Medical & Dental School LSU Committee Application for 2014 Entering Class These pages comprise the College of Basic Sciences LSU PMED/PDENT Review Committee application packet. Students wishing to use the Premedical/Predental Review Committee for the 2014 class must attend one of the mandatory information meetings. Registration Cards are distributed ONLY at the information meetings. After attending one of the meetings, you should complete the required forms and carefully follow the guidelines contained within. Letter of Evaluation Forms are also available online. In addition to this packet, they must be submitted by the appropriate deadlines. CORE REQUIREMENTS—Complete the minimum core courses, hours and/or degree as stated in the Medical School Admission Requirements (MSAR) or ADEA Official Guide to Dental Schools for each professional school to which you apply. The LSU Medical and Dental Schools typically only accept Louisiana residents. The LSU Medical School in New Orleans will accept up to 40 non-Louisiana residents. Students interested in applying out-of-state or students who are out-of-state residents should pay particular attention to admission requirements concerning residency. 2. MCAT/DAT/OAT—The MCAT, DAT, and OAT are standardized examinations required for admission to professional school. See calendar for deadline dates. 3. FORMAL APPLICATION TO PROFESSIONAL SCHOOL MEDICAL SCHOOL—Applicants to medical school must complete a formal standardized application for admission. Allopathic Medical (MD) schools participate in AMCAS (American Medical College Application Service: http://www.aamc.org/audienceamcas.htm ). Osteopathic Medical (DO) Schools participate in AACOMAS (American Association of Colleges of Osteopathic Medical Schools Application Service: http://aacomas.aacom.org). These standardized applications are generally available around the beginning of May. Students applying to medical schools who do not participate in AMCAS or AACOMAS must contact these schools directly to request an application. DENTAL SCHOOL—Applicants to dental school must complete a formal application for admission. The LSU Dental School in New Orleans (http://www.lsusd.lsuhsc.edu, 504-941-8124) is now participating in the national application service (AADSAS). AADSAS (American Association of Dental School Application Service) is available through the American Dental Education Association website (http://portal.aadsasweb.org ) beginning around May 15, 2013. TEXAS MEDICAL AND DENTAL SCHOOL APPLICATION SERVICES (TMDSAS)-TMDSAS is a centralized service for applicants to the public medical and dental schools in Texas. Students complete an application through TMDSAS. TMDSAS verifies applications and forwards to the individual schools to which the student is applying. TMDSAS is accessed at http://www.utsystem.edu/TMDSAS /. Baylor, which is a private school, does not participate in TMDSAS. Medical applicants to Baylor apply using AMCAS. Baylor dental applicants should contact the Baylor School of Dentistry directly for application information (www.tambcd.edu). OPTOMETRY/PODIATRY APPLICATION SERVICES—OPTOMCAS is the centralized application for optometry schools as is accessed at: http://www.optomcas.org/. The Podiatry application service is AACPMAS and is accessed at: http://www.e-aacpmas.org/ . 4. LETTERS OF EVALUATION—Applicants to medical and dental school will be required to submit letters of evaluation. Professional schools strongly encourage applicants to use the LSU Premedical/Predental Review Committee; however it is NOT required. EARLY DECISION PROGRAM (EDP) Many medical schools offer an Early Decision Program (EDP). Check the Medical School Admissions Requirements (MSAR) to determine if individual medical schools offer an EDP and to obtain deadline dates. The EDP school application deadline is August 1, 2013. Medical schools notify EDP applicants of their decisions by October 1, 2013. In order to guarantee that your committee evaluation packet will be submitted by the deadline dates, students should have their LSU PreMedical/PreDental Review Committee packet, essay, resume, in by April 15, 2013, should plan to take the MCAT no later than April 27, 2013, and their faculty evaluation letters submitted no later than May 1, 2013. While the committee may be able to submit your packet by the deadline if you do not meet these deadlines, it is NOT guaranteed. After receiving their MCAT scores, students may opt out of EDP by notifying the College of Basic Sciences Dean’s Office in writing. They may then request that their committee packet be sent to additional schools. The decision to apply to an EDP program should be made carefully. According to the American Association of Medical Colleges, students accepted through EDP show “extraordinary credentials for admission.” Students considering an EDP application should contact the admissions office at the medical school they are considering for information on criteria for acceptance. EDP programs are regulated by AAMC and applicants must agree to the following: •to not apply through EDP if they have already submitted an initial or secondary application to any M.D. program. •to apply to only one AMCAS medical school through the EDP. •to not submit additional applications until notified of non-acceptance in EDP or until the October 1 EDP notification deadline has passed. •to attend the school if offered an EDP acceptance. All EDP applicants accepted by a medical school must adhere to these regulations; any violation will result in an investigation by AMCAS. Medical Schools agree to: •notify each applicant of an admission decision by October 1. •defer each EDP applicant to the regular applicant pool as soon as an EDP decision is made. Some advantages of EDP: •informed early of admission decision •demonstrates a commitment to a particular school •can save money in application fees Some disadvantages of EDP: •limits opportunities •late application to other schools if denied EDP admission IMPORTANT DATES IN 2013 FOR 2014 MEDICAL APPLICANTS January 24 & 26, 2013 March 23, 2013 MCAT examination dates. (http://www.ada.org/dentpin.aspx ). This will be your Uunique personal identifier for registering for the DAT and for the AADSAS application. Register to take the Dental Admission Test (DAT). (http://www.ada.org/dat.aspx ). The (DAT) Dental Admission Test may be taken at anytime. It is scheduled through the Dental Admission Testing Program and taken at a Prometric Testing Center in your area. Students wanting to be reviewed by the committee must provide official DAT scores to the College of Basic Sciences by 4:30 pm, June 1, 2013. Students wanting to repeat the test, must wait 90 days before attempting to retake it. Please note that the