Copies: Scheduling Advisor Revised: May 8, 2014 Center for Student Involvement 1 Directions for Completing the Sales and Solicitation Permit A Sales and Solicitation Permit must be completed by any organization that is soliciting signatures, selling tickets, or fundraising (either by the organization or with an outside vendor). This form must be filled in prior to the activities occurrence. Persons not affiliated with the University may not engage in sales or solicitation among the general University population while on University property unless sponsored by a registered student organization or University department. If an outside vendor is engaged, a Joint Venture/Vender Form is required. Organizations, outside vendors or University departments may not sell food within the Kent Student Center, on the Risman Plaza or on the Student Green. 1. Enter the name of the sponsoring organization (no initials, please). 2. Enter the organization representative’s name, phone number and e-mail address on the designated lines. 3. Enter the name, email, and phone number of your organization advisor on designated lines. 4. Fill out the items to be sold or collected (i.e. flower sale, petitioning for signatures, etc.). Include amount being charged and use of revenue, if appropriate. 5. List the date(s), time(s) and location(s) of the sale or solicitation. If the event is being held in a residence hall, the signature of an authorized Residence Services staff member is necessary. If the event is being held in or near a cafeteria, the signature of an authorized Dining Services staff member is necessary. Appropriate signature line is located below the CSI approval line. Signatures must be obtained before forwarding to the CSI for approval. An email directly from the applicable representative containing their name on the signature line is also acceptable. They can email it back to you, and you can forward it to the CSI for approval (see step 6). 6. Email/forward the completed form to your Center for Student Involvement Liaison. The Center for Student Involvement liaison will then review, approve and forward the permit to the Scheduling Office for processing. 7. Once the Scheduling Office receives the approved permit, a confirmation e-mail will be sent to the organization representative, advisor and CSI liaison. This confirmation e-mail is the only statement that the event has been confirmed. Revised: May 8, 2014 2
