KINDERGARTEN INFORMATION FROM THE TJS SCHOOL NURSE Colette Crescas, RN, MPA, CSN 973-593-3182 ext. 3184 COMMUNICATION WITH THE SCHOOL NURSE Roll Call: Please call the Roll Call line when your child is going to be absent or late. I’ll be sending home “Health Office Alerts” letting you know if a student in your child’s class has a contagious illness, infection or infestation, such as strep throat, conjunctivitis, head lice, etc. IMMUNIZATIONS: New Jersey State law mandates that all immunizations must be received prior to the admittance of school. The following immunizations are required: Three doses of HEP B (Hepatitis B) administered at appropriate intervals. Four doses of DTP (Diphtheria, Tetanus, Pertussis) PLUS One booster after 4th birthday or any five. Three doses of IVP (Polio) PLUS One booster after 4th birthday or any four. Two doses of MMR (Measles, Mumps and Rubella) administered on or after 12 months of age. One dose of Varivax administered after 12 months of age or documentation of the chickenpox disease (month and year). PHYSICAL EXAMS: An updated physical dated within the past year must be submitted to the school nurse before school begins. If your child’s birthday falls in September or October, please contact the nurse. MEDICATION ADMINISTRATION IN SCHOOL: If your child requires prescription or non-prescription medication please obtain the required forms from the district website under the Health Services tab. Please have the forms completed by your child’s doctor then return to me prior to the start of school. Please also provide the medication in its’ original container at that time. Parents/guardians are required to transport all medication directly to the school nurse for the child. Please do not send medication of any kind in your child’s backpack or on the school bus. ILLNESSES, RASHES OR FEVERS: Parents should refrain from sending their child to school with illnesses such as colds, fevers, viruses or stomach disorders. If your child has been dismissed from school with a fever (100 degrees or higher) please do not send your child back to school until they are fever-free for 24 hours or more without the use of fever-reducing medication. If your child is sent home with contagious symptoms (rash, diarrhea, vomiting) we respectfully ask that you to keep them home from school for 24 hours or until your child is symptom free. ALLERGIES: We have a number of children entering K and 1st Grade with Life Threatening Food Allergies (LTFA). We require a physician’s statement of prior anaphylactic reaction and the date, written orders for the administration of an epi-pen and/or Benadryl. Please also complete an Individual Health Plan for your child so that we understand the allergy and know how to respond Thank you in advance for helping us to provide a safe and healthy learning environment for the coming year!!
