Cognos Report Request Form Instructions The Cognos Report Request Form provides the BI Team with the requirements necessary to create or modify a Cognos report. Being detailed and specific in your request will allow us to provide you with the information that you need in a complete and timely manner. We may need to contact you or schedule a meeting with you to review and/or clarify the report requirements or changes you are requesting. Please feel free to email the BI Team if you have any questions or need assistance in completing the report request form. Please allow a minimum of 7 to 10 business days to complete the report request. Depending on the complexity of the request, we may require more time to deliver the report. **Your request will be recorded and submitted to the BI Team. The team determines priority and responsibility for all requests. Note: in some cases the report request may be referred to a Report Writer from your department. Once you have completed the form click on File then Save & Send to save a copy of the form for your records and send to the request as an attachment to IRTHELP@stthomas.edu and the IRTBITEAM@stthomas.edu Below are some tips for completing steps 3 through 9 in Detail Sections 2 or 3 of the report request. Title for New Report Create a title that is meaningful and provides an accurate description of what the report is providing. What is the purpose of the new report? Describe the new report being requested. Include specifics, if known, for selection criteria of records to be included in the report. o For example: I need a list of all undergrad students in the College of Arts and Science with a Cumulative GPA greater than 3.5. Please describe, in detail, the modification you are requesting. Describe the modification being requested. Include specifics, if known, for selection criteria of records to be included in the report. o For example: Please modify the Student Athletic report in the AARC folder to include locker information. List data fields/columns you would like displayed in the final report. List what data fields/columns you would like displayed in the report and what order you would like them in. Filters set limits to the data retrieved in a query. o For Example: Output only undergraduate students in the College of Business, who have a CUM_GPA >= 3.5. (Three filters are needed; (1) UG Students, (2) College of Business, (3) CUM_GPA >= 3.5) List any Summary information you require. o For example: I need a count of all seniors by college, then program. Specify the order the data should be sorted by. How do you want the report to be sorted? o For example: sort ascending by Academic Period, then by College, and finally by Student Last Name. List any prompts/parameters you want included in this report. Parameters and prompts provide dynamic limits on a query. Parameters are placeholders that require a value to determine what data to report on, and prompts ask the user to provide a value or values for the corresponding parameter. o For example: Term, College, Major, etc…
