This template should be completed and submitted to both Professional Bodies by higher education providers seeking 5-yearly
reaccreditation of their programs. Evidence documents should accompany the completed template. Evidence associated with
Standards 5-7 is only required where there have been significant changes since the program’s last professional accreditation/
reaccreditation review. Electronic submissions are preferred and need to be addressed, along with the supporting documentation, to
both professional bodies via: accreditation@cpaaustralia.com.au and accreditation@charteredaccountantsanz.com
