SPECIAL NOTE: This Request for Qualifications (RFQ) does not obligate the Minnesota State Colleges
and Universities (MnSCU) system, its Board of Trustees or the System Office to award a contract or
complete the proposed project and each reserves the right to cancel this RFQ if it is considered to be
in its best interest. Proposals must be clear and concise. Proposals that are difficult to follow or that
do not conform to the RFQ format or binding specifications may be rejected. Responding consultants
must include the required information called for in this RFQ. MnSCU reserves the right to reject a
proposal if required information is not provided or is not organized as directed. MnSCU also reserves
the right to change the evaluation criteria or any other provision in this RFQ by posting notice of the
change(s) on:

For this RFQ, written questions or communications regarding this RFQ shall be submitted via email
and shall include the name of the questioner. For this RFQ, posting on the captioned web site above
constitutes written notification to each vendor. Consultants should check the site daily and are
expected to review information on the site carefully before submitting a final proposal. Consultants
must acknowledge any addendum when submitting a proposal. Failure to acknowledge any
addendum may result in rejection of Consultant’s RFQ response.

Section I. General Information
Background
Minnesota State Colleges and Universities is the fifth-largest system of higher education in the United
States. It is comprised of 31 two-year and four-year state colleges and universities with 54 campuses
located in 47 Minnesota communities. The System serves approximately 400,000 students each year.
The Minnesota State Colleges and Universities is an independent state entity that is governed by a 15
member Board of Trustees. The law creating the system was passed by the Minnesota Legislature in
1991 and went into effect July 1, 1995. The law merged the state's community colleges, technical
colleges and state universities into one system, other than the University of Minnesota campuses. For

The MnSCU Facilities Design and Construction team is responsible for overseeing the effective and
efficient delivery of construction projects for the system in coordination with the colleges and
universities.
Nature of RFQ
The State of Minnesota, acting through its Board of Trustees of the Minnesota State Colleges and
Universities, on behalf of the system office, hereinafter referred to as “MnSCU”, is soliciting
qualifications for licensed Architects, Professional Engineers and Industrial Hygienists interested in being
added to the Specialty Consultant Master Roster. The Facilities Professional/Technical Specialty
Consultants Master Roster List shall include licensed Architects, Professional Engineers and Industrial
Hygienists for the following services:

Firms or individuals that meet the qualifications will be placed on the Specialty Consultant Master Roster
and may be contracted for services directly by MnSCU colleges/universities or the system office. To
qualify for the Specialty Consultant Master Roster as described in further detail in this RFQ, Responders
shall meet the requirements of this RFQ and submit the requested documentation. Responders who are
successfully listed on the Specialty Consultants Master Roster may be given preference to provide selected
Consultant services for future MnSCU facilities projects.
Note: The Specialty Consultants Master Roster is separate and distinct from any lists which may be
maintained by the State of Minnesota Department of Administration, including but not limited to the
Master Roster for Consultant Services or any other lists maintained by the Real Estate and Construction
Services Office, or vendor lists maintained by the Capital Planning and Project Management Division of
the University of Minnesota or any other organizations. Responders interested in providing services to
MnSCU, must therefore respond to this RFQ and comply with the process described herein. MnSCU

