Outcomes:
1.0 We will provide a safe learning environment that encourages risk-taking needed for
learning, where the voice of each learner is valued and heard, within a supportive
community.

6/1/2015: We continue to work collaboratively with the OPP, parish, psychologist, speech and language
pathologist, CYW, social worker and public health nurse. We have implemented the P.A. L. S program.

Building Collaborative Practices Through Inquiry
Outcomes:
2.0 We will cultivate a co-learning stance within an inquiry based approach to learning,
which will deepen understanding for all learning.

6/1/2015: The Holy Family Staff developed a co-learning stance within an inquiry based approach to learning,
which deepen our understanding of learning. All staff members were involved in a Collaborative Inquiry throughout
the year related to their students' needs. Through examining student evidence, relevant research and monitoring
student outcomes we were able to develop teaching practices that lead to student success. The staff was engaged
in professional dialogue and watched webcasts for educators about inquiry based learning.
10/14/2014: The Holy Family Staff will continue to develop a co-learning stance within an inquiry based approach

Enhancing Transitional Practices
Outcomes:
3.0 We will support the diverse learning needs of all learners, ensuring successful
transitions along the journey of learning from Early Years to Secondary School
Outcome Progress Comments
6/1/2015: This year, an area of focus was to support the diverse learners in our school community and ensure
successful and smooth transitions from Early Years to Secondary. In addition, the use of a team-approach help
deepen our understanding of all learners. We will continue to support seamless transitions between the grades as
well as the transition from Grade 8 to Grade 9 for our students. We enjoy a very positive collaboration with our
high school St. Michael's and our Grade 8 students had many opportunities to visit and participate in various
activities related to their upcoming transition to secondary school. Through such processes as Team meetings,
Transition meetings, Divisional and Cross-Divisional meetings, IPRCs and Case Conferences, we were able to
support the needs of all learners as they move through the grades.

4.0 We will implement assessment practices that will make thinking and learning visible (to
teachers, students, and parents) in order to inform instruction that enables all students to
learn.

6/1/2015: Teachers implemented assessment practices that made thinking and learning visible in order to inform
their instruction and to enable all students to learn. There has been an increase in the number of teachers who
were able to demonstrate their understanding of assessment for and as learning. Students have shared with their
teachers and peers, strategies that support their learning. Students were actively involved in the co-creation of
success criteria. Students were also aware of the expectations prior to and during learning, through the use of the
learning goal.

5.0 We will implement effective teaching practices that are responsive to the needs of all
learners, through the use of activating, developing and consolidated learning that will
incorporate 21st century global perspectives.

6/1/2015: Our professional learning focus was in the area of mathematics. It was supported through various
processes such as Collaborative Inquiry, monthly Staff Professional Development sessions, co-teaching and coplanning with our SAT, and collaboration with our Math Consultant. We focused on the implementation of the 3
part lesson in mathematics and the elements of classroom environment, developing pedagogical knowledge of
teaching math, computational fluency and the use of manipulatives.
