Old Business/Action Item
o “Blacked-out” confidential accident report info readable in e-mail solution to use
a darker marker. Steve reiterated the confidentiality is an issue and that “Word”
had a means to redact text. Jay Herzmark discussed with Joseph Kwok and was
assured they would blacken the names out more thoroughly.
o URGENT -Warden list – need to complete this ASAP. Margie emailed a
reminder email to Steve West so as to ask the Dean to look into the Bloedel
warden non-response and/or compliance with request for volunteers.
o Woodshop Protocol, including tools training and woodshop check-in/out is part of
the regular Grad Student orientation.
o David found out that there is no OARS beta-testing available at this time for
others to test (faculty, staff).
New Business
o Accident/Incident OARS reports (2).
ƒ Unique Event No. 9665 – Jay Herzmark suggested the corrective measures
include more detail of the event and avoidance of future events. Steve
West said more information regarding the site of the accident would be
helpful.
ƒ Unique Event No. 9802 – Steve West suggested posting a sign cautioning
procedure for set-up/break-down on the door where the tables are stored.
David will contact the respective supervisors with the committees’
recommendations.
U-wide November Meeting news. Neal reported for David the main topics
discussed; . For specific details, UW EH&S minutes are posted at their web site:

Wilderness First Responder – Advanced First Aid memo to Dean was reviewed.
Steve suggested beginning with a program flyer requesting interest from
prospective attendees before submitting the memo to Dean. Budgetary
constraints and probable lack of numbers make this proposal unlikely. Suggested
this would be a specialized training for a specific course/job.
o Other
ƒ Jay asked if CFR had regular safety inspections scheduled. Steve
indicated inspections took place in labs by respective lab “owners." Jeff
offered that EH&S could assist in a clean out. Margie will email Steve
about lab cleanout.
ƒ Jay asked which of us had done the asbestos training. One out of four,
which according to Jay is better than the UW average (~17%). Suggested
for all.

o E-mails:
ƒ RE: Accident reporting L&I policy changes. David will send a general
email introducing the new OARs release scheduled for January 2009.
ƒ RE: “Blacked-out” accident report info readable in e-mail appears fine,
now. Margie’s concern regarding the November OARS report: Joseph will

RE: Urgent -Warden list confirmation e-mail. Margie will forward past
email directed to Bloedel warden selection to Steve, who suggested
implementing the other two buildings now. David contacted Sally and
Steve regarding distributing supplies and training for Anderson Hall and
Winkenwerder Hall wardens. Decision made to get Bloedel confirmations
and then Sally will schedule training for all wardens.
o Provide incoming students w/ Woodshop Protocol –inc. training and checkin/out. David contacted Michelle as to whether this is part of the regular student
orientation program and found out orientation was for Grads only.
o OARS beta-testing comments. Mike Wetzel tried the new method; nothing to
compare with, having never used the old system. David asked if the tutorial is
open for others to test (faculty, staff) and found out there is no beta testing
available at this time.
New Business
o Accident/Incident reports (1). Committee reviewed and questioned the
emergency procedure used. Was Tacoma really the closest emergency facility
available – what about calling 911? David contacted the supervisor, to discuss
Pack Forest emergency protocol and found out this OARS report should have
been initiated by School of Architecture and reviewed by their safety committee;
not a CFR issue.

recommended by David.
o OARS update: David will contact CFR population via email regarding on-line
w/tutorial available after January 1, 2009.
o Aquatic noxious weeds, DOE grant – a 5yr study being pursued as a source of
funding.
o Other
ƒ Not brought up during the meeting, but David sent an email confirming to
UW EH&S (copying the CFR committee members) the CFR Facilities and
Sites listed in the OARs site.
ƒ David sent an email to the CFR population alerting everyone of Daylight
Savings time – November 2nd – as a good time to check/change batteries
in smoke detectors, while turning back clocks.

Warden List and supplies update Steve West provided a copy of an email in which
potential wardens (Briggs, Xue, Krause, O’Shea, Lewis, Edmonds, Vogt, Zabowski,
Bolton, Johnson and Moskal) were requested to contact Margery Cooper with their
willingness to serve as a volunteer Fire Warden. There have been no contacts made to
date. Margie forwarded a copy of this correspondence to David Z.
Warden supplies are available through EH&S web site. David suggested our ordering the
necessary supplies now. Cost was discussed.

