Alternate Exam Time Request Form for M261
This form is due Monday, 10 days before the exam date. For those working with
RDS, this form need be filled out only once during the semester. For other students, it must be filled out separately for Exams 1,2,3. Do not turn in the form more than two weeks before the exam. Late requests may not be considered. The only instances in which a student will be considered for an alternate exam time are:
• another class scheduled for the same time
• University approved activity scheduled for the same time
• documented serious illness or documented death in the immediate family
No alternate testing time will be authorized without use of this form.

7:00 to 8:15 pm. on the day the exam is scheduled. Since an override was provided for you to enroll in Math261 this is the only alternative time allowed.

Use of this form does not guarantee the right to an alternate testing time. Students are expected to take the exam the same day as it is being offered unless their class schedule (or documented reason above) makes it impossible to do so.
I will take the exam on the same Thursday from 4 to 5:15pm

I am unable to take the exam at the alloted time for the following reason. (Attach appropriate documentation.) I will either put an additional 100 points on the final exam or I will take a different exam on the following days:
(i.) For Exam 1, it is the Wednesday one day before Exam 2. I understand that course coordinater, Dr. McArthur, must be informed exactly on Wednesday of the seventh week of classes that I will be taking a makeup Exam 1 in the PAC(e)
Center (or RDS for special cases). I will let her know via email with subject:
Math261: Makeup Exam 1. If email is not received then the Final Exam will automatically be worth an additional 100 points.
(ii.) For Exam 2, it is the Wednesday one day before Exam 3. I understand that course coordinater, Dr. McArthur, must by informed exactly on the Wednesday of the eleventh week of classes that I will be taking a makeup Exam 2 in the PAC(e)
Center (or RDS for special cases). I will let her know via email with subject:
Math261: Makeup Exam 2. If email is not received then the Final Exam will automatically be worth an additional 100 points.
(iii.) For Exam 3, it is the Friday of the fourteenth week of classes. I understand that course coordinater, Dr. McArthur, must be informed exactly on Wednesday of the thirteenth week of classes that I will be taking a makeup Exam 3 in the PAC(e)
Center (or RDS for special cases) on Friday of the fourteenth week of classes. I will let her know via email with subject: Math261: Makeup Exam 3. If email is not received then the Final Exam will automatically be worth an additional 100 points.
With respect to final exams, a student is not expected to take more than two that are regularly scheduled (with no registration overrides) on one day. Common final exams take precedence over other exams. For those taking two math courses with common finals, an override was necessary to enroll in both. As a result, it is expected that both finals will be taken on the day they are scheduled for. If the two courses are Math261 and
Math340 then the regular time will be for Math340 and an alternate time for Math261. If there is a combination of Math261 with another math course that has the same common final exam time, then Math261 takes precedence.
