The CD appendices are organized into folders by chapter, and numbered as they are
referenced in the text of each chapter. Some appendices contain text, some contain tables, and
others contain figures. Combinations of these file types may also be found in a single appendix.
Text Appendices: All text appendices are presented in two formats: as Microsoft Word
documents (DOC) and as Rich Text Format (RTF) documents for use with other word processing
applications. If the document will not open by simply double-clicking on it, try running the word
processing application that you normally use first, and then use the OPEN command under the
FILE menu to open the document.
Table Appendices: Tables are presented either in Excel format (XLS) or as Microsoft
Word tables (DOC). All Excel tables are also presented as tab delimited text for importation into
other spreadsheets or statistical software. Microsoft Word tables are also presented as Rich Text
Format (RTF) documents for use with other word processing applications.
Figure Appendices: Most figures are presented as both TIFF and JPEG images, or as
embedded figures in a Microsoft Word document (DOC) and a Rich Text Format (RTF) document.
The RTF documents can be opened by other word processing applications. All JPEG images were
derived from the TIFF images excepting the images in Chapter 20, for which the reverse translation
holds. All derived JPEG images were produced at the maximum quality and file size.
