All information contained in the gang related incident offense report shall be forwarded to the Superintendent of State Police for inclusion in the Uniform Crime Report. All incidents must be reported where Gang Activity or a Known Gang Activity is involved. • Including Part I and Part II Offenses with or without an arrest involved. • Contact with known Gang Members. * AT LEAST TWO (2) GANG MEMBER CRITERIA CODES ARE REQUIRED. SCENARIOS THAT WOULD REQUIRE FORM COMPLETION (FORM # S.P. 153): • SUBJECT IS ARRESTED FOR THE AGGRAVATED ASSAULT OF A PERSON. THE INCIDENT WAS FOUND TO BE GANG RELATED WITH TWO OF THE LISTED CRITERIA INVOLVED. • SUBJECT IS ARRESTED FOR DWI AND FOUND TO BE A GANG MEMBER BASED ON TWO OF THE LISTED CRITERIA. • SUBJECT IS QUESTIONED AS A SUSPICIOUS PERSON AND IS FOUND TO BE A GANG MEMBER BASED ON TWO OF THE LISTED CRITERIA. • GANG GRAFFITI IS OBSERVED AND DOCUMENTED AS CRIMINAL MISCHIEF - NO ARREST IS MADE StreetGangIncidentOffense Page1of1 5/30/2013
