Observation and interviews demonstrated that, at the Conant School, special education
classes are clustered and in trailers which are removed from the regular education
classrooms.
Description of Corrective Action:
The Director of Pupil Services (or Designee) will meet with the Building Principal and other
relevant parties to identify the relocation of special education classrooms so that they are
no longer clustered and removed from other regular education classrooms. Moreover, the
Director of Pupil Services will facilitate training regarding the requirement that school
districts must provide facilities and classrooms for eligible students that:
Maximize the inclusion of students into the life of the school
Provide accessibility to fully implement each student's IEP
Provide physical equality to the same average standards of general education facilities
and classrooms
Anticipated Results:
The relocation of identified Special Education classrooms will be completed by the end of
the April 2012 break

The district has proposed that the Director of Pupil Services and the building Principal will
assess the allocation of classroom space within the school and identify appropriate space
to relocate the special education classes. The district has proposed that the classrooms
will be relocated in April 2012, however since the district has already begun consideration
of this issue the Department is ordering that the classrooms be relocated by January
2012.
Department Order of Corrective Action:
The district administration and building principal will develop a plan by October 2011 to
relocate special education classrooms in order to maximize inclusion of special needs
students into the life of school. This plan will include completion date of January 2, 2012

Required Elements of Progress Report(s):
The district will submit a written plan that includes a proposed floor plan demonstrating
that the special education classrooms at the Conant School will be relocated within the
facility to maximize inclusion in the life of the school and a statement of assurance from
the superintendent that the classrooms will be relocated no later than January 2, 2012.
Please provide this to the Department on or before October 31, 2011.
The district will provide confirmation regarding a scheduled onsite visit by the DESE to
review the classroom relocation at the Conant School. Please provide this to the
Department on or before January 20, 2012.

Criterion & Topic: ELE 10 Parental Notification

Department CPR Finding: Review of records and interviews showed that progress reports on
language acquisition are issued once, at the end of the academic year, which is not the same frequency
as general education reporting.
Narrative Description of Corrective Action: In response to the Coordinated Program Review,
the District added a .2 FTE ELL Coordinator position for the 2011-2012 school year. In
addition to annual training regarding forms, processes and procedures for ELL students, ELL
translation services offered by the school district will also be reviewed. Moreover, the District
has developed an internal tracking form; a checklist for each ELL student that ensures
compliance with all ELL procedures, including but not limited to progress report timelines.
This ELL reporting checklist will indicate corresponding dates for all elementary building
based report cards.

Evidence of Completion of the Corrective Action: Student records include required parent
notification letters, progress reports, translated report cards, and student schedules. Student
work samples and assessments will also be found in the student records maintained by the
District.
Description of Internal Monitoring Procedures: The ELL Coordinator will annually review the
student records of identified ELL students to ensure that appropriate documentation of ELL
services is included in the student record maintained by the District. On an annual basis, the
Director of Pupil Services will randomly select student records for review as well. To ensure
that appropriate staff members are trained in the maintaining of required documents within the
student record, the Director of Pupil Services will facilitate an annual Fall training for
elementary building principals, designated ELL staff and school counselors, regarding the
maintenance of ELL records and required documentation including but not limited to progress
reports. Verification will be provided through sign-in sheets.
