Timothy Kenefick, Mischka Maxwell PURPOSE The purpose of this procedure is to ensure that all Vendors, Contractors, and Professionals who perform work for the University meet insurance requirements. II. SCOPE This procedure applies to all University Departments/Colleges. III. PROCEDURE All Vendors, Contractors, and Professionals must provide Florida Tech with a Certificate of Insurance evidencing the coverage prior to the commencement of any work on campus. The Certificate must include Florida Tech as the additional insured. The Certificate should also indicate whether the General Liability policy is written on a claims-made or occurrence basis. In addition, the Certificate must indicate that it is the responsibility of the Insurance carrier to provide the University with 30 days’ notice prior to cancellation or expiration of the Insured’s policy. If the ‘Scope of Work’ or service is considered to present an unusual or exceptional risk, the University may require additional insurance above the University’s standard requirements. Architects, Engineers, or other Professionals/Consultants may be required to carry additional professional liability insurance. The University will determine this requirement on a case by case basis. It is recognized that certain small vendors and contractors may not be able to afford to purchase the standard insurance limits the University requires. However, no exceptions to the limits outlined are permitted. A copy of the Vendor’s, or Contractor’s, or Professional’s Certificate of Insurance must be sent to the Procurement Department to ensure compliance with the University’s insurance requirements prior to the commencement of any work on campus. Coverage Limits Commercial General Liability (CGL): Each Occurrence $1,000,000 General Aggregate $2,000,000 Automobile Liability $1,000,000 Workers Compensation (WC) Statutory Limits set by the State of Florida Employers’ Liability $1,000,000 Bodily Injury Each Accident $1,000,000 Disease Policy Limit $1,000,000 Disease Each Employee Commercial General Liability should include Bodily Injury and Property Damage Liability, Contractors Liability, Contractual Liability, Product Liability and Completed Operations. The COI should also indicate whether the General Liability policy is written on a claims-made or occurrence basis. Umbrella/Excess Liability: The above limits may be reached by means of an umbrella or excess liability policy. Other: If a product, service, or activity is considered to present an unusual or exceptional risk, the Risk Manager may require additional insurance above the University’s standard requirements. Examples include but are not limited to large construction projects, environmental-related risks, and transportation companies. In addition to the above, the University also requires that COI’s issued under this procedure include the following: Additional Insured: Florida Institute of Technology is to be named as Additional Insured under the Commercial General Liability, Automobile Liability and any Umbrella/Excess Liability policies. Waiver of Subrogation Clause: To be included in favor of Florida Institute of Technology Notice of Cancellation Clause: The COI must indicate that it is the responsibility of the insurance carrier to provide the University with 30 days’ notice prior to cancellation or expiration of the insured’s policy. Niebuhr Chief Financial Officer Date: April 11, 2014 Subject: Vendor Certificates of Insurance In an effort to reduce the University’s insurance risk and ultimately our costs, we are asking for your help in obtaining a certificate of insurance from vendors and/or contractors performing work on campus. This is an existing University policy to which we are paying closer attention. The attached chart is a guideline on the types of vendors that need to provide a certificate. The requirement for some vendors may be waived upon a specific request to my office. The Certificate of Insurance must name Florida Institute of Technology as an “additional insured.” The minimum coverage should be $1,000,000 per occurrence and $2,000,000 annual aggregate. The certificate should be forwarded to Purchasing before the contracted work can begin. The following information should be listed on the Certificate: 1. Full Name of Insured 2. Name of Insurance Company providing coverage 3. Type of insurance (e.g. Commercial General Liability, Worker’s Compensation, Automobile Liability) and dollar coverage 4. Policy number and effective dates 5. Date and description of operations-department must be listed 6.
