AFFIDAVIT STATEMENT OF ASSURANCES
We, __________________________ (Name of School), consistent with N.J.A.C. 12:412.4, shall ensure that education and training programs are designed to enable all adult
students to demonstrate the knowledge and skills specified in the programs approved by
the New Jersey Departments of Education and Labor and Workforce Development. We
shall ensure the quality of education or training, facilities and operating practices through
the use of nationally recognized standards of operations. We, further, shall ensure the
quality of instruction and program(s) established in the areas of curricula,
faculty/instructors, facilities, equipments, supplies, fiscal and administrative capacity,
student support services, practices in recruitment and admissions, publication of
academic calendars and catalogs, grading policy, program length, tuition and fees in
relation to subject matter taught and the objectives of the degrees or credentials offered,
measures of program length in clock hours, student achievement, acceptable student loan
default rates, resolution of student complaints and compliance with the school’s
responsibilities under Title IV.
The applicant assures that it will comply fully with the nondiscrimination and equal
opportunity provisions of the following laws: Section 188 of the Workforce Investment
Act; Title VI of the Civil Rights Act of 1964, as amended; Section 504 of the
Rehabilitation Act of 1975, as amended; the Age Discrimination Act of 1975, as
amended; and Title IX of the Education Amendments of 1972. In addition, the
vendor/applicant also assures that it will comply with the US Department of Labor’s
regulations at 29 CFR, Part 37 and all other regulations implementing the laws listed
above.
