OFFICE OF THE UNIVERSITY ARCHITECT Guidelines for the Submission of an Architect / Engineer Technical Proposal Project Name Kent State University Project Number KSU- Contingent upon acceptance of your Technical Proposal submission, your firm will be recommended as the Architect/Engineer (A/E) Firm for the above-referenced Project. The Architect/Engineer Agreement for professional design services, via Kent State University Office of the University Architect’s (KSU-OUA) Web site at the Internet address: http://www.kent.edu/universityarchitect/designassociates/formsfordesignassociates.cfm, shall be the terms and conditions of performance of your services. The Scope of Services and your Technical Proposal shall be made part of your Architect/Engineer Agreement. Below are guidelines for preparation of your Technical Proposal. In order for the Kent State Office of the University Architect Contracts Manager to prepare your firm’s Architect/Engineer Agreement for this Project, please download and submit with your Technical Proposal the following forms (for the applicable Project Delivery Method) from the KSU OUA Web site at: http://www.kent.edu/universityarchitect/designassociates/formsfordesignassociates.cfm: KSU OUA Exhibit C – Minimum Stage Submission Requirements KSU OUA Architect/Engineer EDGE Participation / Intent to Perform form KSU OUA Architect/Engineer Equal Employment Opportunity Requirements KSU Standard Affirmation and Disclosure Form Executive Order 2011-12K The Technical Proposal shall demonstrate how your firm will best implement Kent State University’s program for the Project. The Technical Proposal should comply with the following requirements: Item 1: Your Technical Proposal should be clearly and concisely written to facilitate our review of your Technical Proposal and responses for your firm or team. Include your understanding of the Project budget including Construction Budget, contingency, Owner equipment, Percent for Art, Kent State University Office of the University Architect fees, Architect/Engineer fees, Consultant fees, Construction Management fees (if required), advertising, etc. Item 2: After reviewing the Program of Requirements with Kent State University Office of the University Architect, describe your understanding of the goals set forth on this Project by the Office of the University Architect. Also describe how your proposal responds to unique or special goals of Kent State University Office of the University Architect. Item 3: Provide a detailed description of your proposed approach to the staffing, communication and development of all Project Stages (for the applicable Project Delivery Method), i.e.: Predesign (or Organizational Meeting), Schematic Design, Design Development, Construction Documents, Bidding and Award, Conformed Documents, Construction Administration, and Close-Out. KSU-OUA 01/2014 GUIDELINES FOR THE SUBMISSION OF AN A/E TECHNICAL PROPOSAL Page 1 of 3 Item 4: Provide information on how you intend to address Project Management, Scheduling, Cost Estimating and containment, and Construction Administration, on this Project. Item 5: Indicate Preliminary Project Schedule including submission of Schematic, Design Development and Construction Documents allowing 60 days for receipt of executed Contract. Additionally, show a proposed time frame for Bidding and Construction. Item 6: Submit your proposed fee for Basic Design Services which shall include all Consultants, review, evaluation and validation of the Program of Requirements, Bidding, and administration of Construction Contracts. Include the minimum number of hours per week of on-site construction inspection per the advertisement included in your Basic Services. Propose an allowance for notto-exceed Reimbursables, detailing the projected costs of basic building permits, asbestos abatement analysis, site survey and soil borings. Submit your proposed fee for any additional services required or requested, including a Change Order Fee Allowance, if applicable. Include a listing of hourly fees for staff in the event that hourly services are requested or allowed. Item 7: See Exhibit A, Article 7 of the A/E Agreement Standard Terms and Conditions for details about Insurance and Indemnification requirements. Submit with the A/E Technical Proposal a Certificate of Insurance for Professional Liability Insurance (ACORD Form 25). To request a modification of the required coverage, the A/E firm must submit a detailed letter to the Director of the Office of the University Architect for consideration. Please note that your firm’s Consultants must also comply with the applicable Standards Terms and Conditions. Item 8: Building Security considerations should be included in the Scope of Work for design services. Kent State University’s security considerations shall be part of fulfilling the Program of Requirements. Since each project will have significant differences in security needs, there are no guidelines established by Kent State University to generally address this issue. Therefore, the A/E Firm should bring security issues to the attention of Kent State University Office of the University Architect during the Program Verification process in order to identify specific needs and incorporate security considerations into the Project. Item 9: The following information is required of all Architect/Engineer Firms and their primary Consultants in order to complete the release of funds: A. Equal Employment Opportunity Requirements (EEO) for your firm and your primary design disciplines (Consultants); B. Approximate fee amounts itemized for all primary design disciplines (Consultants) and identify the contact names of each Consultant and their assigned roles on this Project; C. EDGE Participation Intent to Perform documentation; D. Ohio Bureau of Workers’ Compensation Certificate of Premium Payment; E. Certificate of Insurance (ACORD 25 form); F. List names of your staff and your Consultants’ staff and indicate their roles they will be performing on this Project; and H. Provide information for all contracts that your firm has had with any state agency or university in the last 24 months: State Agency Contracted Services Percentage Completed Amount of A/E Contract $ Please provide this information with your Technical Proposal for your firm and all your primary design disciplines (Mechanical, Electrical, Structural, Construction Management, etc., as applicable). The KSU OUA Architect/Engineer Agreement Summary Worksheet, Equal Employment Opportunity Requirements, and EDGE Participation forms are provided for your use. Item 10: Ohio Revised Code (ORC) Section 9.24 prohibits the State from awarding a Contract to any offeror(s) against whom the Auditor of State has issued a Finding for Recovery if the Finding for Recovery is unresolved at the time of award. By submitting a technical proposal, offeror warrants that it is not now, and will not become subject to an Unresolved Finding for Recovery under ORC 9.24, prior to the award of any Contract arising out of this RFQ, without notifying the Kent State University Office of the University Architect of such finding. Your Technical Proposal shall consist of not more than ten (10) single-spaced pages. The tenpage requirement is meant to include resumes, text, etc. Additional information, such as cover letters, F110-330 Statement of Qualifications submissions, tabs/dividers, index or charts are not to be included.