registration timeline requires 2 to 3 weeks. Register early! Note Dental Schools highly recommend that you use the Premed/Predent Review Committee at your university. LOUISIANA STATE UNIVERSITY MEDICAL/DENTAL COMMITTEE PROCEDURES 1. FILES - DUE IN THE DEAN’S OFFICE (351 HATCHER) BY 4:30 pm, on deadline dates listed below. Files are reviewed on a first completed basis Students must have completed a registration form before a file will be started. A completed file consists of 1) MCAT/DAT scores, 2) Personal Information Form, 3)Signed Dean of Student’s Release of Information Form, 4) Committee Resume Form, 5) a copy of your AMCAS/Dental Essay, and 6) at least 2 Faculty Letters of Evaluation from courses you have earned credit for or in whose lab you have worked on this campus. In order to use the LSU Premedical/Predental Committee, a student must have been enrolled full time during the last 2 semesters at LSU. Only students with an overall and science g.p.a. of 3.0 or better will be allowed to use the Premedical/Predental Committee unless you have earned a 3.5 or better with full-time loads in your last 3-4 semesters. If you are a traditional student who has earned most of your credit at LSU, it would be to your advantage to use the Premedical/Predental Committee. Applicants without the above-stated requirements may still apply to medical/dental school on their own. You are to request that your letters of evaluation be sent directly to the schools to which you are applying. This should be done after you submit your AMCAS application. It is always best to send your AMCAS application in as soon as possible. If you have any questions regarding your status, contact a premedical/predental advisor in 351 Hatcher Hall. MCAT/DAT Test - Students are required to take the MCAT no later than May30, 2013 in order to use the committee for the 2014 entering class. Personal Information Form (PIF) with Photo –All sections must be completed and turned in to the BASC Dean’s Office in 351 Hatcher Hall by May 1, 2013 at 4:30 pm (April 15 if Early Decision Applicant). The application must be typed. ____ 5. Signed copy of the “Authorization to Release Information” form from the LSU Student Advocacy & Accountability Office by May 1, 2013 at 4:30 pm. _____ 6. Medical/Dental Essay - The personal comments in your AMCAS/AADSA/AACOMAS Essay should include personal insights regarding your desire to attend medical or dental school. Avoid, if possible, repeating information found elsewhere on the application. Because many admissions committees place significant weight on this section, consider and construct your remarks carefully. Also, use this space to continue, explain, or elaborate on answers given elsewhere in your application. It is strongly recommended that you complete the AMCAS application as soon as possible. You will submit a copy of this essay to the LSU Premedical/Predental Review Committee. Please do not exceed the character limits of the application services. _____ 7. Letters of Evaluation - You must have a minimum of two letters of evaluation from faculty members or research mentors at LSU. Science majors are advised to request evaluations from at least two science faculty members. Letters from teaching assistants or lab instructors are not acceptable. Evaluators should use the Committee evaluation forms provided. You may request additional non-faculty evaluations; this evaluator should use his or her own personal/business letterhead. There should not be more than 4 total evaluations submitted. Completed forms must be turned in directly to this office by your evaluators. (Students are never to turn in completed letters of evaluation on their own.) The letter packet is sent directly to the Admission Offices of the schools which do not participate in those services (including the LSU Medical School in Shreveport). Any changes in the list of medical/dental schools listed on your Personal Information Form must be submitted in writing. Note: It is your responsibility to keep current on the status of your file. LOUISIANA STATE UNIVERSITY Authorization to Release Information The Family Educational Rights and Privacy Act (or FERPA, 20 U.S.C. § 1232g; 34 CFR Part 99) is a Federal law that protects the privacy of student educational records. At age 18, FERPA rights transfer from the guardian to the student. As a result, there is limited educational information that the Student Advocacy & Accountability office can discuss with anyone other than the student directly. If you have previously used the Premedical/Predental Committee, you may request a copy of your Committee evaluation be sent to the medical/dental schools to which you are re-applying. Only your composite committee evaluation will be sent; faculty letters cannot be sent again by this office. The request form to release a copy of your committee evaluation can be picked up in the BASC Student Services Office, 351 Hatcher Hall. This can only be done the following year after your file is reviewed by the Premedical/Predental Committee. Under no circumstances will the re-applicant be reviewed again by the Committee. There is a $10 charge to have your composite evaluation resent. This must be paid by check or money order only and made payable to the College of Basic Sciences. 2. MCAT registration is available on line (http://www.aamc.org). Check the website for dates. The DAT may be taken at anytime. It is scheduled through the Dental Admission Testing Program and Prometric Registration Center. Registration is available online at http://www.adea.org. Students wanting to repeat the DAT, must wait 90 days before attempting to retake it. 3. It is very important to get your application in early. Earliest submission for AMCAS and AADSAS is approximately June 1, 2013. The AMCAS application is expected to be available via the AAMC web site on approximately May 15, 2013. The AADSAS application is usually available the first week of May. Check the web site for availability (http://www.adea.org). 4. As a reapplicant, it is in your best interest to solicit new faculty letters to be submitted directly to the schools to which you are applying. These letters of evaluation should be written on departmental/individual letterhead and not on the Committee faculty evaluation form. 5. You should also know that it is generally considered a big “NO-NO” to enroll in a graduate program that you do “NOT” intend to complete. 6. It is usually best to schedule an appointment at the medical or dental school in the summer to discuss how you can improve your application for the following year. You should attempt to obtain new letters of evaluation and to update your application to include new information that was not a part of the original application.