shall determine, in its sole discretion, if Responder meets the requirements to be placed on the Specialty
Consultant Master Roster. Firms that qualified for the Specialty Consultant Master Roster specific
categories in 2014 and wish to remain on the list must reapply. Firms that qualified for the Specialty
Consultant Master Roster last year (2015), do not have to respond to this request.
This RFQ is undertaken by MnSCU pursuant to the authority contained in provisions of Minnesota
Statutes § 136F.581 and other applicable laws.
Accordingly, MNSCU shall select the vendor(s) whose submittal(s) demonstrate in MNSCU’s sole
opinion, the clear capability to best fulfill the purposes of this RFQ in a cost effective manner. The
MNSCU reserves the right to accept or reject submittals, in order to serve the best interests of MNSCU.
This RFQ shall not obligate the MNSCU to award a contract and it reserves the right to cancel this RFQ if
it is considered to be in its best interest.
General Selection Criteria
The selection of Specialty Consultants will be by a selection committee, consisting of members of MnSCU’s
Facilities Design and Construction staff who will consider the Responder’s overall suitability to provide the
required services based upon the Responder’s submittal.
The selection committee will review all submittals to determine responsiveness. Non-responsive
submittals will be rejected without evaluation. Responsive submittals will be evaluated by the selection
committee based upon the qualifications data provided by Responders and compared to the
requirements included in the applicable Specialty Consultant Requirements Matrix (Attachment 3). Each
candidate Responder shall meet the criteria in the applicable Requirements Matrix to be eligible for
inclusion in the Specialty Consultant Master Roster.
In accordance with this RFQ, and subject to all conditions thereof, the signatory on the Transmittal Letter
agrees that the firm’s response to this RFQ is complete and accurate. All responses received by the
deadline will be evaluated by MnSCU. Each Respondent will be notified of its acceptance or rejection for
inclusion on the Specialty Consultants Master Roster.
This Specialty Consultants Master Roster solicitation is anticipated to be effective by June 20, 2016 or
when posted on the MnSCU Facilities web site and will remain in effect through May 31, 2018, unless
otherwise terminated earlier. In order for a firm to remain on the Specialty Consultant Master Roster
list, Responders will be required to resubmit qualifications every two (2) years. MnSCU intends to
reopen the RFQ process annually for firms interested in being added to the Specialty Consultant Master
Roster or for possible expansion to other disciplines. While on the list, firms must immediately notify
the MnSCU system office, in writing, of any changes in qualifications or changes in staff and provide
qualifications of all new employees being proposed to provide services to MnSCU.

Specific Project Selection
Responders may perform studies, provide reports, design reviews, designs, observations, testing or
construction administration services on MnSCU renovation and/or construction projects on behalf of
MnSCU to assure that the project(s) are designed and constructed in conformance with the MnSCU
Facilities Design Standards and meet the specific project requirements. Specialty Consultant categories
are: 1) Waterproofing, 2) Exterior Masonry, 3) Exterior Windows & Curtain Walls, 4) Roofing, 5) Industrial
Hygiene Services for Hazardous Materials Abatement and indoor air sampling. See Attachment 3 for
descriptions of anticipated services and criteria.
Projects will vary in nature, scope, budget, schedule and locations throughout the state. MnSCU intends
to pre-qualify multiple firms in each category to perform these services. Responders may be selected
directly or through an RFP process depending of the fee level. Responders shall be willing to perform
work at MnSCU institutions located throughout the state of Minnesota (See Attachment 3 for campus
locations). Approved Responders shall work with the College or Universities’ delegated project
manager(s), the system office, Architect/Engineer (A/E) design teams or campus Owner’s
Representative.
MnSCU will use different contract formats to secure services from firms on the Specialty Consultant
Master Roster based upon the, scope and nature of the services and the amount of fees. MnSCU Policy
5.14.2 Consultant, Professional or Technical Services, will govern the contract use for a specific project.
MnSCU retains the right at all times to not use the Specialty Consultant Master Roster process for any
project or to use a public competitive RFP process for any Consultants’ services.
Definitions
Wherever and whenever the following words or their pronouns occur in this proposal, they shall have
the meaning given here:
MnSCU: The State of Minnesota, acting through its Board of Trustees of the Minnesota State Colleges and
Universities on behalf of the system office and/or any MnSCU Colleges or Universities (C/U).
System Office: The central administrative MnSCU of the Minnesota State Colleges and Universities
located at 30 7th Street East, Suite 350, St. Paul, Minnesota, 55101.
Specialty Consultant: Firms or individuals who possess a high level of expertise in specialized aspects of
building design and construction, typically outside the core basic services performed an
Architect/Engineer design team. These services include waterproofing, exterior masonry, exterior
windows and curtain walls, roofing, and industrial hygienist.

