SUMMER 2016 CAL GRANT EVALUATION REQUEST

Student: Complete this application if you wish to be considered for a Cal Grant for the summer 2016 term. You must also meet
the following conditions:
• Be an undergraduate student who has not yet completed a first bachelor’s degree.
• Be enrolled at USC at least half-time for the summer term, and be considered eligible for the Cal Grant by USC.
• File a 2015–2016 FAFSA before June 30, 2016, with USC’s Federal School Code (001328) indicated, if you have not already done so.
• File a 2016–2017 FAFSA before June 30, 2016, with USC’s Federal School Code (001328) indicated, if you have not already done so.
Additionally, to determine Cal Grant eligibility prior to the summer term, check the box below that applies to you:
I received a Cal Grant for the fall 2015 semester, but I did not receive an award for the spring 2016 term at USC or any other institution.
I was newly admitted for the spring 2016 semester and received a Cal Grant for that semester, but I did not receive a Cal Grant for the fall 2015
semester at any other institution.
I plan to graduate early and would like to use my Cal Grant eligibility this summer.
