Subject: Certificate of Insurance Requirements & Request
Dear Sir/Madam,
Procurement Services requires that a valid certificate of insurance copy (COI) be on file for any vendor providing service on
University property, privately owned property where the University is allowed use, or for other services, which are considered high
risk. Before a purchase order can be issued to your firm, a valid insurance certificate must be submitted. The certificate must be
signed by an authorized agent of an insurer licensed to do business in Florida and must be resubmitted annually if it is for an ongoing
contract.
If coverage is valid for any company name or address other than that which would be listed on the purchase order, please furnish that
information along with the certificate of insurance. The certificate must show current coverage in the required amounts cited below.
Listed below are the minimum required coverage amounts and specific additional insured language.
Commercial General Liability (CGL):

Commercial General Liability should include Bodily Injury and Property Damage Liability, Contractors Liability, Contractual
Liability, Product Liability and Completed Operations. The COI should also indicate whether the General Liability policy is written
on a claims-made or occurrence basis.
Umbrella/Excess Liability: The above limits may be reached by means of an umbrella or excess liability policy.
Other: If a product, service, or activity is considered to present an unusual or exceptional risk, the Risk Manager may require
additional insurance above the University’s standard requirements. Examples include but are not limited to large construction
projects, environmental-related risks, and transportation companies.
In addition to the above, the University also requires that COIs issued under this procedure include the following:
Additional Insured: Florida Institute of Technology is to be named as Additional Insured under the Commercial General Liability,
Automobile Liability and any Umbrella/Excess Liability policies.
Waiver of Subrogation Clause: To be included in favor of Florida Institute of Technology
Notice of Cancellation Clause: The COI must indicate that it is the responsibility of the insurance carrier to provide the University
with 30 days’ notice prior to cancellation or expiration of the insured’s policy.
Please have your office or your insurance agent fax a valid COI copy to our Purchasing office at 321.674.7509 or email a copy at
purchasing@fit.edu. Nevertheless, the Purchasing Office cannot issue the purchase order to your firm without a valid COI on file. If
