1. Complete the Student Section of this form.
2. Ask your academic advisor or professor to complete the Advisor/Professor Section below.
3. Return the completed form to the OISS front desk.
4. Submit the Non-Face-to-Face Request to the OISS, if you are only enrolled in a non-face-to-face course in the given term.
5. OISS will email you at your my.emich account when your request is granted or if additional information is needed. Check email!

If you only need one course to finish your program of study, the course cannot be taken online. If your last course(s) is/are non-faceto-face only (including independent study/research/thesis/degree-required practicum),you are also required to submit the OISS “Request
for Non-Face-to-Face Class in Final Term” form. Your RHF will not be approved until all forms are complete and submitted to OISS.
Statement of Understanding
I understand that I have 60 days beyond my program end date (the end date of my I-20) to do the following. I understand that once the
60 days ends, my SEVIS record will be complete and I must leave the United States or file for reinstatement (if eligible):
- Submit an Optional Practical Training (OPT) application to the U.S. government. The U.S. government must receive my
OPT application before the 60th day following my I-20 end date.
- Be admitted to a new degree program at EMU and obtain a new I-20 with the new degree information before the 60th day
- Be admitted to a new degree program at another U.S. school and complete the transfer before the 60th day

The Department of Homeland Security (DHS) requires students in F and J visa status to maintain a full course of study (8 credit hours for
graduates/12 for undergraduates) during the fall and winter terms (and summer if it is their first or last term at EMU). However, DHS
recognizes that a reduced load is sometimes justified (subject to final approval by OISS) for the academic reasons below. Please review
the student’s program of study and confirm that he/she is in their final term by completing the section below. Please sign the form and
give it to the student for submission.

Things to consider BEFORE taking a Last Term

Academic
 Confirm your graduation with your academic advisor
o For graduate students- go over your program of study to ensure you have taken the necessary
classes and earned the necessary grades for your degree.
o For undergraduate students- you are able to conduct a degree audit via my.emich to see if
you have met the degree requirements. For more information, see
http://www.emich.edu/registrar/uachieve/
Immigration
 Confirm your next steps with the OISS within the 60-day grace period following
your last term’s end date
o F-1 students are allowed to stay in the United States for 60 days following their last term’s
end date. During these 60 days, you are also allowed to update your SEVIS record for
Optional Practical Training (OPT), a new degree program at EMU, transfer to another school,
or apply for a change of visa status. After these 60 days, your SEVIS record completes.
Please inform the OISS as soon as possible of your next steps and instructions. For more

o You are allowed to stay in the United States during the 60-day grace period. However, you
are not allowed to travel outside the United States and re-enter as an F-1 student. Nor are
you allowed to work on or off-campus campus without OPT authorization.
Financial
 Pay any outstanding tuition and university fees
You will not be able to receive your diploma or transcripts until you have a zero balance.

Pay particular attention to the application deadlines for graduation and application deadlines for name
inclusion in the commencement program.
