Background
The intersection of Old Town House Road/Forest Road is within the Top 5% of High Crash Locations on
Cape Cod. The intersection is currently at the 25% Design stage, paid for by the Town of Yarmouth, and
is eligible to receive Highway Safety Improvement Program (HSIP) funding if a Road Safety Audit
(RSA) is conducted and the proposed design incorporates the safety improvements as identified as part of
the RSA.
Due to a high number of crashes at the intersection of Old Town House Road/Forest Road, the southern
leg of Forest Road, which intersected Old Town House Road at an acute angle, was reconstructed in
recent years. The reconstruction resulted in offsetting the southern leg of Forest Road approximately 225
feet to the east along Old Town House Road, to intersect at 90 degrees, creating two offset intersections;
however, the intersection continues to experience a high level of crashes.
Project Data
The audit team conducted an RSA for the intersection of Old Town House Road/Forest Road in
Yarmouth, Massachusetts, on Monday, December 14, 2009. The RSA agenda appears in Appendix A.
Table 1 lists the audit team members and their affiliations. Appendix B provides contact information for
all team members.

Project Location Description
The intersection of Old Town House Road/Forest Road is located in the Town of Yarmouth, as shown
in the aerial image in Figure 1. Along Old Town House Road, the mix of residential and commercial
uses includes a small shopping plaza at the northeast corner of the intersection, and Bayberry Hills Golf
course to the northwest of the intersection. The Old Town House Road Park and Recreation area is
located on the north side of Old Town House Road; pedestrian and vehicle access to the area is provided
on Old Town House Road approximately 750 feet east of Forest Road. The northern leg of Forest Road
provides access to the Town of Yarmouth Solid Waste Facility, including the transfer station used by
Town residents. The Yarmouth Senior Center is located approximately 800 feet south of the intersection;
thus the driver population in the area is generally older than is typical.
Prior to the RSA, in order to begin assessing possible safety issues, the team reviewed collision diagrams
and a crash detail summary based on crash records supplied by the Yarmouth Police Department. Of
16 crashes related to the intersections from 2006 through 2009, nearly 70% were angle crashes. Crashes
typically occurred on dry pavement (75%) and during daylight hours (88%). It is notable that the age of
drivers involved in these crashes ranges between 18 and 103, with approximately 39% of all crashes
involving drivers over the age of 60. Crashes most commonly occurred when vehicles exited Forest Road
from the north and south onto Old Town House Road. Appendix C provides the detailed intersection
crash data from 2006 through 2009. Other intersection data such as traffic volumes are presented in
Appendix D.
The intersection is the junction of the following roadways, which are categorized according to MassDOT
Executive Office of Transportation Office of Transportation Planning functional classifications:
Old Town House Road is an urban minor arterial roadway and is Town-owned. Old Town House Road
runs east–west, connects West Yarmouth Road and Nightingale Drive in Yarmouth, and has one travel
lane in each direction. To the east of Forest Road, Old Town House Road has two raised planted islands
separating the eastbound and the westbound directions; these were installed in recent years for traffic
calming purposes. Per the speed regulations, the speed limit on Old Town House Road at the intersection
with Forest Road is 45 miles per hour (mph) in both directions. However, no speed limit signs are posted
in the vicinity of the intersection.
Forest Road is an urban minor arterial to the south of Old Town House Road, and a local roadway to the
north of Old Town House Road. Forest Road is Town-owned and provides a north–south connection
between Main Street (Route 28) and the Town of Yarmouth Solid Waste Facility. South of Old Town
House Road, per the speed limit regulations, the speed limit on Forest Road is 25 mph in the northbound
direction and 45 mph in the southbound direction. A shared-use path is provided along the east side of
the roadway between Route 28 and Old Town House Road, where it terminates. North of Old Town
House Road, Forest Road functions as a driveway for the Solid Waste Facility. An east–west bicycle path
intersects Forest Road approximately 400 feet north of Old Town House Road.

Road Safety Audit Observations
Safety issues and possible enhancements were determined based on RSA team discussions and field
observations on Monday, December 14, 2009. The safety issues identified include:









Complexity of the intersection geometry;
Overgrown and/or poorly located landscaping elements;
Access management;
Unclear, incorrect, and insufficient signage;
Faded pavement markings;
High percentage of older drivers;
Vehicle queuing during peak demand periods at transfer station;
Lack of pedestrian accommodations; and
Lack of bicycle accommodations.
Several of these issues require further study and engineering judgment in order to determine the
feasibility of implementing enhancements to address them.
The following sections describe in more detail the safety issues and enhancements determined during the
RSA.

