May 17, 2006
The meeting came to order shortly after 9 am in Anderson 114.

• The minutes from the two previous meetings (3/15/06 and 4/17/06) were reviewed and
approved.
• David Zuckerman discussed the Accident/Incident Reports
• 7/8/05 – Fall due to tripping – area tidied up. Follow-up: Determined it is NOT Graham Visitor
Center in arboretum or Anderson Hall, perhaps Winkenwerder or Bloedel. Tripped over
typewriter left on floor upstairs admin building is what report stated. Steve checked upstairs
Anderson , but there were no typewriters and doesn't recall there being any at the time of the
incident.
• 2/1/06 – Struck by tree branch under tension while using chain-saw. Reviewed chainsaw safety
w/ crew. Affected party was OSHA complient re: PPE's.
• 2/9/06 – Back injury from lifting – instructed to avoid lifting heavy items. Affected party has
history of back condition. Reviewed incident w/ affected party. Needs to be cautious while
working and avoid stresses on back whenever possible.
• Woodshop Management Action Items

The meeting came to order shortly after 9 am.

• The minutes from the last meeting (3/15/06) will be reviewed and approved at the June
meeting.
• EHS staff Introductions: Attending meeting to primarily provide support for any committee
issues and efforts. Will periodically attend scheduled meetings.
• Accident Report Action Items from last meeting:
• David Zuckerman followed-up on hornet sting incident: CUH staff do have 911 access (cell
phone). Also, recommendation to schedule safety meeting with all UWBG grounds staff to
review sting protocol and other warm weather risks was made.
• Beverly Anderson still researching feasibility to install rollers/glides on heavy tables in Forest
Club Room. (Passed on to Margery Cooper?)
• EHS staff Denis Sapiro gave update on April UWWide H&S meeting:
• OARS - New edition to be launched shortly. David Zuckerman will provide EHS with CFR
facilities list.
• New No Smoking Policy – UW smoking areas to be designated, but no smoking anywhere on
campus now.
• Dog's on campus: Working to allow other service dogs besides for the Blind.
• Woodshop: Training is scheduled. Two other items:
• Remove Fred Hoyt and David Zuckerman from Users list.
• Provide backup key check-out when Margery is not available.
• New Business:
• David Zuckerman raised ergonomics as a key issue for group 10. EH&S will support w/
classes and website info.
• Nutria in the news. Steve West has students monitoring and mapping dens, etc. Big issue for
state. EH&S will begin control program soon – Charles Easterday. David Zuckerman to
determine if City or State will be responsible for arboretum. For more info.

March 15, 2006
The meeting came to order shortly after 9 am.
Present: David Zuckerman, Beverly Anderson, Sharon Doty, Steve West, and Ara Erickson
Absent: Bill McKean, Neal Bonham
• The minutes from the last meeting (2/15/06) were reviewed and accepted.
• Ergonomic issues were discussed as a potential future training topic.
• It was noted that the Arboretum incident informally discussed last time has been formally
reported and will eventually reach the committee. Available at this meeting was a summary of
incidents for Group 10 for the period 7/1/05 through 12/31/05. The data showed a rather even
distribution of types of incidents among cuts/contusions, falls, insect bites, and repetitive
injuries/strains. The subsequent discussion resulted in four action items:
• Beverly Anderson will pursue the feasibility of installing rollers/glides on the (very) heavy
wooden tables in the Forest Club Room.
• Beverly Anderson will post notes on the stacker chairs used in the Forest Club Room to stack
only 1 at a time in order to avoid back strain/injury.
• David Zuckerman will pursue the feasibility of 911 access for all field workers so that urgent
incidents can be reported immediately.
• David Zuckerman will investigate which area in the Arboretum was involved in the tripping
incident and arrange for its clean-up.
• David Zuckerman reported on the most recent U-wide Safety meeting, whose topics included
recent statistics involving Workers' Comp and workplace violence, and a reminder to always tie
down loads.
• A possible trainer for woodshop power tools has emerged. Alex Montgomery is an
Instructional Tech in the School of Art (Ceramic & Metal Arts) and, on a trial basis, is willing to
provide a training session for CFR in the CFR woodshop once each quarter. His services would
be most likely for safety training only. Another solution may need to be found for preventive
maintenance on the machines themselves. Invitations to the first training will go out to all,
including those already on the list if they want a refresher course. The committee recommends
acquiring a better box for goggles and earplugs, and a better first aid kit.
• David Zuckerman reported on actions being taken to counter illicit activity in the Arboretum,
which is damaging some of the collections. These counter actions include such things as
permanent closures of some parking lots, randomly scheduled closures of the drive gates, and
consideration of some sort of employee uniform to increase their visibility as authority figures.
• Red campus emergency phones are in the process of being installed at selected campus
locations. They are on a phone system that will be the first to come back up after a major
emergency.
• Improvements are being made to OARS (On-line Accident Reporting System). Bugs should be
reported to David Zuckerman.
• Next meeting scheduled for April 19, 2006 in Anderson 107A.

