Priority applications for the Institute are open Sat., August 1, 2015 and due on or before Sun., Nov 1, 2015. Are you willing to take charge of your career growth and development? 4. Does your schedule allow you to attend the entirety of ALL requirements for Emerging Nurse Leader Institute? 5. Are you in a job that provides direct patient care such as a CN II, CNIII or ANII, or equivalent? If you answered YES to all of the above questions, you are eligible to apply for the Rising Nurse Leader Institute at this time. A PPLYING This document outlines essential information needed to complete the Rising Nurse Leader Institute application. The on line application is available at https://www.surveymonkey.com/r/J3YTJCK. Carefully follow the instructions below to complete the on line application. In addition to the on line application, please submit requested supporting documents (Letter of Recommendation and optional Resume/CV) in a PDF format electronically to Kathleen.guiney@ucdmc.ucdavis.edu or fax # 916703-9903/9904. Admission to the program will be determined by a blind review (with no name or identifying information attached) and the applicants will be evaluated by three nurse reviewers. P RIORIT Y D EADLINE Applications received by 11:59 (PDT) Sat., Oct 17th, 2015 will be considered for priority review and consideration for admittance with priority acceptances announced in midNovember, 2015. The final application deadline is 11:59 p.m. (PDT) Sun., Nov 1, 2015 with acceptance announced mid-December. Please note that completed applications received between Oct 17th and Nov 1 will be reviewed on a space-available basis only. Once the cohort is full, we will accept applications for the wait list only. The online application cannot be saved between sessions therefore; it must be completed in one single session. The application requests basic demographic information, a leadership checklist and fours short essay questions listed below. • • • • Tell me about the best supervisor you have worked with. Why did his/her management style work so well for you? What are your career goals? Tell me about the most powerful gesture of recognition you have ever received. Why was that recognition so meaningful to you? Please describe some of your talents, skills, and knowledge you would like to share with others. What is your greatest talent? You may type your answers to the essay questions on a word document and paste them directly into the online application. After you familiarize yourself with the application, it is important to start preparing certain requirements that take additional time such as a letter of recommendations. 1. Please give your recommenders as much advanced notice as possible. Therefore, we suggest you request a letter of recommendation as soon as you decide to apply to the program. The recommendation should come from someone (c0o-worker, supervisor, and colleague) that knows you well and can speak to your work and qualifications for the program. It is important to follow up with the recommender to make sure they completed your letter by the deadline. 2. Prepare your resume or CV (optional). Include any leadership roles, professional activities, research involvement at any level, teaching experiences (informal and formal), community service, projects and achievements. Save the resume in a PDF format that can be easily downloaded and electronically transmitted.