Safety Issue #1. Intersection Geometry
Observation:
At this intersection, nearly 70%
of the crashes from 2006
through 2009 were angle
crashes. Police noted that most
of these crashes occurred when
one of the drivers failed to yield
the right-of-way. Field observations indicated that the offset
geometry, combined with
unclear lane usage and sight
distance limitations, may result
in driver error when
maneuvering through the intersection (e.g., driver may
misjudge the gap or does not see
the 2nd car around a left-turning
vehicle).

The presence of the shopping plaza driveway on Old Town House Road and the close proximity
of the Forest Road southbound approach may also lead to driver confusion by making it difficult
for drivers to determine who has the right of way within the intersection. In addition, westbound
left-turning vehicles on Old Town House Road block sight lines of eastbound vehicles turning left
into the shopping plaza.

Two of the crashes (approximately 13%)
were rear-end crashes, both of which
occurred on Forest Road. According to
Yarmouth Department of Public Works
(DPW) staff, the peak demand at the
transfer station occurs between 10:00
a.m. and 2:00 p.m. on Saturdays and
Sundays (particularly during the Fall
season) which can result in periodic
queue build-up on Forest Road southbound. Rear-end crashes may be a
result of such queuing conditions and
also attributed to driver indecision about
when it is safe to enter the intersection
(e.g., vehicles stop short). The Forest
Road northbound approach is also
susceptible to rear-end crashes.

An RSA team member noted that, in general, there are too many places for a motorist to look for
oncoming vehicles when entering and/or navigating through the intersection.
Enhancements:
1. Reconstruct the intersection to remove the offset alignment of Forest Road and to
simplify the movements (i.e., consolidate two Forest Road intersections into one) and
clarify control (i.e., signalization).
2. Consider vehicular movements to/from the shopping plaza driveway southbound onto
Old Town House Road to right-in/right-out only.
3. Consider vehicular movements to/from Constance Avenue in the redesign.
4. Provide adequate turning radii to accommodate emergency vehicle (e.g., fire truck)
access through the intersection and to the shopping plaza.
5. Introduce a traffic signal to reduce angle crashes, clarify control, and better serve
concentrated peak demand generated by the transfer station.
6. New traffic signal should include emergency pre-emption.

Safety Issue #2. Pavement Markings
Observation:
Pavement markings at the intersection
are either worn or missing—adding to
the confusion of motorists accessing
Old Town House Road from Forest
Road or the shopping plaza driveway.
Field observations indicated that the
lack of pavement markings makes it
difficult for motorists to determine if
vehicles traveling westbound on Old
Town House Road are turning left or
going through the intersection.
Forest Road northbound consists of an
exclusive left-turn-only lane and an
exclusive right-turn-only lane; however,
it is unclear which lane motorists should
use to continue through the intersection
to the shopping plaza driveway.

Safety Issue #3. Sign Improvements
Observation:
The RSA team noted that signage is generally insufficient at the intersection and existing signs do
not meet current reflectorization standards. Intersection warning and speed limit is not provided
in the vicinity of the intersection.
During the field visit, the RSA
team noted that signs on Forest
Road northbound at Constance
Avenue are incorrect.
Pedestrian crossing signage is
missing on the Old Town House
Road westbound approach.

crossing signage on the Old Town

and westbound approaches in the short-term (unsignalized condition).
2. Install pedestrian crossing warning signage on the Old Town House Road westbound
approach in the short-term (unsignalized condition).
3. Replace signage on the Forest Road northbound approach at Constance Avenue with
W2-2L installed on right side of Forest Road northbound in advance of intersection with
Constance Avenue.
4. Upgrade all existing signage with new reflectorized signs to meet standards of
consistency with the MUTCD.

Safety Issue #4. Sight Distance
Observation:
Trees located along the edge of Old Town House Road inhibit sight lines on all of the intersection
approaches and block visibility of warning and guide signage (e.g., pedestrian crossing signage
on Old Town House Road eastbound approach) as well as pedestrians waiting to cross the
roadway.
The RSA team also noted that
the Garden Club planting area,
located on the southwest corner
of the intersection, may block
sight lines during
Spring/Summer. Similarly, the
wood posts located on the
southwest corner of the
intersection, which previously
supported a guard rail, also
block sight lines and should be
removed.
The RSA team noted that
vehicles in the two Forest Road
northbound approaches block
each others’ sight lines.

should be relocated or removed. In the short-term, all trees in the vicinity of the
intersection should be trimmed to improve sight lines.

