1. Submit this in person to a Pre-Health Professions Advisor who will verify that
your file is complete.
2. By submitting this form you are requesting to be reviewed by HPAC at the next
available meeting date.
3. The list of available HPAC review dates for the current cycle is posted; these are
the only dates available.
4. You are responsible for contacting a Pre-Health Professions Advisor after the
HPAC review to find out the results.
5. After your HPAC review, when you are ready to have us mail your letters of
recommendation you must make the request in writing. We may request that you
supply mailing addresses and/or labels for out-of-state schools. Mailout request
forms will be available on the Pre-Health Professions Advising web site.