Responder: A firm which replies to this RFQ.
RFQ: Request for Qualifications
Applicable Law
A contract entered into as a result of this RFQ shall be governed and interpreted under the laws of the
State of Minnesota.
Deviations and Exceptions
Deviations from and exceptions to terms, conditions, specifications or the manner of this RFQ shall be
described fully on the vendor's letterhead stationery, signed and attached to the proposal submittal
page(s) where relevant. In the absence of such statement the vendor shall be deemed to have accepted
all such terms, conditions, specifications and the manner of the RFQ. A vendor's failure to raise an issue
related to the terms, conditions, specifications or manner of this RFQ prior to the proposal submission
deadline in the manner described shall constitute a full and final waiver of that vendor's right to raise
the issue later in any action or proceeding relating to this RFQ.
Authorized Signature
The Transmittal Form (Attachment 1) must be completed and signed in the firm's name or corporate
name of the Responder, and must be fully and properly executed and signed in blue or black ink by an
authorized representative of the vendor. Separate Transmittal Forms must be submitted for each
specialty service you wish to provide.
Proposal Rejection and Waiver of Informalities
This RFQ does not obligate the Minnesota State Colleges and Universities (MnSCU) system, its Board of
Trustees or MNSCU to award a contract or complete the proposed projects and each reserves the right
to cancel this RFQ if it is considered to be in its best interest. MNSCU also reserves the right to waive
minor informalities and, not withstanding anything to the contrary, reserves the right to reject any and
all submittal’s received in response to this RFQ.
Section II. Consultant Requirements
Consultants may be required to use the Minnesota State Colleges and Universities (MnSCU’s) internetbased Enterprise Project Management System (EPMS) in the participation or administration of a Project.
The functionality of this software may include, but is not limited to, the processing of : a) contract
modifications, including Contract Amendments, Requests For Information (RFI’s), Proposal Requests
(PR’s), Construction Change Directives (CCD’S), and Change Orders (CO’s), b) financial correspondence,
including Applications for Payment and invoices, c) submittals, including construction schedules, product
data, shop drawings, samples, and d) other Project related information.
MnSCU will provide the Contractor with access to the software and initial training for the selected
Contractor’s designated Project representative(s) at no cost to the Contractor. Except for electronic

access and initial training, MnSCU assumes no responsibility for any real or potential costs associated
with the use of EPMS by the Contractor.
Studies, Reports, Construction Observation and Testing: Tasks may include, but are not limited to the
following:

Forensic evaluation of building systems to determine actual or potential source(s) of leakage,
weakness, failure or deteriorated conditions or sampling and evaluation of suspected hazardous
materials.
Provide recommendations for corrective actions.
Provide design reviews of project design documents prepared by others.
Provide expert recommendations for the building systems specialty to be addressed.
Provide analysis of specialty design documents and application of MnSCU Facilities Design
Standards.
Provide cost estimates for corrective actions.
Provide full or part time construction observation to ensure conformance with Project
Documents.
Testing may include infrared scans, selective destructive testing, monitoring of air quality, and
evaluation of materials for conformance with ASTM or similar specified standards and other
project requirements.
Provide review comments of design documents for each phase of design.
Predesign: Tasks may include, but are not limited to the following:

Obtain building documentation from C/U or MnSCU.
Identify and analyze specialty design issues.
Discuss options, alternatives and solutions.
Provide a planning cost estimate to include specialty design, construction, contingency, and
observation and testing costs as appropriate to the scope of the project.
Provide projected design and construction schedule(s).
Design (Schematic Design thru Construction Documents Phases): Tasks may include any or all of the
following:
1. Schematic Design (SD) Phase:
• Review, analyze and evaluate the project program, budget and schedule.
• Interview facility, maintenance and operations personnel at the C/U for their input.
• Review original building documents.
• Provide field verification(s) of existing conditions including conducting destructive test openings
(properly patched) as required.
• Include directly associated structural, mechanical and electrical engineering and other subconsultant services and asbestos testing reports, as required by the specific project.
• Schedule SD review meetings with both C/U and MnSCU personnel.
2. Prepare a formal written SD Phase Report to include the following:
• All SD Submittal Requirements per the MnSCU Facilities Design Standards.

