guidelines for Filling Out the Salaried Leave and Overtime Reporting Form 1. Enter leave into the correct Pay Periods: Example: 4/20/15 should be on the 4/16/15 – 4/30/15 Pay Period. No other leave outside of 4/16/15 – 4/30/15 would be on the leave slip. 2. Enter exact time you are gone. This is a workers compensation issue! 3. You must work 4 hours before a 15 minute break is allowed. Do not record breaks. 4. If you take time before or after regular lunch break, record the times outside of your lunch break. 5. Supervisors must audit and sign form. 6. Make sure codes and times on the left, match what is entered on the right.
