This template should be completed and submitted to CPA Australia by higher education providers seeking 5-yearly reaccreditation of
their programs. Evidence documents should accompany the completed template. Evidence associated with Standards 5-7 is only
required where there have been significant changes since the program’s last professional accreditation/ reaccreditation review.