Describe the scope of the project work to be performed related to the Specialty Consultant’s
work. Highlight issues of the project and provide proposed solutions.
An updated SD construction cost estimate and schedule.
3. Prepare a formal written Design Development (DD) Phase Report to include the following:

All DD Submittal Requirements per the MnSCU Facilities Design Standards.
Design Standards Variance Requests.
An updated DD construction cost estimate and schedule.
A Quality Assurance Plan.
4. Prepare a formal Construction Documents (CD) Phase Report to include the following:
•
Prepare drawings and specifications per the MnSCU Facilities Design Standards as applicable.
Include Division 0 documents from the MnSCU Facilities website and insert prevailing wage rates
for the county where the work is to be performed from the Department of Labor and Industry
website.
5. Formal Construction Documents shall include:
•

Project Manual: Provide a separate title page with signature blocks for designers’ approval(s),
table of contents, drawing schedule, and division 0 documents. Provide technical specifications
for each division and section that applies to the project scope of the work.
Provide an updated CD construction cost estimate and schedule.
Prepare all drawings and specifications necessary to convey the exact scope of the work and the
full intent of the design related to the Specialty Consultant’s work.
Prepare all CD’s using computer aided design and drafting technology in an electronic data
exchange file format acceptable to MnSCU. All drawings and specifications shall conform to the
MnSCU Facilities Design Standards and adhere to all applicable building, life safety, and energy
codes in accordance with interpretations of authorities having jurisdiction.
Bid & Award Phase: Tasks may include all or a portion of the following:
• Coordinate bid dates and times with the C/U personnel where the project is located.
• Advertise the project for bid following MnSCU’s On-Line Document Distribution (QuestCDN
private interface) procedures.
• Conduct a Pre-bid meeting and provide meeting notes. Prepare and post addenda on QuestCDN
as needed.
• Attend the bid opening, provide a written bid tab and provide a written award recommendation
letter to the C/U.
• Post bid results to QuestCDN after award of the project.
• Note: Small projects may use bidding procedures other than QuestCDN.
Construction Administration Phase: Tasks may include all or a portion of the following:

Schedule and conduct Pre-construction, Construction (progress), and Pre-Installation Meetings.
Provide minutes for each meeting.
Verify all required permits are obtained by the Contractor.
Review and approve shop drawings and submittals.
Review and approve Contractor payment applications.
Initiate construction change modifications.

Coordinate with C/U personnel and campus schedules.
Verify construction conformance with the MnSCU Facilities Design Standards and project

Coordinate construction observation and testing services, as applicable.

• Conduct inspection(s) for Substantial Completion(s), provide written punch list(s).
• Coordinate and document receipt of warranties, operation and maintenance manuals.
• Provide “as-built” record documents to C/U and MnSCU in a format acceptable to MnSCU on
disk(s).
• Coordinate project final closeout(s) including verification(s) of punch list completion(s) and final
payment application submittals.

