Department Program Review Findings:
Interviews and classroom observations indicate students identified as Limited English
Proficient (LEP) are not receiving sheltered content instruction from a trained and
qualified teacher and additional instruction in English as a Second Language by a certified
ESL teacher.
Description of Corrective Action:
At the time of the site visit there were two students attending who were identified as ELL
students. SEC currently, SEC has no students attending who are ELL at this time.
IF/When SEC enrolls any identified LEP students we will work with the the sending district
to provides ESL or ELL consultant to provide SEI assistance.
As per DESE regulations regarding ELL and SEI, all teachers will be required to obtain SEI
endorsement by the year 2016.
At this time, it is our understanding that the SEI endorsement course is available to
identified cohort groups throughout the Commonwealth of Massachusetts. SEC has not
been identified in any cohort group. It is our understanding that the SEI endorsement
course, and/or Professional Development trainings, and/or the SEI endorsement test will
become available in the Spring of 2014. Once the course, PD opportunities and the test
are available, SEC will insure that all teachers either enroll in an endorsement course or
attend PD and take the appropriate test leading to endorsement.
The issue of having internally trained ESL/ELL teachers should be resolved by May, 2014
assuming the SEI endorsement test and courses availability remain on schedule.

Springdale states the efforts the program has made to comply with this standard and the
program's intent to have teachers trained in ESL/ELL by May, 2014. However, currently,
the program does not have students identified as limited English proficient (LEP).

Springdale must submit a copy of the written plan addressing how to effectively serve
limited English proficient (LEP) students as indicated in the description above.

While a review of documentation shows the student separation policy contains the

records and interviews indicate that the length of time, reason for the intervention, who
approved and who monitored the time out is not documented.

necessary for separations.

Department Program Review Findings:
Observations revealed that Springdale Educational Center Day Program is not maintaining
the Student:Licensed Educator: Aide ratio of 8:1:1 as most recently approved by the
Department in one classroom.
Description of Corrective Action:
This item is regarding Young Adults Transitional Classroom.
Criteria 1.2, Program and Student Description clearly states that our school operates the
SEC young adults program (classroom) from 8:00 a.m. - 2:00 p.m.
Criteria 6.2 School-To-Work clearly states the types of vocational activities and work
opportunities our students participate in.
Student IEP's in this classroom reflect that the personnel responsible for the vocational
portion of their day are SPED OR paraprofessionals. Parents, students, and LEA's have
agreed to this placement, i.e. signed IEP's.
The schedule has been reworked to allow for the following to occur during the morning
academic/social emotional time:
8:00 a.m. - 9:00 a.m. : Licensed Teacher
9:00 a.m. - 9:30 a.m. : Licensed Teacher
9:30 a.m. - 10:00 a.m. : Clinician for Psychoed and/or Therapy group
10:00 a.m. - 11:00 a.m. : Licensed Teacher
11:00 - 2:00 p.m. includes lunch and vocational activities.

Department Program Review Findings:
A review of documentation and interviews indicate that one teacher is not appropriately
licensed for the subject area(s) being taught.
Description of Corrective Action:
Mr. Jay Yarymowicz is the teacher of record for the High School students. He currently
holds professional level license in History. He has applied for Moderate Special Needs 5 12 and has been working towards completing all requirements identified by the DESE to
qualify for the Initial level license Moderate Special Needs 5 - 12. Mr. Yarymowicz
recently passed the required MTEL for this license and completed all elements identified
by DESE.
On June 4, 2013 Mr. Yarymowicz sent his packet of required documentation to the DESE
Licensure Department for their review to grant his Initial level 5 -12 Moderate Special
Needs license. He and we will make all attempts to expedite the process. However, given
the length of time it may take, this issue should be resolved by September, 2013.

While interviews indicate that new staff receive all required trainings and are not assigned

trainings are not offered in new staff orientation. In addition, the Department is unable to
verify that staff are not assigned direct care duties until receipt of all required trainings
due to the absence of such documentation.

While interviews indicate all staff receive all required trainings, a review of staff records

unable to verify if all staff receive an average at least two hours of in-service training per
month due to the absence of staff training documentation.
Description of Corrective Action:
The staff training sign in form has been revised to reflect the concerns of the CPR and
include clear statements of all mandatory training for all staff. It includes title and
description of the training, name and title of the trainer, time begin and time end, number
of hours and indicates whether handouts were used.
In addition, every two months, a summary of all trainings per staff will be included in
their personnel file.