Accident reporting policy changes Discussion on what constitutes a supervisor was
brought up during the last meeting. Anyone who is a WA State employee and oversees
the affected party. Discussed current safety procedures/guidelines posted on the CFR
website are up-to-date and in compliance with the updated EH&S website link for

Three main L&I requirements:
8 hr requirement for reporting any hospitalization of even one employee

Supervisor accident reports need to be completed w/in 7 days to comply with state and
Federal reporting requirements for the OSHA 300 log.

Reminder: Depts. are required to inform their employee’s next-of-kin (emergency
contact) about hospitalization due to work-related injury.
David will send above info to the Building Coordinator, Sally Morgan, to distribute to all CFR
supervisors and PI’s.

mixing gas in a designated outside area. David will discuss this idea w/ City staff. Jay Herzmark
came out w/ suggestions on how to manage during the interim, e.g., sweeping compound to keep
dust down, dollies for transporting gas cans.

o Warden List – Steve rewrote the Bloedel request for warden’s in an email
[Attachment A] sent June 18th and the individuals have been directed to respond
to Margie by email. Additionally Steve included the individual lists for
Anderson, Bloedel and Winkenwerder (also available).
A question regarding how/where to procure supplies for the Wardens (i.e., vests, hats, etc.) was
brought up. David will find out more details.

developed regarding the process. Resolution pending Human Resources
decision and notification.
ƒ Asbestos Awareness Training (All CFR) – David will test the training to
determine how the process works and will report back next meeting.

Canopy Crane, ONRC and Pack Forest to facilitate the collection of this

committee): Edict from President’s office to determine the top 10 UW

ƒ All workplace accidents that result in hospital admission of
affected party, EH&S needs to be notified within 8 Hours by
supervisor of affected party of incident

U-wide meeting held at Tacoma campus. Highlights reported by David:
ƒ Video of staged office fire – 4 minutes to full involvement
ƒ Record-keeping is important for safety committees to keep: Classes,
OARS, meetings, etc. e.g. Workplace Violence – attendance required
every 2 yrs. Need to keep track of who’s taken class. Supervisor and PI’s
responsibilities to also track who’s taken the class.
ƒ Denis Sapiro suggested having meeting minutes posted online (CFR’s
minutes are already posted online). Discussion of having them physically
posted on a specific bulletin board at a particular site. David to follow up
o Health concern: Freshman Pond spraying fowl feces from fountain. Jay will
contact Charles Easterberg.
o If Woodshop tools training required, contact CFR Building Coordination team

rules provided by Alex Montgomery (trainer) which will be posted in the
woodshop, following discussion with cfrbldgs.
o Field Safety Manual and other Safety Plans (re: ONRC, Pack Forest, Arboretum,
BGMC and other Seattle campus buildings) probably have their own. Margie
provided copy of Field Safety Manual to David.
New Business
o Lab training question for Zareen to be directed to Joy (per Jay).
o Beta test for on-line “Asbestos Awareness Training” went OK, scheduled for
summer release UWide.
o U-wide meeting news reported by David. Minutes provided on EH&S website.
o CFR had the workplace violence training, but Jay Herzmark pointed out that it is
required for everyone and if someone missed it, can take an open enrollment class

o David informed committee, thanks to Dean’s office funding, control of class one,
noxious wetland weed, Garden Loosestrife will be contracted out to Aquatechnix