RFQ Submittals shall be clear, concise, and clearly follow the format detailed in this RFQ. RFQ Submittals
that are difficult to follow or that do not conform to the RFQ format may be rejected. Responders shall
include required information requested in this RFQ. MnSCU reserves the right to reject a proposal if
required information is not provided. MnSCU reserves the right to change any provision in this RFQ and
will provided all potential Responders notification through addendum posted on the MnSCU Facilities
web site:

This RFQ is not a guarantee of work and it does not obligate MnSCU to award any contracts. MnSCU
reserves the right to cancel this RFQ if it is considered to be in its best interest.
A. This document, including attachments, constitutes a formal RFQ. Therefore, Responders shall
carefully follow the instructions herein to be considered fully responsive to the RFQ. MnSCU
reserves the right to reject a Submittal that is determined to be incomplete or which does not
follow the required structure and format. MnSCU reserves the right to waive minor informalities.
B. Submittals shall be in a package with the Responder’s name and address clearly written on the
outside. The Submittal shall be signed in ink by a member of the Responder’s firm.
C. Not withstanding anything to the contrary, MnSCU reserves its right to:

1. Reject any and all Submittals received in response to this RFQ.
2. Terminate this RFQ or prepare and release a new RFQ.
3. Select more than one Responder in each category.

to ensure a web response by 12:00 pm, Monday, May 23, 2016. When emailing questions, please include
a subject line; “Question Regarding MnSCU RFQ for Specialty Consultants Master Roster - Question from
(Insert firm name).” Questions must include the name of the questioner and their telephone number and
e-mail address. Anonymous inquiries will not be answered. Only potential Responders responding to this
RFQ should send inquiries on behalf of their firm. Questions will be answered by website addenda
obtained from the Minnesota State Colleges and Universities Facilities website,

Responders are responsible for checking the website daily for any updated information on this RFQ that
is posted.
NOTE: To maintain integrity of and to avoid any possible unfair competitive advantage among Responders
during the RFQ solicitation process, there will be no meetings or communications outside the provisions
in the RFQ. To ensure all potential Responders receive the same information about the solicitation, only
questions submitted by email for clarifications about the RFQ will be addressed. Other persons are not
authorized to discuss RFQ requirements before the qualifications submission deadline. MnSCU will not
be bound by and Responders shall not rely on information regarding RFQ requirements obtained from
non-authorized persons.
From the date of release of this RFQ until Responders are selected and formally notified, Responders
shall not communicate with MnSCU’s staff concerning the RFQ except through MnSCU’s contact person
pursuant to the procedure described in the RFQ. If any Responder attempts any unauthorized
communication, MnSCU may reject that Responder’s submittal.
Section IV. Additional RFQ Response and General Contract Requirements
Owner’s Right to Amend RFQ
The Owner reserves the right to amend any segment of the RFQ prior to selection of successful
Responders. If a change occurs in MnSCU’s requirements causing a modification of the RFQ scope of work
or statement of requirements, such change will be communicated in writing as an addendum to the RFQ.

Responders shall be responsible to meet the requirements of all addenda and shall acknowledge receipt
of all addenda in their Submittal cover letter.
Form of Contract between Owner and Respondent
A contract entered into as a result of this RFQ shall be governed and interpreted under the laws of the
State of Minnesota. On a project-by-project basis, a Specialty Consultant that is contracted to provide
services will receive a Purchase Order from MnSCU prior to performing contract services. No work shall
begin until a fully executed contract and Purchase Order has been issued and the Consultant is notified to
begin work. All work will be subject to the requirements of this RFQ, subsequent RFP’s and the contract.
All Consultants will be responsible for the performance of their sub-consultants.
MnSCU may use different contracts to secure services from qualified Specialty Consultants based upon
the scope and nature of the specialty consulting services and the resulting fees amount. The contract

Physical Requirements
Specialty Consultants’ on-site personnel must be capable of climbing up and down a 20 rung ladder and
conduct facilities inspection(s) in cramped and dirty spaces, throughout outdoor construction sites and
in all weather conditions.
Insurance Requirements
The Specialty Consultant will be required to maintain and furnish satisfactory evidence of insurance in
accordance with the requirements of the specific contract used for the services required for each

