North Central Thames Foundation School (NCTFS) Individual Placement Description - Whittington Hospital NHS Trust Placement The department The type of work to expect and learning opportunities Location Supervisor(s) Main duties of the placement Typical working pattern in this placement F2 Respiratory Medicine Respiratory ward Nightingale (21 beds) with occasional medical outliers during winter months. Two Respiratory teams led by Drs Patel/Kaiser and Dr Lock/Dr Restrick. Patients equally divided between the two teams depending on whom patient is known to through previous admissions or outpatients. Respiratory teams work daily with ward nurses, respiratory nurse specialists, physiotherapists, occupational therapists, dieticians, speech and language therapists, smoking cessation advisors and psychologists. Community Respiratory Team (CORE), TB nurses, lung function team, and liaise closely with ITU. All F2 Doctors will generally be ward based during the ‘normal’ working day and expected to deliver the daily medical care of all the patients on their ward irrespective of specialty. During this placement F2 will Take history’s and examine patients Identify and synthesise problems Prescribe safely Keep an accurate and relevant medical record Manage time and clinical priorities effectively Communicate effectively with patients, relatives and colleagues Use evidence, guidelines and audit to benefit patient care Act in a professional manner at all times Cope with ethical and legal issues which occur during the management of patients with general medical problems Educate patients effectively Become life-long learners and teachers. Present/teach at Respiratory department lunchtime meeting on Monday at least once during 4 month attachment Able to confidently do spirometry and interpret Perform ABG’s and interpret Knowledge of NIV use/indications and optimization of settings Multidisciplinary team working skills Increased confidence in interpreting CXR’s and HRCT/CTPA’s. Increased knowledge and practice of acute and chronic COPD/Asthma management In particular F2’s will be responsible for presenting patients on the consultant ward round, and at the Wed morning Lung Cancer MDT or respiratory meeting in radiology seminar room, sometimes doing a ward round independently and discussing patients with consultant or registrar (if senior colleagues are offsite or in clinic). Nightingale Ward, Whittington Hospital Dr Lock/Dr Restrick, Drs Patel/Kaiser The F2 doctor is responsible with other staff for the ward care of patients will clerk new admissions, see and assess patients under their care on a daily basis and undertake practical procedures. They are also responsible for maintenance of the patients medical record. Mon: Board round then ward work, Respiratory meeting/teaching 1pm (1 hour) Tues: Board round, ward work then Consultant ward round at 1400 Wed: Respiratory/Lung MDT 0900 till 1030 board round 1045 then ward work. Thurs: Board round, ward work, then lunchtime teaching and ward work Fri: Board round ward then Consultant ward round, weekend handover On call requirements: Generally one ward cover evening per week 1700 – 2100.
