Dear MCLIMS Parents,
The Mandarin Chinese Language Immersion Magnet School is proud to present a quality After School Program in collaboration with childrens Initiatives After School Programs! This program is designed to provide the needs of the school community in an after school setting. The quality of our program will thrive with the cooperation of the school community (i.e. administration, staff, parents, children). Please read the following information thoroughly:

REGISTRATION:
• Registrations are processed on a first come first serve basis and are considered complete ONLY upon receipt of
a signed contract, registration form, and payment. Please make all checks or money orders payable to: MCLIMS
• Beginning December 3 registration forms are to be dropped off to the ASP Office so that your registration is accepted on a first come first serve basis. Registrations submitted to any other location (i.e. classroom teacher,
front school office) will be collected at the end of each day, at which time, registrations will be processed. Keep in mind that registrations submitted to the After School Office will take priority.
• An itemized statement will be mailed to your home via U.S. mail by December 28 listing your child’s schedule if your registration is received by December 22.
• During the first week of school statements for registrations received after December 22 will be available for pick up in the ASP office beginning January 5.
PAYMENT :
You may select one of the 2 Payment Options (Bi-weekly or 4 Monthly Payments) as listed on the front cover page. All due dates are listed on Front Cover Page, Page 2 and the Contract Document.

OUTSTANDING BALANCES:
Registration cannot be accepted from anyone who has an outstanding balance from any previous semester.
Registrations submitted must include a separate check for their outstanding balance in addition to their first payment.
In addition, any accounts with outstanding balances remaining after December 4 will be rolled over to the Spring
2016 semester and will be assessed a $25.00 rollover fee . Also, any Spring 2016 outstanding balances greater than $100 remaining after April 8 will be rolled over to the Fall 2016 with a $25.00 rollover fee.
Failure to meet your financial obligations may result in suspension of services to you .

• If you enrolled in an enrichment class and it is canceled due to lack of enrollment, a full refund/credit for that class will be issued. In the event of teacher illness, a substitute will be provided OR students will be contacted as early as possible to inform them of a class cancellation. Make up classes will be scheduled and you will be notified by the Coordinator.
For Half Time & Enrichment Only Students: If your child is not picked up immediately upon dismissal of an enrichment class or by 5:00, which is the end time for Half Time Ext Day, you will be assessed an Occasional Ext Day
Card of 5 Units at a cost of $50.00 which will provide supervision until 6:00 PM. Anyone picked up after 6:00PM will be assessed a fee of $2.00/minute

By signing below, I understand the contents of this registration packet and agree to the following for the Spring
Semester of 2016. Disregard of this contract and/or the contents in the registration packet will result in immediate dismissal from MCLIMS ASP.

TIMELINES: I have received a copy of the timeline (front cover page) with all important dates, including registration dates, payment due dates, and the dates that Extended Day and Enrichment Classes begin and end.

CLASS TIMES: I understand the times of Extended Day Options, Enrichment Classes, and Homework according to the Extended Day Options Form and the Master Schedule of Enrichment Classes provided in this registration packet.

ADD/DROP: Only 2 changes are permitted during Add/Drop period (until January 22). No class changes will be made and no refunds or credits will be issued after January 22.

PRORATION OF CLASSES: ANY class attended during the weeks of January 11-22 will be prorated should you drop classes. You are responsible for all costs of any classes not dropped after January 22. After January 22, your bill will be prorated ONLY if your child is required to attend Tutoring Classes by his/her school teacher OR if your child withdraws from MCLIMS. NOTE: NO refunds OR credits will be assessed in the event of HISD canceling after-school activities for inclement weather or any other extenuating circumstances .

OCCASIONAL DAYS:
• If I purchase sets of Occasional Days I understand the full amount for each set is due at the time I sign up.

If I purchase sets of Occasional Days and they run out, another 5-Unit Occasional Card will be automatically billed to my account.

SIGNING CHILDREN OUT: Either myself or other individuals authorized to pick up my child must sign my child out from the after school program at the time of pick-up. Failure to sign the student out from the school office will be regarded as an “Incident” and may result in suspension from the program for the remaining school year .

