CFR Safety Committee Meeting

Present: Art Breitsprecher, Phil Hurvitz, Jay Johnson, Dave Zuckerman,
Brad Coston, Greg Brazil
The meeting came to order at 1:40
I. Reviewed minutes
1) April minutes are pending
2) May there wasn’t a quorum. No meeting was held.
II. New Business
1) The back of an employee at the Arboretum “popped”. No damage was reported. Employees
name is K.C. Morgan.
2) Art reported that by July 1, 2004 every job needs to be evaluated for ergonomic assessment.
It was unanimously voted to suspend meetings until October 1, 2003.

Absent: Kelley Duffield
The meeting came to order at 1:35
I. Minutes from the meeting held on February 5, 2003 were reviewed and accepted as revised.
II. The College of Forest Resources Safety Plan was discussed.
1) Greg distributed copies of The Guide to Departmental Health and Safety Plans
(HSPLAN.DOC) containing the EH&S outline of information required for compliance with the
State of Washington. Also distributed were copies of the CFR Emergency Response Guide
(specific to Anderson, Bloedel, Winkenwerder, the Plant Lab Annex and the Center for Urban
Horticulture.
2) There was discussion of the exemplary plan designed by the Wind River Canopy Crane and
how the other sites will create theirs.
3) Safety Coordinators responsible for the creation and implementation of the Health and Safety
Plan for their sites are:
o Pack Forest: Mason McKinley
o ONRC: Deric Kettel
o Canopy Crane: Mark Creighton
o Anderson Hall, Bloedel Hall, Winkenwerder Hall: Greg Brazil
o Arboretum: To be determined
o CUH: To be determined
III. Old Business
1) Dave Zuckerman reported that the lath-house at the Arboretum would be destroyed April 5,
2003.
IV. New Business
1) A student questioned Phil Hurvitz if the College would put signs on the doors indicating what
chemicals are in the room? This prompted conversation about the Laboratory Safety System
(LSS).
2) It was explained that LSS is a UW database that shows what chemicals are present at each
location. The database is to be updated by the Principal Investigator responsible for the assigned
lab space. Updates should take place at least annually.
3) It was suggested by Jay Johnson and agreed upon by the committee, this is an important
matter that should be directed to the Dean, who will inform the Division Chairs, who in turn,
would convey the information to the Principal Investigators.

The meeting came to order at 1:30.
I. Minutes from the meeting held on January 8, 2003 were reviewed and accepted as revised.
II. Eyewashes and Emergency Showers.
1) The State of Washington requires new eyewashes to be installed along with emergency safety
showers.
a) There were plans to install a shower in the hallway of Bloedel Hall, near room #178. If a
shower were installed at this location, when activated the excessive water would jeopardize the
server room as well as the library in the lower level of the building causing extreme damage and
expense.
b) It has been decided upon by the Project Manager, Ed Yoosefian, Carol Green with the Forest

Resources Library and the Department of Environmental Health and Safety to place the shower
in the hall across from the restrooms on the ground floor. The shower is to include a permanent
drainage system leading into the regular plumbing along with a basin in which to catch the water.
It will not have walls.
III. Old Business
1) Bob Edmonds is requesting that more signs be made and mounted at Pack Forest indicating
ALL DOGS MUST BE ON A LEASH.
2) Professor, Jay Johnson has been appointed to the Safety Committee by Dean Bare.
IV. New Business
1) There was no new business to report.

Minutes
Present: Art Breitsprecher, Brad Coston, Phil Hurvitz, Dave Zuckerman and Greg Brazil
The meeting came to order at 10:30.
I. Minutes from the meeting held on December 6, 2002 were reviewed for corrections. A motion
was made to approve the minutes as revised. The motion was seconded and approved.
II. Old Business
1) Sunshades were checked to confirm that the safety film was placed on the correct side of the
glass. It is.
2) Bob Edmonds checked for signs at Pack Forest indicating dogs should be leashed. There’s
one, however, there should be more. There was discussion that the other sites should be checked.
3) The Dean may appoint Jay Johnson for the Safety Committee.
III. New Business
1) The cedar lath-house at the Arboretum is considered unsafe. Fred Hoyt has contacted physical
plant regarding its demolition. It’s presently cordoned off with caution tape.
2) Neal Bonham at the Arboretum attended a West Nile Virus Seminar. There will be additional
information offered through the City of Seattle. Dave Zuckerman will keep up informed.
