Racketeering and Insurance Fraud
The Division of Insurance Fraud (DIF) announced the
arrest of Sandy Morales, owner of Jacksonville’s Gate
Parkway Diagnostic Center, on charges related to
organizing a complex PIP fraud scam that resulted in the
fraudulent filing of more than $100,000 in billings to at
least twenty different insurance carriers. This arrest
follows more than 180 additional arrests tied to this fraud
network that have been made since September 2012.
An investigation led by the Department’s Division of
Insurance Fraud found that Morales elaborately staged
multiple car crashes, paid participants to pose as
passengers, and then paid those participants to seek treatment at his clinic for fake
injuries. On top of operating as a clinic owner and as an organizer, the investigation
revealed that Morales recruited additional clinic owners and directed patients to their
PIP clinics in an effort to perpetuate the scam and create a larger fraud network.
To date, the clinics involved include: Indian Rehabilitation Center Inc., Arlington
Rehabilitation Services Inc., Baymeadows Injury Center LLC, Health Point Injury
Center LLC, One Touch Therapy Center, Inc., Saint Jose Injury Center, LLC, Therapy
Diagnostic Tech Medical Inc., Wellness Rehab Services Inc., Atlas Massage Therapy
Corp., and Gate Parkway Diagnostic Center. All clinics are now closed.
If convicted, Morales faces a maximum sentence of 45 years in prison. Bond was
set at $600,012.00, and Morales was booked into the Duval County Jail. The case
will be prosecuted by the office of Fourth Judicial Circuit State Attorney Angela
Corey. Assistant State Attorney Joe Licandro is lead on the case, and has been
instrumental in its success.
To date, these arrests have led to 115 convictions, along with $950,000+ in court
ordered restitution, as well as $60,000 ordered to cover investigative costs and
$50,000 in fines ordered to the Insurance Fraud Trust Fund.
Morales was previously arrested by DIF in Miami in September 2014 for organizing a
staged accident in March 2010 which resulted in $46,000 in fraudulent billings. That
case is still pending.

Watch DIF on A&E American Takedown
We work daily to fight insurance fraud in our state, and I often share
with you stories about the cases we are working and the arrests
we've made. Insurance fraud can take many forms and many of
them can be complex and hard to understand. That's why part of my
job is to educate Floridians about these scams so that you can
detect them and better protect yourself from falling prey to a
fraudster or scam artist who may try to take from you.
Advocacy efforts are important, and it's important to me to shed light
on a problem that costs all of us hard-earned money each year in the
form of higher insurance rates. Last year, nearly half of all the
insurance fraud referrals our Division of Insurance Fraud received
were about potential PIP (personal injury protection) fraud. PIP fraud
often involves the staging of car accidents and subsequent medical
treatment for phony injuries that allegedly occurred during the staged
crash. Fraud rings concoct these scams in order to submit insurance
claims on PIP benefits that are included in motor vehicle insurance
coverage.
This week, the television channel A&E brings PIP fraud to life during an episode of its newest show,
American Takedown, which goes behind the scenes of some of America's biggest criminal cases.
The episode follows the breakdown of a large-scale PIP fraud case, and allows viewers an in-depth
look at the inner workings of complex financial crime investigations.
The featured case, known as "Operation Cold Call," uncovered a network of attorneys, doctors and
clinic employees who were engaged in recruiting, paying and treating "patients" for injuries that did
not exist following staged car accidents. For more than a year, insurance fraud investigators worked
undercover, embedded into the clinics that were accepting patients who were illegally solicited. To

Tampa Chiropractor, 3 Others Arrested
The Florida Division of Insurance Fraud (DIF) announced the arrest of chiropractor Douglas Price, 65, owner
of the Florida Pain Trauma and Injury Clinic in Auburndale, and three others on charges related to the
operation of a complex PIP fraud scam. Price, along with Paulin Sanon, 45, and Sonya Janet Rivera, 48, of
Haines City, and Juliena Julien, 56, of Winter Haven, are accused of recruiting patients to visit Price’s clinic
for treatment of fake injuries following staged accidents.
Between October 2014 and March 2015, an investigation uncovered evidence that Price’s Auburndale and
Tampa clinics were engaged in patient brokering, and also that clinic staff were performing treatments that
they were not qualified or licensed to perform. Price and his associate, Sonya Rivera, offered $500 as
compensation for 15 clinic visits, and an additional $500 after 25 visits to the clinic for treatment of injuries
that did not exist. A monetary offer was also extended for the recruitment of additional individuals to attend
the clinic.
Based on their roles in the scheme, Price will be charged with three counts of patient brokering; Sonya
Rivera and Paulin Sanon, one of Price’s marketers, will both be charged with one count of patient brokering;
and Juliena Julien will be charged with the unlicensed practice of healthcare.
The National Insurance Crime Bureau, the Winter Haven, Haines City and Auburndale Police Departments,
along with the Polk County Sheriff’s Office and Bristol West Insurance Company assisted with the
investigation, and the case will be prosecuted by Attorney General Pam Bondi’s Office of the Statewide
Prosecutor.