ENRICHMENT CLASSES AFTER 4:25 ONLY : I must enroll my child in either a 3:55 PM enrichment class,
Bridge until the start time of an Enrichment Class beginning after 4:25 or Occasional Extended Day in order for my child to participate in an Enrichment Class beginning after 4:25 PM.

CHANGES IN HOW A STUDENT GETS HOME : I MUST notify the ASP Coordinator in writing (phone calls not accepted) via fax or written note dropped off in the ASP Office or in the ASP Box (located in front school office)

LATE PICK UP:

If my child is picked up after 6:00PM I will be assessed a late pickup fee of $2.00/minute.
•
When I am late picking up my child from Half-Time or Enrichment I will be automatically issued a 5-Unit
Occasional Card which will provide supervision until 6:00 PM. 1 Unit from this card will be used for the late pick up and I understand that I will have 4 Units remaining.
• After the 5th late pick-up I may be asked to make other arrangements for After School Child Care .
• The official time for our after school program will be gauged by the clock located in Pick UP Location in the school.
• Please Note: If late pick-up fees are not paid within 24 hours, the fee is automatically doubled. (Check or money order only)
BEHAVIOR EXPECTATIONS: The HISD Code of Student Conduct will serve as the basis for all discipline expectations to ensure the safety of all students and staff members. Parents will be notified immediately if their child has violated any portion of the school’s previously communicated Code of Student Conduct. Services will be suspended should disrespectful or unsafe behavior be directed towards a student or staff member, either by a parent of a student or a student enrolled in the program. I understand that it is my responsibility for reading and knowing the expectations of the HISD Code of Conduct. I also understand that the program reserves the right to deny services on a permanent basis if behavior expectations are not being met.

SPECIAL NEEDS: We do not provide staff trained to deal with special needs. Please contact the school administration with any questions.
14.
TECHNOLOGY DEVICES: No technology devices may be used in the after school program by any student in grades K-5. All such devices must be kept in the student’s backpack.
15. All information published in this registration packet applies to this contract. In addition, I understand and agree to adhere to any guidelines amended to this contract and that I will be notified in writing by the after school program.
16. I understand that my email address and/or phone number(s) may be given to the Provider of any Enrichment class in which I have enrolled my child should the Provider find it necessary to contact me.

Description of Extended Day Options
Friday $72.00
FULL or HALF TIME OPTION #1: For Full Time you may enroll in an Enrichment Class everyday (Mon-Fri), either 1 st
hour or
2 nd
hour. For Half Time you MUST enroll in an Enrichment Class ONLY the 1 st
hour everyday. The cost of each class is in addition to the cost of Extended Day.
Homework Assistance will be provided during the hour your child is not in Enrichment.
Also, Early Dismissal Days are included.
FULL or HALF TIME OPTION #2: You must enroll in either 2, 3 or no more than 4 Enrichment Classes on any day/time. For
Full Time you may enroll in an Enrichment Class everyday (Mon-Fri), either 1 st
hour or 2 nd
hour. For Half Time you MUST enroll in an Enrichment Class ONLY the 1 st
hour . The cost of each class is in addition to the cost of Extended Day.
Homework Assistance will be provided during the hour your child is not in Enrichment. Also, Early Dismissal Days are included.
FULL or HALF TIME OPTION #3: This option provides you with Extended Day Activities and Homework Assistance provided daily by the ASP Staff. This option is open ONLY to students who enroll in ONE or NO Enrichment Class. For Half Time you
MUST enroll in an Enrichment Class ONLY the 1 st
hour . Homework will always be a priority. The Extended Day Activities consist of: Art; Chinese Games; Board Games; Story Hour; Crafts, etc. Also, Early Dismissal Days are included.
OCCASIONAL Units – Full Time (FT) and Half Time (HT) Units are sold in sets of 5. This is designed for you to send your child to Ext Day on an average of 2-3 days per week. Each time your child reports to the After School Program, 1 Unit will be used. FT Occ provides services from 3:30-6:00 and HT Occ provides services from 3:30-5:00. There is a 1 time Supply Fee of
$25.00. Also, if your child is enrolled in Half Time Ext Day and you do not pick up your child by 5:00, an AUTO Purchase of 5
HT Units will be assessed to your account which will provide services until 6:00 PM. Anyone picking up after 6:00 PM will be assessed a late pick up fee of $2.00/minute.
EARLY DISMISSAL – This is ONLY for those students who are NOT enrolled in Full OR Half Time Extended Day everyday. If you are enrolled in Occasional Ext Day, and you need Early Dismissal Occasionally, you may use one of your FT Units from
12:30-3:30. If your child remains in after school after 3:30 PM, a second unit will be used.
BRIDGE TO 2