At the time that a Specialty Consultant is selected for services on a specific project, it shall have in effect
insurance, which shall be reflected in Certificates of Insurance to be provided to the MnSCU Authorized
Representative at the campus before the contract is executed.
Minnesota Government Data Practices Act
The requirements of Minnesota Statutes § 13.05, subd. 11 apply to the contract. The vendor must
comply with the Minnesota Government Data Practices Act, Minnesota Statutes Chapter 13, as it applies
to all data provided by MnSCU, its schools and the MnSCU in accordance with the contract and as it
applies to all data created, gathered, generated or acquired in accordance with the contract. All
materials submitted in response to this RFQ will become property of the State of Minnesota and will
become public record after the evaluation process is completed. Pursuant to the statute, completion of
the evaluation process occurs when MnSCU has completed negotiating the contract with the selected
vendor. If the vendor submits information in response to this RFQ that it believes to be trade secret
materials as defined by the Minnesota Government Data Practices Act, the vendor must:

mark clearly all trade secret materials in its response at the time the response is submitted;
include a statement with its response justifying the trade secret designation for each item;
defend any action seeking release of the materials it believes to be trade secret, and indemnify
and hold harmless the State of Minnesota, MnSCU, its agents and employees, from any
judgments or damages awarded against the State or MnSCU in favor of the party requesting the
materials, and any and all costs connected with that defense. This indemnification survives
MnSCU’s award of a contract. In submitting a response to this RFQ, the responder agrees this
indemnification survives as long as the trade secret materials are in possession of MnSCU.
MnSCU will not consider the prices submitted by the Responder to be proprietary or trade secret
materials.
Costs of Responding to RFQ
All costs incurred in responding to this RFQ shall be borne by the Responder.

To be considered for selection, hard copy RFQ Submittal(s) shall be received at the address identified
above and time stamped by MnSCU reception no later than Wednesday, June 1, 2016, 2:00 pm, CST
The responder shall submit two [2] copies of its RFQ response and a compact disc or flash drive with the

RFQ response in pdf format. Proposals are to be sealed in mailing envelopes or packages with the
responder’s name and address clearly written on the outside. One copy of the proposal must be
unbound and signed in blue or black ink by an authorized representative of the vendor.
Proposals received after this date and time will be returned to the responder unopened.
Fax and e-mail responses will not be considered.
A firm (“Responder”) that desires to be considered for inclusion on the Specialty Consultants Master
Roster shall submit their Qualifications by mail or courier as further described below. The package shall
clearly include the Respondent firm’s name, address, and the following title: “RFQ Response for Specialty
Consultant Master Roster”.
If delivery of the submittal is not made by courier or in person, the use of certified or registered mail is
suggested. Note: Use of certified or registered mail does not relieve the Responder of the responsibility
to ensure the submittal is received on time.
Submittals by fax, email or other electronic transmission is unacceptable and submittals delivered by
these means will not be considered.
A Responder's submittal received after the deadline will not be accepted or considered. Receipt by
MnSCU of a submittal received after the closing date and time as stated herein shall not be construed as
acceptance of the submittal. Late submittals will be logged as to date/time received and thereafter
returned unopened to the Responder.
Submittals that do not provide the information requested will be considered unresponsive with the
requirements of this RFQ and will be rejected. Late submittals will not be considered and will be
returned unopened. MnSCU will not be responsible for postal or courier deliveries not received by
deadline, delays caused by weather conditions, or any other associated delays.
Qualifications Submittal
A.
Two (2) hard copies of the RFQ Submittal with a cover with the name of the RFQ, Respondent’s
name and submission date. Include an index for easy reference to the material. Insert labeled tab
sheets in front of each submittal section.
B.
One (1) electronic copy on a CD in ‘pdf’ format. The document should be password secured against
copying of text, images and other content. Label and date.
C.
All copies of proposals shall be submitted with one staple in the upper left hand corner. No three
ring binders, edge binding or plastic inserts allowed.
D.
All pages shall be sequentially numbered within each section (1.1, 2.1, etc.). Font size no smaller
than 10 point.

