Student record review and documents indicate that written information on the student's
progress toward his or her annual goals are not always reflective of the goals and
benchmarks set forth on the student's IEP.
Description of Corrective Action:
Meetings will be held with all SE teachers and service delivery personnel, no later than the
end of January 2014, to review the specifics of Criterion #13.
An email inquiry will be sent in no later than the end of January 2014 to all personnel
completing progress reports to ask for specific questions regarding progress reporting.
Progress reports from January through June 2014 will be reviewed by the Director and
SSO Administrator to review compliance of progress reports and goals and benchmarks.
Attention will be focused on if there are continuing difficulties and whether or not it is
systemic or individual. Either individual or group follow up meetings will be held to
address any non-compliance noted.
A department PD will be planned for either late in the 13-14 school year or the beginning
of the 14-15 school year to focus on Writing Measurable Goals & Objectives and Progress
Reporting.

and then again April 2014 through June 2014. Adherence to goals and objectives will be
the focus of the review. A follow up meeting with personnel will be held to discuss any
ongoing compliance concerns.
The SSO Director and Administrator will investigate and schedule a professional
development opportunity for all SE personnel who develop IEPs to focus on Writing
Measurable Goals and Objectives and progress reporting.

Student record review and documents indicate that although parents, at the conclusion of
the IEP Team meeting, are provided with a summary of the proposed IEP, they do not
consistently receive the complete IEP within ten school working days.
Description of Corrective Action:
A meeting will be held with Special Education personnel in December 2013 to review and
update all personnel about the specifics of SE Criterion #18B.
The agenda of the meeting will include review of our internal time frame break down so
that each Team member has a clear understanding of what time frame to complete their
portion of the final IEP so that it can be complete and in the mail within the mandated and
expected time frame. The meeting agenda will also include review of the District
expectation that a comprehensive summary including all the necessary components as
indicated in the CPR finding will be provided at the end of the Team meeting along with a
copy of the Team meeting notes.

Student record review and documents indicate that the justification on a student's IEP of
non-participation in the regular education setting is not reflective of their overall needs
and does not state how the nature or severity of the student's disability limits his or her
participation in the regular education setting with the use of supplementary aids and
services.
Description of Corrective Action:
Although our belief is the our personnel have a solid understanding and practice regarding
least restrictive environment (LRE), we will review Criterion #20 at an Education Team
Leader meeting and the SE department meeting.
A email message will be sent no later than January 2014 regarding some sample
justification language for IEP writing and a meeting will be held in early 2014 to review
IEP writing regarding removal from the GE classroom.
IEPs will be reviewed from January through March 2014 and a second time from April
through June 2014 with specific attention the justification section of the IEP. Follow up
individual or group meetings will be held to address any ongoing concerns.
The district will continue with steps to enhance Co-Teaching and UDL to demonstrate the
position and practice of educating diverse learners in the general education classroom.
Workshops on both Co-Teaching and UDL will be held during the 13-14 School Year
including both SE and classroom teachers.
