Summary Statement: State the major function(s) of the position and its role in the university. The Coordinator, Early Head Start Health, assists in the planning, implementation and evaluation of Early Head Start (EHS) services under supervision of the Director and in collaboration with the Early Head Start Program Coordinator, Infant/toddler Care Coordinator, the Family & Community Partnership Coordinator, the CFRC Training Coordinator, and parents. The major function of the EHS Health Coordinator is to promote collaboration among Early Head Start families, staff and community health professionals in order to identify all child health and developmental concerns, and to link children and families to an ongoing source of continuous, accessible care to meet their basic health care needs. Individual provides ongoing monitoring and evaluation activities to ensure program compliance with all Head Start Performance Standards, Federal rules and regulations, and applicable state and local regulations. 2. List the major responsibilities, including percentage of time devoted to each. Provide enough detail to enable a person outside the department to understand the job (percentage first with heading and then bulleted information). 20% - Child Health Evaluation Determine the health status of each child enrolled in the Early Head Start program in collaboration with parents and as quickly as possible but no later than 90 calendar days from the child’s entry into the program to: o Make a determination as to whether or not each child has a medical home o If a child does not have a medical home, assist parents in locating one Determine whether a child is up-to-date on well child exams (using the EPSDT schedule) o For children who are not up-to-date; assist parents in making the necessary arrangements to bring the child up-to-date. o For children who are up-to-date, ensure that they continue to follow the recommended schedule. 30% - Health Care Tracking Track the provision of health care services using the ChildPlus database o Enter health data o Monitor health services using ChildPlus reports 20% - Services to Pregnant Women Assist pregnant women to access comprehensive prenatal and postpartum care, through referrals, immediately after enrollment in the program Support the home visitors in the provision of prenatal education on fetal development (including risks from smoking and alcohol), labor and delivery, postpartum recovery (including maternal depression), and the benefits of breast feeding Visit each newborn within 2 weeks of the child’s birth to ensure the well-being of both the mother and the child 10% - Patient Monitoring Obtain or arrange for further diagnostic testing, examination, and treatment by an appropriate licensed or certified professional for each child with an observable, known or suspected health or developmental problem: o Review, evaluate, and interpret health records and other vital health service data Position Description – Coordinator, Early Head Start Health o o o o Page 3 Develop and implement a follow-up plan for any condition identified during a routine well child examination Maintain a system of ongoing communication with the parents of children with identified health needs to facilitate the implementation of the follow-up plan Provide assistance to the parents, as needed, to enable them to learn how to obtain any prescribed medications, aids or equipment for medical and dental conditions Assist with the Individualized Family Service Plan (IFSP) 10% - Parent Consultation Support the involvement of parents in the provision of health services Consult with parents when child health or developmental problems are suspected or identified Help parents understand the use of and rationale for all health and developmental procedures Ensure that the results of diagnostic and treatment procedures are shared with and understood by the parents Talk with parents about how to familiarize their children in a developmentally appropriate way and in advance about all of the procedures they will receive while enrolled in the program Assist parents to enroll and participate in a system of ongoing family health care and encourage parents to be active partners in their child’s health care process 10% - Administrative Management Work with the management team and/or other program staff Attend EHS meetings as requested Assist the Project Coordinator in the development, review and revision of plans to implement the requirements of the Performance Standards (1304.20 Child Health and Developmental Services) Provide staff and/or parent training and development, including on-site consultation, coaching and mentoring Work with the disabilities coordinator and parents in the IFSP process, as appropriate Participate in the annual program self assessment Participate in transition planning for children, as appropriate 3.
