Specialty Pharmacy Oncology Service Medication Adherence Program For Oral Chemotherapy Welcome to the Medication Adherence Program. Oncology Pharmacists specialize in oncology medications and infusion therapy. We work together with your oncologists, nurse practitioner, nurses, and other health care providers to ensure safe medication use. We have seven Board Certified Pharmacotherapy and Oncology Specialist Pharmacists on site. How the program works We offer clinic appointments with an oncology pharmacist to review your oral chemotherapy treatment. To make sure our patients understand their treatment, we check on them at home between clinic visits to help with issues they may be experiencing. At the first meeting, we will review • • • • • • The purpose of their medicine How to take their medicine and special administration instructions The importance of medication adherence and what to do when you miss a dose Common and rare side effects for your medicine and how to handle them Review your medication list for potential drug interactions Answer other medication-related questions After the first meeting, our pharmacy team will call you once a month. They check on your progress, answer any questions, and prepare your next refill. We offer free of charge, mail delivery of your oral chemotherapy directly to your home. During the telephone calls, we will • Make sure you are taking your medicine as prescribed • Ask you if you have missed any doses. Provide tips to help you remember to take your medicine. • Ask if you are experiencing new side effects. • Review relevant lab results for your specific medicine. • Work with your oncologist for any new issues.