3. The wood guard rail posts along the south side of Old Town House Road should be
removed.

Safety Issue #5. Pedestrian Accommodations
Observation:
The RSA team members noted that several nearby residents walk to the Old Town House Road
Park and Recreation area and the commercial uses. Pedestrian accommodations are provided
along the south side of Old Town House Road where they can then access the recreation area by
using an unsignalized crosswalk located at the recreation area driveway (approximately 750 feet
to the east of Forest Road). However, an RSA team member indicated that pedestrians
occasionally do not follow this route, instead crossing Old Town House Road at the intersection
with Forest Road; no crosswalk is provided across the eastern leg of the intersection.
The RSA team members also noted that in absence of a sidewalk, a pedestrian footpath has been
created along the north side of
Old Town House Road between
shopping plaza driveway and the
Old Town House Road Park and
Recreation area.

1. Consider installing a crosswalk across the eastern leg of the intersection if signalized.
2. Install sidewalk along the north side of Old Town House Road between the shopping
plaza and the Old Town House Road Park and Recreation area entrance.

Safety Issue #6. Bicycle Accommodations
Observation:
A shared-use bicycle path is provided along the east side of the Forest Road, providing a north–
south connection between Route 28 and Old Town House Road, where it terminates. A second
shared use bicycle path runs east–west through the area and crosses the northern leg of Forest
Road approximately 400 feet north of Old Town House Road; however, no connection between
the two bicycle paths is provided at the intersection.
During the field visit, the RSA team noted that these segments would be connected in the future
to the regional bicycle path, the Cape Cod Rail Trail, which would likely result in an increase in
bicycle and pedestrian activity in the area.

conflicts between bicycle users
and transfer facility traffic
(passenger cars and trucks) –
both of which have activity
periods on the weekends during
the midday. Visibility of
cyclists and pedestrians at this
crossing is inhibited due to the
presence of signage for the
transfer station posted on the
gate.

1. Redesign of the intersection should incorporate a bicycle connection between the two
existing bicycle paths. Installation of a traffic signal, and a bike lane or path, would
provide a safe connection
2. The alignment of the bicycle path across Forest Road near the transfer facility on the
north side of Old Town House Road should be modified to minimize conflicts with
transfer facility traffic. In the short-term, the transfer facility signage posted on the fence
should be relocated to improve cyclist and pedestrian visibility.

Safety Issue #7. Speed
Observation:
Per the speed regulations, the speed limit on Old Town House Road in the vicinity of the
intersection is 45 miles per hour (mph) in both directions; however, no speed limit signs are
posted in the vicinity of the intersection. An advisory speed limit sign for 30 mph is posted in
the eastbound direction of Old Town House Road approximately 300 feet east of the intersection
in advance of the recreation area driveway. RSA team members noted that the 45 mph speed
limit may be too fast for the area and could contribute to the high incidence of crashes at the
intersection.
Enhancements:
1. Conduct speed study to determine the prevailing speeds and adjust as necessary based on
findings.
2. Post speed limits.

Recommendations
Based on its observations and discussions, the RSA team identified the issues and possible enhancements
that could improve the safety at the intersections of Old Town House Road/Forest Road in Yarmouth,
Massachusetts.
Short-term recommendations include:
•
Re-striping and maintaining pavement markings.
•
Providing adequate regulatory and warning signs for drivers.
•
Upgrading existing signage with new reflectorized signs.
•
Trimming trees along the edge of Old Town House Road.
•
Relocating or removing existing transfer facility signage at the bicycle path crossing.
To enhance the safety of the intersection of Old Town House Road/Forest Road, the long-term
recommendation is to improve the intersection geometry, manage driveway access, and install a traffic
signal. Further study and design work needs to be conducted to determine the feasibility of making
improvements to the intersection.
Table 2 summarizes these safety issues, possible enhancements, estimated safety payoff, time frame, cost,
and responsibility. Safety payoff estimates are based on the approximate percent of crashes reduced by
enhancement and are categorized as low (<30%), medium (30% to 70%), and high (>70%). The time
frame is categorized as short-term (<1 year), mid-term (1 to 3 years), or long-term (>3 years). The costs
are categorized as low (<$10,000), medium ($10,001 to $50,000), or high (>$50,000). It is the
responsibility of the Town of Yarmouth (local jurisdiction) to ensure that the designer incorporates the
relevant safety enhancements identified as part of this RSA.

Instructions for Participants:
• Before attending the RSA on December 14th, participants are encouraged to drive
through the intersection and complete/consider elements on the RSA Prompt List
with a focus on safety.
• All participants will be actively involved in the process throughout. Participants
are encouraged to come with thoughts and ideas, but are reminded that the
synergy that develops and respect for others’ opinions are key elements to the
success of the overall RSA process.
• After the RSA meeting, participants will be asked to comment and respond to the
document materials to assure it is reflective of the RSA completed by the
multidisciplinary team.
