Background
The Federal Highway Administration (FHWA) defines a Road Safety Audit (RSA) as the formal safety
examination of an existing or future road or intersection by an independent, multidisciplinary team. The
purpose of an RSA is to identify potential safety issues and possible opportunities for safety
improvements considering all roadway users. This RSA evaluates the following intersections on Hartford
Avenue (Route 126) in Bellingham, MA, as shown in Figure 1.

A safety audit was scheduled for these intersections as part of the required MassDOT design process for
the four high crash clusters on Hartford Avenue (Route 126). The intersection of Hartford Avenue (Route
126) at Stallbrook Market Place/Charles River Plaza ranks 50th on the MassDOT top 200 intersection
crash clusters from 2009-2011. The intersection and surrounding area adjacent to the I-495 southbound
ramps ranks 165th on the MassDOT top 200 crash clusters from 2008-2010 and was identified as a
Highway Safety Improvement Program (HSIP) intersection for 2009-2011. The intersection of Hartford
Avenue (Route 126) at N. Main Street was ranked 179th on the MassDOT top 200 intersection list from
2007-2009 and was identified as an HSIP intersection for 2009-2011.
A key objective of the RSA is to identify both short-term and long-term safety improvements that can be
made at the subject intersections.
Project Data
A Road Safety Audit was completed for the study area intersections abutting Hartford Avenue (Route
126) in the Town of Bellingham on September 11, 2014. The agenda for the RSA meeting held at the
Bellingham Municipal Center is provided in Appendix A of this report. As shown below in Table 1, the
audit team consisted of a multidisciplinary team with representatives from state, regional and local
agencies providing expertise in the engineering, planning, maintenance and emergency response fields.

Within the email invitation sent on August 7, 2014 to
each participant in the RSA, background material was
provided. This information included collision
diagrams, MassDOT crash data summaries, and speed
regulations for the study area intersections. During the
RSA meeting, these materials were reviewed as a
group prior to the field visit to the intersections.
During the RSA field visit, various safety issues were
observed and identified. Following the RSA field
visit, the team returned to discuss additional concerns
and potential solutions for the existing safety issues.

Project Location and
Description
Study Area Roadways
As shown in Figure 1 Hartford Avenue (Route 126) is a four-lane roadway that runs in the east-west
direction through the project study area and in the Town of Bellingham with a posted speed limit of 25
miles per hour to the west and a speed limit of 40 miles per hour to the east of Crossroads Plaza. Hartford
Avenue (Route 126) is classified as an urban principal arterial under MassDOT jurisdiction between the I­
495 Northbound ramp and the Crossroads Plaza intersection west of I-495. At the intersections with
North Main Street and Stallbrook Market Place/Charles River Center it is under Town jurisdiction.
Additionally, to the west of North Main Street, Hartford Avenue is no longer a state numbered route as
Route 126 continues south on North Main Street. The travel lanes on Hartford Avenue vary in width
throughout the corridor, but are typically between 10-12 feet wide. A sidewalk that is approximately five
feet wide is generally present along the northern side of Hartford Avenue (Route 126) through the study

