beginning their field education experience for the first time.

Complete and Submit a MSW Field Education Application
Complete and Submit a FERPA Disclosure Form
Complete and Submit a Field Manual/Code of Ethics Acknowledgement
Review Agency Roster and identify fields of practice interested
Complete and Submit Field Agency Prior to Choice Form
Begin contacting prospective field agencies to set-up interviews
Complete Interview process (a minimum of two interviews per student)
Complete & Submit Field Agency After Interview Choice Form
Complete & Submit Field-at-Place-of-Employment Proposal (FAPE), if applicable
Complete & Submit Agency Acceptance Form(s)


Field Office should have received Agency Acceptance Forms by Friday, January 18, 2013.
Final list of students & field assignments posted on the Field Education Department’s bulletin board
(411K, Polsky Building), Lakewood Campus (location TBD), and e-mailed to all prospective students by
Friday, January 25, 2013.
