Physical Recovery Emergency Planning (PREP)
POST EVENT RESPONSE PROCESS
Purpose Statement:
Post event physical recovery emergency response will involve assessing the campus built-environment
after an incident has occurred. Assessment should include any follow-up actions required to address
losses and risk mitigation resulting from events.
Resources:
A Physical Recovery Emergency Planning (PREP) listserv (PLANTOPS-PREP@LISTSERV.UH.EDU) and
SharePoint site (https://share.uh.edu/plantops/PREP/SitePages/Home.aspx) have been created to
facilitate team communications and to house all PREP Emergency Reports and post event tracking data.
The SharePoint site will be accessible to key university stakeholders as well as PREP team members for
event notices and updates as they are posted. This will reduce unnecessary paperwork, redundancy,
and confusion and ensure that real-time information is available when needed. For responses that
require collaboration, the command center for physical recovery emergency efforts will be located in
GSB122C Conference Room. In the event that the incident has impacted campus network access,
updates will be posted in this location. For individual responses where network connectivity is not
available, the areas will post in their identified command center locations as noted in their business
continuity plans.
Process:
Updated and real time notices are posted throughout the process as they are made available by the
PREP Physical Response Incident Lead (PRIL). The PRIL’s physical recovery response report should
provide the following information: 1) what happened; 2) when the event happened; 3) where the event
happened; 4) who did the event happen to; 5) details as to how the event happened; and 6) why the
event happened. To report this information, it will be important to utilize the Emergency Reporting
Form and the Post-Event Communications Report Form. Physical area owners – i.e. areas with majority
involvement in the incident – are responsible for response coordination, notices, emergency form
completion, recovery management and post event reporting. The forms may be accessed at the PREP
website, http://uh.edu/plantops/about-us/committees/PREP/index.php. The correct chronology for a
post event physical response is provided below:
Incident Occurs:
1. Incident is reported to Information Technology Availability Center (ITAC), Facilities Service
Center (FSC) or Police Department (PD).
2. ITAC, FSC, or PD identifies 1) the incident type and 2) the Physical Recovery Incident Lead (PRIL).
3. Call is dispatched to appropriate PRIL (Plant, Environmental Health and Safety, Fire Marshal,
Information Technology, Police Department, Research, Facilities Planning and Construction,

Real Estate, etc.) Depending on the nature and severity of the incident, the UH Public Safety
Emergency Management Bureau will also be contacted.
PRIL will notify dispatch of estimated time of arrival, if possible -- notification is sent to all
impacted parties (including the escalated facilities emergency call tree as applicable).
PRIL arrives on site and completes an initial assessment.
Additional resources will be called in as needed, including emergency response contractor(s).
Recovery begins immediately led by the physical area(s) with the greatest impact.
Initial assessment summary will be reported to PRIL’s area management by phone or applicable
dispatch area.
When incident DOES affect / impact other areas –
9. PRIL’s PREP member notifies the PREP committee and appropriate AVP via email or phone,
depending on incident severity, and as applicable.
10. PREP PRIL posts incident notices and updates throughout the recovery to the PREP SharePoint
site -- (https://share.uh.edu/plantops/PREP/SitePages/Home.aspx).
11. A remediation meeting or conference call is scheduled – PREP is alerted -- representatives from
any area affected by the incident should attend.
12. Representatives of affected areas will discuss damage assessment, impact to operational and
business continuity, and identify next steps.
13. PRIL completes emergency reporting form located on the PREP SharePoint site.
14. Representatives from impacted areas will meet on the site and regrouping
meetings/conferences are scheduled as applicable.
15. PRIL will post continuous updates throughout the process as they are available. Information
shall be validated and then organized and reported (and named in the following sequence) :
a. Date
b. Building Number
c. Building Name
d. Incident Name
e. Incident description.
16. PREP team or affected area completes post-event communication report on day 2. Form
located at PREP SharePoint site (https://share.uh.edu/plantops/PREP/SitePages/Home.aspx).
17. A communication notice will be sent to applicable AVP that the final report has been posted.
Based on the severity of the incident, it may be necessary to send a communication notice to
the Emergency Management Team (EMT) that the final report has been posted.

Major Events
The physical recovery emergency planning and response process will be the same as noted in this
document for minor and major events. However, for major events where the university’s EOC has been
activated, the PREP team members will filter their unified reports and updates through the EOC as
required by the NIMS process and will not use the SharePoint site for major response communications.