Orlando Chiropractor Arrested With Owner
in Unlicensed PIP Clinic Scheme
The Florida Division of Insurance Fraud (DIF) arrested Dr. Troy Godsey, 53, and Renata Berriel Deazevedo,
40, for their roles in a scheme to defraud multiple insurance companies of more than $100,000.
Troy Godsey, a chiropractor licensed in Florida since 1998, established Spine Health Solutions, P.A. in
Orlando under the guise that he was the sole owner of the health care clinic. Because he is licensed as a
chiropractor, he was granted an exemption in September 2013 from clinic licensing requirements with the
Agency for Health Care Administration. This allowed Godsey and Deazevedo to operate a PIP fraud
scheme without alerting health care regulators. While paperwork may have indicated that Godsey ran the
clinic, Deazevedo managed the day-to-day activities of the business and Godsey served only as a straw
owner.
The clinic allegedly provided medical treatments to people who were injured in car accidents and would bill
various insurance companies to receive reimbursements under the “patients’” PIP auto coverage.
In total, three insurance companies were billed nearly $300,000 for the alleged treatment of 20 “patients.”
The companies paid out more than $100,000 in fraudulent claims before catching on to the scheme and
alerting the Division of Insurance Fraud.
Facing multiple felony charges including the operation of an unlicensed health care clinic and filing false and
fraudulent insurance claims, the two face more than 50 years in prison if convicted.

Glen Clasen Promoted To Lieutenant Of DIF Ft. Myers Office
Glen Clasen was promoted to Lieutenant of the Division of Insurance Fraud’s (DIF) Ft. Myers office.
Prior to joining DIF, Clasen was a member of the United States Navy aboard the USS John F Kennedy (CV-67) serving
as a Boatswains Mate / Rescue Swimmer and later, as a police officer with the Fort Myers Police Department in
uniformed patrol. Clasen began his career with DIF in 2006 as a detective assigned to the Fort Myers field office and
was promoted to lead detective in 2013. During his career he has investigated a variety of complex fraud cases, acted
as a field training officer and is currently appointed as a Special Deputized U.S. Marshal sponsored by the Federal
Bureau of Investigation. Clasen holds a Bachelor of Science degree in Criminal Justice from Florida Gulf Coast
University and became a Certified Fraud Examiner in 2009.

Two Arrested in Separate Fraud Cases Involving Same Miami PIP Clinic
The Division of Insurance Fraud (DIF) arrested two subjects in June for separate PIP-related crimes involving the same
clinic in Miami. Massage therapist Mariela Quintana, 46, was arrested and charged with insurance fraud, grand theft and
organized scheme to defraud. Quintana was an employee of D & J Rehabilitation Center, a clinic in Miami. Patients
were solicited to attend D & J Rehabilitation Center following an auto accident, and Quintana signed treatment forms for
therapy she never provided to the patients. Quintana was charged with fraudulent billing of $9,606.40 to Gainsco.
Quintana was previously arrested by DIF in April 2015 for similar fraudulent activity at the same clinic.
Noel Vazquez Lazo, 44, was arrested for organizing a staged accident in May 2013. Vazquez paid for the policy used by
the at-fault vehicle, and also paid and directed the participants to go to D & J Rehabilitation Center and Health &
Wellness Services. As a result of this staged accident, fraudulent claims in excess of $89,000 were filed against Direct
General and State Farm.
Owners and Staff of Louisville KY Chiropractic Clinics Charged in Federal Probe
Beginning in November 2013 and continuing through July 2014, five named defendants recruited unsuspecting
chiropractors to either open chiropractic clinics or to staff existing chiropractic clinics in the Louisville area, with the intent
to fraudulently bill insurance companies. The defendants are accused of scheming to fraudulently bill insurance
companies $5 million for services never performed over the eight-month-long period.
Claudia Lopez, Oskel Lezcano, Ariel Borrego-Hernandez, Sergio Betancourt, and Ledinson Chavez operated and
controlled multiple chiropractic clinics in the Louisville area including: Xpress Diagnostics Center, Inc.; Prudential
Chiropractic Medical Center, PLLC; Klondike Chiropractic Medical Center, LLC; Be Well Chiropractic Center, Corp.;
Chiropractic and Medical Center, LLC, even though the clinics were placed in various chiropractors’ names.
The clinics used Lezcano’s billing companies, Gold Hands Medical Billing Corp and Gold Hands Medical Billing B, Inc.,
to process the fraudulent billings for payment.
This case is being prosecuted by Assistant United States Attorneys Joseph Ansari and Lettricea Jefferson-Webb and is
being investigated by the Federal Bureau of Investigation (FBI), the United States Postal Inspection Service, the Internal
Revenue Service Criminal Investigation, the Louisville Metro Police Department and the National Insurance Crime
Bureau (NICB).

Seven Charged in California Vehicle Damage Scheme
The California Department of Insurance, assisted by Snohomish County Marshall's Office and federal officers, arrested
seven people in California and Washington for allegedly conspiring to defraud eight insurance companies by submitting
fraudulent auto insurance claims totaling more than $175,000.
According to detectives, the suspects were involved in an organized auto insurance fraud ring, submitting more than 70
fraudulent claims to multiple insurance companies for damage to vehicles that occurred prior to the vehicle being
insured. The suspects allegedly used multiple aliases on the insurance claims to avoid detection by insurers. The
investigation also revealed Mirage AutoCraft inspected the vehicles and was also involved in the conspiracy.

The 2015 FIFEC Conference was a success once again. This year, the 23rd Annual meeting of insurance
industry representatives, law enforcement and lawyers had 1015 attendees and law enforcement and state
prosecutors attending the 65 class sessions offered. FIFEC has become a huge production, with 38 persons
making up the Executive Board and managing 16 subcommittees.

Fraud (DIF) is attempting to locate Jose G. Casasnovas in reference to
the outstanding warrant.
Casasnovas was a participant in a staged accident that occurred in
August 2011. As a result of this staged accident, more than $91,000
in fraudulent claims were submitted to two insurance companies.
Casasnovas has yet to be found since his warrant was issued in
September 2013. His warrant has nationwide extradition.
