Explanatory notes
1
This unit standard will be assessed on the basis of evidence of demonstrated
performance in the workplace or in simulated situations that demand performance
equivalent to that required in the workplace.

Forms can be completed electronically or in hard copy, but must be completed in
accordance with workplace practices and any instructions on the form itself.

Definitions
Fields refer to information that may include but is not limited to: name, address,
phone numbers, email address, company name, meter readings, measurements,
dates, times, quantity, cost and financial information.
Workplace forms refer to forms that are required to be completed for a workplace.
These may have been generated outside of the workplace (i.e. ACC, supplier order
forms).
Workplace requirements are documented policies and procedures or established
protocols for workplace performance.

All activities associated with this unit standard must comply with the requirements of
Privacy Act 1993.

Complete workplace forms.
Range
evidence of three different forms is required;
each form must contain a minimum of eight fields.

Evidence requirements
1.1
Workplace forms are completed in accordance with all instructions.
Range
instructions may include - instructions on form, workplace policy or
procedure, industry standards, supervisor direction.
1.2
Workplace forms are completed accurately and in accordance with workplace
requirements.
1.3
Relevant supporting evidence is supplied as indicated on the forms.
1.4
Information provided is clear and legible.

before they can report credits from assessment against unit standards or deliver courses

NZQA before they can register credits from assessment against unit standards.
Providers and Industry Training Organisations, which have been granted consent and
which are assessing against unit standards must engage with the moderation system that
applies to those standards.
