Annual Faculty Performance Evaluations
Effective Date: December 2001
Page No. 1 of 4
GENERAL PURPOSE:
This policy is intended to provide a process and procedure that will insure the annual,
routine, fair and equitable evaluation of instructional faculty at the college.
BACKGROUND:
It is the policy of MJI that all members of the faculty who taught courses during the
nearly or just concluded academic year will be evaluated on their classroom
management and teaching performance during the past academic year. Evaluations will
be based on a variety of observations and documents. These include but are not limited
to: submitted evaluations by students; quality check site visits to each class; personnel
file notes; annual development plans; and, any other relevant sources as may be
available from time to time and for individual faculty members.
PROCESS:
During and/or at the end of the spring semester each year, those supervisory staff (e.g.,
Deans or Department Chairs) assigned to evaluate faculty will be required to complete
an annual faculty evaluation form for each of their faculty subordinates. Deans who have
teaching responsibilities will be evaluated by the college president.
The Dean of Academic Administration will provide evaluators with a current evaluation
form template which they will use to prepare their evaluations. A copy of the evaluation
form to be used is attached at the end of this policy statement. It is self-explanatory.
Completed evaluation forms are to be signed by the evaluator and the Dean of Academic
Administration. Three signed copies of the completed evaluation form will be prepared.
One copy will be “temporarily placed in the faculty member’s personnel file folder. The
remaining two copies are to be delivered to the faculty member with an appropriate
cover memo and a self-addressed, stamped envelope. The evaluated faculty member is
asked to sign one of the two evaluation forms (indicating receipt but not necessarily
agreement with the evaluation) and return it to MJI via the provided envelope. Once the

countersigned form is received by MJI it will replace the “temporary” form already on
file in the faculty member’s folder (the temporary form is then shredded for the purpose
of confidentiality). Should the faculty member not return a countersigned copy of
the evaluation, the “temporary” form will be considered permanent and will remain in
the faculty member’s personnel folder.
The evaluation form also contains space for any comments the faculty member wishes to
add to the form.
The annual faculty performance evaluation becomes a part of the faculty member’s
permanent employment file.