E.
Maximum 30 faces (excluding faces of Transmittal Form, cover letter, front and back covers, blank
dividers, and other Attachment forms).
F.
Respondents shall follow the order of “Qualifications Response Contents” found in this RFQ
Qualifications Response Contents
A. Transmittal Form
Each Responder shall submit their qualifications with a Transmittal Form (See Attachment 1). The
form shall transmit the qualifications, identify all materials and enclosures being forwarded
collectively as a response to this RFQ, and be signed by an individual authorized to commit the
Responder to the scope of services described as an acknowledgement of the statements on the
Form.
B. Cover Letter
Provide a one page statement expressing interest and qualifications for inclusion on Specialty
Consultants Master Roster. This cover letter shall be signed in ink by a member of the firm
authorized to commit the Responder to the scope of services proposed. The cover letter shall
include the authorized member’s typewritten name, title, phone number, email address, and
Respondent’s web site address if available. The cover letter shall acknowledge receipt of any
addenda and that they have incorporated the requirements of the addenda into their submittal.
Acknowledge that the Responder’s team is willing to provide Specialty Consultant services to all
54 MnSCU Campuses. Refer to Attachment 2 for campus locations. State any known conflicts of
interest and attest to no known organizational conflicts of interest.
C. Responder’s Qualifications Response
1. Information On Firm(s)
For Prime Responder’s firm and each sub-consultant firm, provide brief descriptions including:
a. Name and location(s)
b. Year established
c. Legal status
d. Ownership, list all partners
e. Total staff, staffing by discipline, and how many employees are located in
Minnesota
f. A Corporate profile of the firm outlining its history
For each firm with multiple offices, briefly summarize; which office is
the primary location, and how many employees there are at each office location.
g. Identify if your firm is certified as a Targeted Group/Economically Disadvantaged or Veteran
Owned Preference firm.

a. List the name, title, and telephone number of the firm’s principal who will serve as the point
of contact.
Provide a complete list of all key personnel (including proposed sub-consultants if applicable),
who will be assigned to these projects. Include key personnel relevant experience,
qualifications for this work, roles and responsibilities. Include resumes (limited to one page
per person), specific professional registrations and any specialized or technical certifications
that your firm or members of your team may have, and relevant experience to past similar
projects.
b. Include a brief statement of the team’s past or current working relationships. For each team
member provide:
1) Name, title and position in firm
2) Home base (if in a multi-office firm)
3) Responsibility on these projects
4) Years of experience
5) Relevant recent experience (if with another firm, so note)
6) Registration – List Minnesota or other registration numbers for all licensed professionals
(include specialty if Professional Engineer), Registered Roof Observer (RRO), Registered
Roof Consultant (RRC), Registered Waterproofing Consultant (RWC), etc.
7) Include copies of licenses for all registered professionals (reduced size legible copies are
acceptable).
3. Project Experience
For architectural or engineering firms involved with design, provide examples of similar projects
of various size and complexity completed within 5 years prior to the date of this RFQ or inprogress, including:
a. Name, location and owner
b. Brief description (e.g. size, cost, relevance)
c. Photographs, sketches and/or plans
d. Design firm of record
e. Indicate the role each proposed team member on each relevant project.
f. Completion date or current status
g. References
For engineering or technical firms, provide examples of relevant projects recently completed or
in progress, including items a-f above. Photographs, sketches and/or plans should represent only
that portion of the work for which the firm was responsible. For example, if the firm designed the
mechanical system, then a photo of the mechanical system is appropriate, not an exterior building
image.
4. Testing Qualifications
Summarize your team’s testing capabilities specific to the category of specialty services included.
Provide name and contact information of the testing firm if subcontracted.

Provide a detailed listing of testing equipment owned and operated by the Respondent or a subcontract
outside testing lab. Indicate the anticipated equipment to be assigned for testing; the equipment’s
operator; and the operator’s background, training, certification, and licensure (where applicable) as it
applies to the equipment proposed for use. Acknowledge test results can be provided within 48 hours of
submittal.