area. A sidewalk is also present on the southerly side of Hartford Avenue from North Main Street to the
Hess gas station driveway.
Interstate 495 is a six-lane divided interstate under MassDOT jurisdiction that generally runs in a
north/south direction through the study area in Bellingham. I-495 begins at the junction with I-195 and
Route 25 in Wareham, MA and extends northwest towards Worcester, making a broad loop around
greater Boston before turning northeast and linking to I-93 north of Boston in Andover. The exit from I­
495 to Hartford Avenue (Route 126) is Exit 18, which connects to Hartford Avenue (Route 126) in both
the eastbound and westbound directions via signalized intersections with the off-ramps. The median
dividing the I-495 northbound and southbound lanes is approximately 175 feet wide and consists of grass
and shrubbery. Both the northbound and southbound through lanes are 12 feet wide with 11 foot
shoulders along the outside edge and five foot shoulders along the median edge. The shared
acceleration/deceleration lane is approximately 11 feet wide for both the northbound and southbound
lanes. The posted speed limit along I-495 is 65 mph in either direction.
North Main Street (Route 126) is a two-lane roadway that begins at Hartford Avenue to the north and
extends south through the Town of Bellingham to Route 140. North Main Street is classified as an urban
principal arterial under Town of Bellingham jurisdiction. North Main Street provides one lane of travel in
either direction for the majority of the roadway, but provides two auxiliary turn lanes at the intersection
with Hartford Avenue. North Main Street has sidewalk present on both sides of the roadway.
Cedar Hill Road, Rawson Road, and Deerfield Lane are all local roads under Town of Bellingham
jurisdiction. Cedar Hill Road is a one-way roadway connecting to Hartford Avenue opposite North Main
Street with one-way circulation allowing vehicles to exit Cedar Hill Road to Hartford Avenue. Rawson
Road and Deerfield Lane both connect to Hartford Avenue to the south and terminate in cul-de-sacs to the
north. All three of these roadways provide access to residential properties. Cedar Hill Road and Rawson
Road both have sidewalk on the eastern side of the roadway only while Deerfield Lane provides sidewalk
on either side of the roadway.
Study Area Intersections
The intersection of Hartford Avenue (Route 126) at North Main Street (Route 126)/Cedar Hill Road is a
four-legged signalized intersection maintained by the Town of Bellingham. North Main Street provides an
exclusive right turn lane and left turn lane. Northbound through movements from North Main Street are
prohibited to enforce one-way circulation on Cedar Hill Road. Cedar Hill Road provides an exclusive left
turn lane and a shared through and right turn lane. The eastbound approach on Hartford Avenue consists
of an exclusive through lane and a shared through and right turn lane. The westbound approach consists
of an exclusive left turn lane and an exclusive through lane. To the west of this intersection, Hartford
Avenue transitions to a two-lane roadway. Crosswalks are provided for all approaches with pushactivated pedestrian crossings. The traffic signal phasing provides a lead westbound left turn phase, a
phase for eastbound and westbound through movements on Hartford Avenue (Route 126) with permissive
westbound left turns, an exclusive pedestrian phase, and a shared northbound and southbound phase with
permissive left turns.
The intersection of Hartford Avenue (Route 126) at Crossroads Shopping Center/Rawson Road is a fourlegged signalized intersection maintained by the Town of Bellingham. The Crossroads Shopping Center
Driveway provides a shared left turn and through lane and two exclusive right turn lanes. The southbound
approach from Rawson Road provides a single multi-use lane. The eastbound and westbound approaches
on Hartford Avenue (Route 126) consist of an exclusive left turn lane, an exclusive through lane, and a
shared through and right turn lane. Crosswalks are provided along the eastbound and southbound

approaches with a push-activated pedestrian crossing for the Hartford Avenue (Route 126) crossing only.
The traffic signal phasing provides a lead protected left turn phase for eastbound and westbound traffic,
an eastbound and westbound through phase on Hartford Avenue (Route 126) with permissive westbound
left turns, an exclusive pedestrian phase, and a shared northbound and southbound phase with permissive
left turns.
The intersection of Hartford Avenue (Route 126) at the I-495 southbound ramps/Deerfield Lane is a fourlegged signalized intersection maintained by MassDOT. The eastbound approach on Hartford Avenue
(Route 126) consists of an exclusive left turn lane for traffic entering Deerfield Avenue, two exclusive
through lanes, and an exclusive channelized right turn to the I-495 southbound on-ramp. The westbound
approach consists of a shared left turn and through lane and a shared through and right turn lane. The I­
495 southbound off-ramp to Hartford Avenue (Route 126) has two exclusive left turn lanes and an
exclusive channelized right turn lane that is yield controlled. The ramp is median divided allowing left
turns from Hartford Avenue (Route 126) westbound to access I-495 southbound. Deerfield Lane provides
a single multi-use lane. An unsignalized crosswalk is provided spanning Deerfield Avenue to connect the
sidewalk on the northern side of the road. The traffic signal phasing provides a lead eastbound left turn
phase, an eastbound and westbound through phase on Hartford Avenue (Route 126) with permissive
westbound left turns followed by a lagging westbound phase for protected left turns. The I-495 off-ramp
and Deerfield Lane each have their own protected signal phases.
The intersection of Hartford Avenue (Route 126) at the I-495 northbound ramps is a three-legged
signalized intersection maintained by MassDOT. The eastbound approach on Hartford Avenue (Route
126) consists of two exclusive through lanes and an exclusive channelized right turn to the I-495
northbound on-ramp. The westbound approach consists of an exclusive left turn lane accessing the I-495
northbound on-ramp and two exclusive through lanes. The I-495 northbound off-ramp to Hartford
Avenue (Route 126) provides two exclusive left turn lanes and a channelized right turn lane that is
signalized and restricts right turns on red. The ramp is median divided allowing left turns from westbound
Hartford Avenue to access I-495 northbound. There are no crosswalks or pedestrian crossing amenities at
this location. The signal phasing provides a lead protected phase for the westbound left turn, a phase for
the eastbound and westbound through movements on Hartford Avenue (Route 126) with permissive
westbound left turns, and a phase for the northbound I-495 northbound off-ramp.
The intersection of Hartford Avenue (Route 126) at Stallbrook Market Place/Charles River Center is a
four-legged signalized intersection maintained by the Town of Bellingham. The eastbound approach on
Hartford Avenue (Route 126) consists of two exclusive left turn lanes into Stallbrook Market Place, two
exclusive through lanes, and exclusive right turn to the Charles River Center. The westbound approach
consists of an exclusive left turn lane to the Charles River Center, an exclusive through lane, and a shared
through and right turn lane. The Stallbrook Market Place and Charles River Center driveways both consist
of a shared left turn and through lane, and an exclusive right turn lane. Crosswalks are provided for all
approaches with push-activated pedestrian crossings. The signal phasing provides a lead, protected
eastbound and westbound left turn phase, an eastbound and westbound through phase on Hartford Avenue
(Route 126), an exclusive pedestrian phase, and a northbound and southbound phase with permissive left
turns.
The area surrounding the audit intersections primarily consists of commercial land uses with several
abutting driveways along Hartford Avenue (Route 126). As shown in Figure 2, there is a Mobil gas
station to the west of North Main Street and a Dunkin Donuts to the east of North Main Street, both
abutting North Main Street and Hartford Avenue. There is also a Sunoco Station and a Hess between the
Dunkin Donuts and Crossroads Shopping Center abutting Hartford Avenue. To the east of I-495, there are
the Charles River Center and Stallbrook Market Place shopping centers that abut Hartford Avenue.

