Confirmed Program:  Summer 2016  Fall 2016  Spring 2017
Costs: In addition to your standard full-time tuition (12-17 credits), costs include a $1,500 program fee for the fall and spring terms and a $1,250 program fee for the summer session.
Submittal of this Commitment Contract and the Program Fee payment secures your spot in the program and serves as a binding agreement between you, KSU, and The Fashion School, of your participation and financial obligations. The original Commitment Contract must be mailed to the address, below, and postmarked by the deadline date. Keep a copy for your records. Once the NYC Studio has received your Commitment Contract, you will receive a confirmation, via email, verifying that you have met the program requirements and may make the Program Fee payment. DO NOT make the
Program Fee payment until you have submitted your Commitment Contract and have received an email from the NYC Studio verifying you have met the requirements.
Please initial each of the following items, as confirmation that you understand and agree to the rules, policies, and obligations of the NYC Studio program. I understand that:
_____ I must have a 2.5 cumulative GPA to attend the NYC Studio program. If I do not have the required GPA, I will not be permitted to attend. It is solely my responsibility to fulfill the requirements of participation.
_____ I will send the Commitment Contract to the address below and keep a copy for my records and I will NOT make a fee payment until I have received an email from the NYC Studio verifying that I have met the eligibility requirements.
_____ By paying the $1,500 or $1,250 Program Fee, I am finalizing my admission to the NYC Studio for the term specified, above, and I am making a commitment to the Bursar’s Office to pay the tuition when billed. I understand that the University is unable to secure a refund for the obligations undertaken when I commit to participate in the NYC Studio program; therefore I understand that this Program Fee is nonrefundable and non-transferrable. I understand standard full-time tuition will be billed at the time that fall or spring bills are sent from the
Bursar's Office and must be paid by the normal due date.
_____ I must abide by the rules and policies of the NYC Studio and the Student Responsibility Guidelines and have received, read and understood these policies, provided to me.
_____ Should I be unable to participate for any reason, I must notify the NYC Studio Director of my withdrawal from the program in writing
(ykim16@kent.edu).
_____ I am responsible for securing housing accommodations at least 90 days prior to the start of the program (March 8th for SU16 students, May
31st for FA16 students or October 19th for SP17 students) and must provide proof of my housing arrangements (i.e. housing provider confirmation receipt with private NYC address) to the NYC Studio by that time, via email. If I am unable to secure housing by the dates, above,
I must notify the NYC Studio, before the deadline. Kent State University assumes no responsibility for your housing including but not limited to costs, fees, landlord problems, utilities, roommate issues, security issues, etc.
Your signature below indicates that you have read and understand the program requirements and understand and agree that you are entering into a legally binding contract for all costs associated with the NYC Studio program.
PROMISSORY NOTE
In exchange for the value granted, I hereby promise to pay to Kent State University the $1,500 or $1,250 Program Fee and pay the tuition billed by the Bursar.