CLASS - If your child is enrolled ONLY in a class that begins after 1:00 PM (Wednesday Private Lesson) or after 4:00 PM, your child must be supervised until the start of the class. Therefore, you must enroll in Bridge to 2 nd
Class.
However, you MUST pick up your child immediately at the end of the Enrichment Class Time.

Mandarin Chinese Tutoring Program by Language Kids
Class Description: Our Mandarin Chinese native-speaker teachers will focus on Mandarin
Chinese language skills development. We will tutor students and prepare them for the Mandarin
Chinese tests in February and April and help your child with Mandarin Chinese language and Math homework assignments. Our lessons will be developed around the Better Chinese curriculum used by MCLIMS and aligned with what your child is learning at school in Mandarin Chinese. This class will meet 2 times a week.
Teacher Description: Our mission is to provide immersion language acquisition programs for children in a fun and nurturing environment that fosters multiculturalism. All our teachers are native
Mandarin Chinese speakers with a minimum education of a bachelors’ degree and with experience working with children. Our founder started her first language school in Mexico City 20 years ago and has extensive experience and knowledge of teaching languages to children.
Private Homework Assistance by ASP Staff
Class Description: In this class, a staff member will meet with your child to assist his/her with the homework assignments. The Tuition Rate listed is for enrollment of 1 student. Note the cost per student should more than 1 student sign up for the same class: 1 student: $360.00; 2 students:
$180.00; 3 students: $120.00
Teacher Description: ASP Staff experienced in assisting students one on one with homework assignments. It is highly recommended that the parent meet with the staff member to address personal needs.

Contract Agreement SPRING 2016
By signing below, I understand the contents of this registration packet and agree to the following for the Spring
Semester of 2016. Disregard of this contract and/or the contents in the registration packet will result in immediate dismissal from MCLIMS ASP.
1.
I understand that I am enrolling only for SPRING 2016 .
2.
TIMELINES: I have received a copy of the timeline (front cover page) with all important dates, including registration dates, payment due dates, and the dates that Extended Day and Enrichment Classes begin and end.
(Timeline and Payment Schedule are listed on front cover.)
3.
CLASS TIMES: I understand the times of Extended Day Options, Enrichment Classes, and Homework according to the Extended Day Options Form and the Master Schedule of Enrichment Classes provided in this registration packet.
4.
PAYMENTS : (Please initial each item below on the line)

_____If I enroll in Enrichment Classes, I will pay at least of 1/4 of the total cost of classes for the Spring 2016
Semester at registration. Payments are due as follows: At Registration, Feb. 5, March 4, April 8.
_____If I enroll in Extended Day (Full/Half) I will follow the Biweekly Payment Schedule according to the Option I
have agreed upon for Spring 2016: At Registration, Jan. 8, 22; Feb. 5, 19; March 4, 24; Apr 8, 22 and May
6 . In addition, I may also opt to follow the !
payment schedule for Extended Day.
_____I will be assessed a late fee of $10.00 each time I do not pay my bill as I have agreed and indicated on
Page 6.
_____I will be assessed $35.00 for each returned check. After 2 returned checks, payment must be made by
money order payable to MCLIMS.
_____I understand that after school services may be suspended if I do not pay my bill on time. Accounts with

