HOW TO APPLY
1. Email this completed application form and all supporting materials to the festival
coordinator Mary at vanfestivallaunch@gmail.com
2. In the subject line of your email list your name, the discipline you are submitting
for, and the title of your performance
Example: Dance/Josephine Smith/Dusk
3. Save your application form and all supporting materials with your full name:

6) SUPPORTING DOCUMENTS
1. Letter of Intent Describe your goals as an emerging artist, what you need to
succeed at the next stage of your career, and why you would like to take part in
Launch! Festival 2015 (200 words in document titled: Intent _JSmith.doc)
2. Artists Statement What is your piece about? Describe the history (creative
origins, artistic process, and underlying themes, etc.) of the work you are
submitting. Indicate where you would like the work to go and what impact it is
intended to have on your audiences. Also describe in what ways this work
represents your unique artistic abilities and style (200 words; document titled:
ArtistsStatement_YourName.doc)
3. Resume Provide all relevant professional experience, background, education or
training of the principal artists involved (Document titled:
Resume_YourName.doc)
4. Official Artists Bio This will be included in any promotional materials for the
festival. Make sure it is well written, edited, and no longer than 200 words. If you
are an artists group of 3 or more (collective, troupe, band, etc.) provide the bio of
the group or the group founder/choreographer, etc, rather than of each individual
(Document titled: Bio_YourName.doc)
5. Photo Provide a high-resolution current headshot of you or your group. Please
also provide a photo credit. (Photo_YourName_PhotoCredit)
6. Previous work If you wish, you may submit samples of your previous work to
highlight your talents. Extra materials should not exceed 5 minutes in length and
are optional.
Please note! The information you submit will be used to promote you if you are a
winning candidate, so please submit finalized written materials!
Applicants will be notified of the final decision by the end of May, 2015
