Variations in Care: Comparing Heart Failure Care Transition Intervention Effects
This mutli-site randomized controlled trial will: (1) compare the effect of implementing two separate care
transition interventions with concurrent controls on variation in readmissions among older patients
hospitalized with heart failure at six medical centers; (2) examine the change in variation over time in
readmissions and mortality among hospitalized Medicare beneficiaries with heart failure at the six medical

Translating the AHRQ Quality Indicators to the VA

Impact of Resident Work Hour Rules on Errors and Quality
The major goals of this project were to compare changes in the rates of mortality and failure-to-rescue
(death after complications) in teaching hospitals and non-teaching hospitals before and after
implementation of the ACGME work hour rules. The secondary goals were to examine differences in
AHRQ Patient Safety Indicators (PSIs) before and after the rule change and to study how length of stay
(LOS), the probability of a prolonged length of stay, and conditional length of stay (LOS once a stay is
prolonged) changed in teaching vs. non-teaching hospitals.

Identifying Patient Safety Indicators from Administrative Data
The objectives of this project were to: 1) apply and test AHRQ’s Patient Safety Indicators as a screening
tool for potential patient safety events in the VA; 2) validate the PSIs with chart-abstracted data from the
National VA Surgical Quality Improvement Program (NSQIP); 3) determine occurrence rates of PSI
events across VA facilities and VISNs; 4) identify patient-level, facility-level, and systems-level factors that
explain variation in rates of PSI events across VA facilities and VISNs; 5) compare the occurrence rates
of PSI events in VA and non-VA hospitals for benchmarking purposes; and 6) explore whether those

Information about Quality in a Randomized Evaluation
The objectives of this project are to: 1) identify factors associated with consumers' use of employerdisseminated information about health plan and medical group performance; 2) determine whether and
how consumers use such information during open enrollment; 3) use qualitative methods to explore why
consumers consistently request quality information, but then fail to use it; and why they claim to value
quality, but then base their decisions largely on price; 4) design and implement an intensive program for
disseminating quality information to receptive or "vulnerable" subpopulations; and 5) evaluate the impact
of this program by randomizing members of a large purchasing organization to receive existing quality
information with or without this supplemental program.

The Outcomes of Hospital Outcome Studies
The objectives of this project were to: 1) evaluate the impact of publicly released hospital outcomes data
on AMI, diskectomies, and deliveries in CA, and data on CABG and PTCA in NY, on hospital volumes
and referral patterns; 2) evaluate whether this impact varies by age, gender, race/ethnicity, insurance
source, diagnostic category, urgency of admission, or market characteristics; 3) explore how hospitals in
CA and NY have used outcomes data, and to solicit feedback concerning the optimal content and format
of outcomes reports; 4) explore how other interest groups, such as consumer organizations and
purchasers, have used outcomes data, and to solicit similar feedback; and (5) to apply these results in
developing a new generation of hospital outcome studies that will be more responsive to the needs of