outstanding balances after Dec. 11 will be assessed a $25.00 Rollover Fee and your balance will be
rolled over to Spring 2016. Also, any Spring 2016 outstanding balances greater than $100 remaining
after April 8 will be rolled over to the Fall 2016 with a $25.00 rollover fee.
ADD/DROP: Only 2 changes are permitted during Add/Drop period (until January 22). No class changes will be made and no refunds or credits will be issued after January 22.
6.
PRORATION OF CLASSES: ANY class attended during the weeks of January 11-22 will be prorated should you drop classes. You are responsible for all costs of any classes not dropped after January 22. After January 22, your bill will be prorated ONLY if your child is required to attend Tutoring Classes by his/her school teacher OR if your child withdraws from MCLIMS. NOTE: NO refunds OR credits will be assessed in the event of HISD canceling after-school activities for inclement weather or any other extenuating circumstances .

OCCASIONAL DAYS:
• If I purchase sets of Occasional Days I understand the full amount for each set is due at the time I sign up.
•
If I purchase sets of Occasional Days and they run out, another 5-Unit Occasional Card will be automatically billed to my account.
8.
SIGNING CHILDREN OUT: Either myself or other individuals authorized to pick up my child must sign my child out from the after school program at the time of pick-up. Failure to sign the student out from the school office will be regarded as an “Incident” and may result in suspension from the program for the remaining school year .

ENRICHMENT CLASSES AFTER 4:25 ONLY : I must enroll my child in either a 3:55 PM enrichment class,
Bridge until the start time of an Enrichment Class beginning after 4:25 or Occasional Extended Day in order for my child to participate in an Enrichment Class beginning after 4:25 PM.

CHANGES IN HOW A STUDENT GETS HOME : I MUST notify the ASP Coordinator in writing (phone calls not accepted) via fax or written note dropped off in the ASP Office or in the ASP Box (located in front school office)
OR via email ( ciasp2@sbcglobal.net
OR mclimsafterschool@sbcglobal.net) for ANY changes in how my child gets home. Without written permission, my child will not be permitted to leave.

LATE PICK UP:
•
If my child is picked up after 6:00PM I will be assessed a late pickup fee of $2.00/minute.
•
When I am late picking up my child from Half-Time or Enrichment I will be automatically issued a 5-Unit
Occasional Card which will provide supervision until 6:00 PM. 1 Unit from this card will be used for the late pick up and I understand that I will have 4 Units remaining.
• After the 5th late pick-up I may be asked to make other arrangements for After School Child Care .
• The official time for our after school program will be gauged by the clock located in Pick UP Location in the school.
• Please Note: If late pick-up fees are not paid within 24 hours, the fee is automatically doubled. (Check or money order only)
BEHAVIOR EXPECTATIONS: The HISD Code of Student Conduct will serve as the basis for all discipline expectations to ensure the safety of all students and staff members. Parents will be notified immediately if their child has violated any portion of the school’s previously communicated Code of Student Conduct. Services will be suspended should disrespectful or unsafe behavior be directed towards a student or staff member, either by a parent of a student or a student enrolled in the program. I understand that it is my responsibility for reading and knowing the expectations of the HISD Code of Conduct. I also understand that the program reserves the right to deny services on a permanent basis if behavior expectations are not being met.
13.
SPECIAL NEEDS: We do not provide staff trained to deal with special needs. Please contact the school administration with any questions.
14.
TECHNOLOGY DEVICES: No technology devices may be used in the after school program by any student in grades K-5. All such devices must be kept in the student’s backpack.
15. All information published in this registration packet applies to this contract. In addition, I understand and agree to adhere to any guidelines amended to this contract and that I will be notified in writing by the after school program.
16. I understand that my email address and/or phone number(s) may be given to the Provider of any Enrichment class in which I have enrolled my child should the Provider find it necessary to contact me.
17.
MEDIA PERMISSION: I authorize MCLIMS ASP to include my child in photographs used for publicity.
