Tabs 2-6 are based on MBF and SBF infants who were issued full
packages of powdered contract formula for the specified participation
month. Tabs 2-5 provide Agency and Clinic counts and percentages of
infants by the number of cans of formula issued and age as of the Printed
First Date to Use (PFDTU) while tab 6 lists MBF infants, by Clinic ID,
who received more than 7 cans and SBF infants who received more than

formula issued, the staff person who issued the benefits, and the infant’s