Old Business Action Item
o Warden List – Steve provided updated lists:
ƒ Anderson list has both a “Fire Director” and an alternate for each floor
(Ground – 3rd) and at each end (East and West) of the building.
ƒ Darryll Johnson will be retiring in March and will necessitate
replacing the ground west warden at that time
ƒ There is no staff/faculty located on 3rd floor west, therefore no
warden was assigned
ƒ The question was asked if the warden for the east side of
the 3rd floor might take on the added responsibility; access
may be a problem
ƒ Steve will ask Kathy Wolf and report back.
ƒ Winkenwerder list indicates both primary and alternate wardens for each
floor (Basement – 2nd) and each end (North or South). Thanks to Steve
for adding the alternates.
ƒ No staff/faculty present in 1st floor south
ƒ Suggested warden for the north end of 1st floor be asked to
cover
Discussion ensued and David reiterated a check should be made
ƒ
Steve will track down all potential wardens and provide Bloedel’s list by
our next committee meeting in March.
Steve West noted there were no Emergency Fire signs. Committee agreed they should be
replaced. Margie indicated the Building Coordination Committee was aware they needed to be
replaced after painting in Winkenwerder. It may only necessitate a request to F/S Works.
Margie will discuss this with the Building Coordinator and pursue the replacement of the signs in
Winkenwerder. …False Alarm – signs are posted…
Jay Herzmark, when asked confirmed UW EH&S provides helmets, vests and any other wardenrelated equipment necessary for effective warden identifications/protection.
David suggested once the lists are completed we should have a fire drill, further discussion to
follow at March meeting.
NOTE: Evacuation Warden/Building Evacuation training is available online at the EH&S
website https://www.ehs.washington.edu/psotrain/corsdesc.shtm#evacwarden This information
is also part of the UW Emergency Response plan.
David and Jay confirmed the on-line asbestos awareness training will be available
soon and David will email the committee at that time. Discussion ensued as to the
necessity of this training.
o Woodshop
ƒ Margie sent an email for interest and received minimal response. She will
find out from Building Coordination team when/if training will take place.
ƒ Confirmed to the committee CFR has scheduled maintenance for tools
through F/S Works.
New Business
o Topics for future meetings:
ƒ David asked about safety issues related to field trips and field research.
Discussion ensued that past issues dealt mostly with safe driving on forest
roads. May want to revisit and remind PIs, faculty that safety needs to be
addressed. David also thought there is a Field Manual that addresses
safety. The committee will discuss further at our next meeting.
ƒ Zareen asked Jay if EH&S provided lab use training and Jay said he could

Michael Merrill is asking each group to provide an up-to-date Health and Safety Plan for them to
have on file for L&I. She said date and disposition (whether a draft or final) did not matter, they
simply need a plan on file for their records. Discussion followed that ONRC, Pack Forest,
Arboretum, BGMC and other Seattle campus buildings probably have their own. Nothing was
established, discussion will continue at March meeting.

January 16, 2008
The meeting came to order at 9:15 am in Anderson 107A
Present: Neal Bonham , Margie Cooper, Zareen Khan , Mike Wetzel, David Zuckerman and
Jay Herzmark (EH&S Rep)
Absent: Kevin Hodgson and Steve West
• Review and approval of October and November 2007 minutes took place via email 12/20/07.
David moved to approve the minutes and Margie 2 nd the motion. Cecilia was asked to post
them online at the CFR Health & Safety Committee Meeting web site

Congratulations and welcome to our newly elected committee members, Zareen and Mike, and
our newly appointed faculty member, Kevin. Welcome back to committee members David and
Neal as well as former appointees Steve and Margie.
• New Business
• David Zuckerman was re-elected committee chair for the upcoming 2008-2009 term and
Margery Cooper was elected alternate-chair and recorder for CFR's Group 10 EH&S committee.
• David discussed Kevin's conflict with our current meeting time (hence his absence today) and
the committee agreed to reschedule the meeting time to 1-2pm, same third Wednesday of the
month.
• Established that two elected members and one appointed member would henceforth constitute
as a voting quorum for the committee.
• David discussed the committee member expectations (also enumerated online our CFR website
at EH&S Committee Mission ):
• i. act in an advisory capacity to the Dean of emergency health;
• ii. act as safety advocates within our own work environments;
• iii. review OARS reports/accidents during monthly hour long meetings
Quarterly OARS reports will now be prepared on a monthly basis, according the most recent
UW-wide meeting.

• Future accident/incident reporting summaries will also include the actual individual report
(with confidential info blacked-out) and we will be Beta testing a new OARS version.
• David reported the U-wide meeting had a Training power point presentation which is available
online along with meeting minutes on the UW EH&S website .
• David also recommended committee members attend the Asbestos Awareness Training and/or
the committee discussed having a presentation made to the committee as a group. David will
look into when the online training will be available .
• Each Group within the UW-wide EH&S have been directed to update and implement
Emergency Response plans including a new Section E for Workplace Violence. David to provide
more information as to the committee's deadline; need to communicate with Building
Coordinator (Sally) .
• Old Business Action Item
• Warden List – Steve following-up with “openings” and confirmed participants and a current
list of wardens by the next committee meeting. Warden training is available online at the EH&S.
This information is also part of the Emergency Response plan.
• Woodshop
• i. Training needed – Margie to send email for interest and contact trainer for availability
• ii. Assure CFR has scheduled maintenance for tools – Ask Bldg Coordinator (Sally)
