Informal Lesson Evaluation Form
These forms can be used when the student teaches a lesson. Please discuss the form and give a copy to the
student at the conclusion of each lesson. Remember these forms will be helpful in completing the final
evaluation. These do not need to be returned to the university professor, but should be placed in the
student’s field placement binder.
