View: View Name = Existing view; Column Name = Proposed name for new column, and name of the source form,
region, and field; Specifications/Comments = short description of column.
Join: View Name = Row 1, Name of 1st view to be joined - Row 2, Name of 2nd view to be joined; Column Name = Row 1,
Name of first column to be joined - Row 2, Name of second column to be joined; Specifications/Comments= Row 1,
Master table (if known) – Row 2, Detail table (if known).
LOV: View Name = Name of the requested LOV to be added, Column Name = Name of the requested Column to be