Wednesday, February 15, 2006
The meeting came to order shortly after 9 am.
Present: Beverly Anderson, Sharon Doty, Steve West, and Neal Bonham
Absent: Bill McKean, David Zuckerman, Ara Erickson
• The minutes from the last 2 meetings (12/21/05 and 1/18/06) were reviewed and accepted.
• Training new users of woodshop power tools continues to be a problem. The committee has
concluded that if the shop is to remain open this will need to be handled internally on a volunteer
basis among the user group. Ideally students would receive training in the fall as part of
orientation; training would occur after that on an as-needed basis. Steve West will follow up with
a message to the user group. (NOTE: not mentioned in the meeting, but Facilities Services has
been requested to assess and advise on the shop's vacuum system.)
• Sharon Doty's 14C-formaldehyde storage issue was resolved by taking the material to another
unit with an existing lock box.
• A safety incident in the Arboretum was reported on an informal basis; the committee has not
yet received a UW report. While chainsawing a downed tree a gardener was hit in the face (no
permanent injuries) by a limb that was not noticed to have been under tension. Neal Bonham will
ensure that the incident was formally reported through OARS, which will result in the
committee's receiving a formal report for review and possible action.
• Next meeting scheduled for March 15, 2006 in Anderson 107A.

January 18, 2006
The meeting came to order shortly after 9 am.
Present: David Zuckerman, Beverly Anderson, Sharon Doty, Steve West, Ara Erickson, Neal
Bonham
Absent: Bill McKean
• The minutes from the last meeting were not available and will be reviewed later.
• New members Ara Erickson and Neal Bonham were welcomed. Chair/co-chair elections were
held: David Zuckerman = chair; Beverly Anderson = co-chair (temporary until vacant staff
position filled). We are in Group 10 in the UW's campus-wide safety committee system. Quorum
arrangements were reviewed: a quorum consists of 3 committee members, 2 of whom must be
elected. The meeting schedule remains the 3 rd Wednesday of each month, from 9-10 am,
typically Anderson 107A.
• New members especially, but any committee member, should register to attend a 2-hour
training session for organizational health and safety committee members on January 25 th or 26
th . Karen VanDusen and Dave Leonard from EH&S will provide information needed for all
committee members. Beth Speck will present "The Art of Enrollment: Engaging Others in a
Shared Vision". Participants will gain skills to enhance their ability to engage both groups and
individuals around shared goals, tasks and vision.
• The woodshop situation was once again reviewed. Surplusing has been delayed because of a
new system which requires waiting for batch stickers to apply to items to be surplused.
Everything will be surplused unless a use can be found within the college. Power tools will be
locked with DO NOT DUPLICATE keys. Steve West will update the draft policy statement
(earplugs should be added). A certain level of supplies will need to be maintained: ear plugs,
safety glasses, blades. Training for new users is the current weak link; David Zuckerman will
pursue previously identified potential trainers, and Beverly Anderson will seek advice from
Drama, Henry, and Art. A maintenance schedule will be needed; perhaps carried out by Facilities
Services on a 6-week schedule. The vacuum system should be restored; Beverly Anderson will
ask Facilities Services to review. It was emphasized that woodshop use must be self-supporting
& that centrally monitored systems for such things as providing “buddy” information is not
possible.
• Sharon Doty reported that her rolling freezer must be strapped to the wall, an outcome of a
recent lab inspection. She also reported that her radioactive waste is now gone. In a related
discussion it was discussed that C-14 dating is not possible in Winkenwerder and possibly
Bloedel because of existing environmental conditions.
• There were no safety incidents to discuss.
• Next meeting scheduled for February 15, 2006 in Anderson 107A (changed to 114).
