RE: BREAKFAST- SNACK PROGRAM
Dear Parents/Guardians;
The snack and breakfast program will begin shortly. If you have not completed and returned the form to participate in the program as of yet
please do so at your earliest convenience. If you have any questions or concerns regarding the snack and breakfast program please feel free
to contact the school.
Thank you for your ongoing support and cooperation.
This year we are once again considering a “pay for service” for those who can afford this service or who would like to see it continued to assist
those in need. Please review and complete the attached form to determine your use for your child(ren)..
I fully support the breakfast/snack nutrition program and would be able to donate $20.00 for remainder of this school year, to assist
with the continuation of this program.
I will not be participating in the program.
I support the program however I cannot commit financially at this time, but would appreciate the subsidy for my child(ren), so they
can participate in the program.