Crash Data
Crash data was received from the City of Bellingham Police Department for the years 2010-2012. Based
on this data, there were a total of 40 crashes reported from the beginning of 2010 through the end of 2012
at the intersection of Hartford Avenue (Route 126) and North Main Street (Route 126) and the
surrounding abutting driveways. Of the 40 crashes that occurred in the time period reviewed, only 22
crashes appear to be a result of the operations of the intersection itself. Of the other 18 crashes reported
for this cluster, eight crashes are at the adjacent Mobil Gas Station driveway, five crashes are directly
adjacent to the Sunoco Gas Station driveways, four crashes are a result of the Dunkin Donut’s driveway
operations for both site driveways, and one crash is at the 194 Hartford Avenue site driveway. Driveway
crashes may have been influenced by adjacent traffic signal operations and queues. At the intersection of
Hartford Avenue and North Main Street, approximately half of the 22 crashes are rear-end collisions, and
the remainder are angle and sideswipe collisions. At the adjacent driveways, the majority of crashes are
angle and rear-end collisions resulting from turning movements in and out of these sites. Of the 18
crashes at the access driveways, six are also “courtesy crashes”, a term used to describe a crash that
occurs when a driver on the mainline yields the right of way to a driver exiting a driveway and a collision
occurs as a result. Of the 40 crashes, only three involved personal injury.
There were a total of 43 crashes reported at the intersection of Hartford Avenue (Route 126) and the
Crossroads Shopping Center Driveway and the adjacent Hess Gas Station. Of the 43 crashes that occurred
in the time period reviewed, 23 crashes appear to be a result of the operations of the intersection with the
Crossroads Shopping Center Driveway while the remaining 15 crashes are at the adjacent Hess Gas
Station driveways. At the intersection of Hartford Avenue (Route 126) and the Crossroads Shopping
Center, there are five crashes that were reported that are eastbound rear-end collisions to the east of the
intersection. It can be assumed that these crashes are a product of queueing from the adjacent intersection
of Hartford Avenue (Route 126) at the I-495 southbound ramps. Of the remaining 23 crashes, over half
are rear-end collisions, and the remainder are angle and sideswipe collisions. At the adjacent Hess Gas
Station driveways, the majority of crashes are angle and rear-end collisions resulting from turning
movements in and out of these sites. Of the 15 crashes at the access driveways, two are also “courtesy
crashes”. Of the crashes reported only six of the crashes involved personal injury.
At the intersection of Hartford Avenue (Route 126) and the I-495 southbound ramps, there were a total of
40 crashes reported. Additionally, as previously stated, five reported crashes from the intersection with
the Crossroads Shopping Center can be attributed to this intersection for a total of 45 crashes. Of these 45
crashes, 27 crashes are rear-end collisions including seven crashes from the I-495 southbound off-ramp,
and 19 crashes on Hartford Avenue (Route 126). The remaining crashes are angle and sideswipe
collisions, including eight crashes between the westbound left turn movement and eastbound through
movement for vehicles entering the I-495 southbound on-ramp, as well as an isolated occurrence of a
vehicle backing out of a driveway on Deerfield Lane and striking a parked car. Of the crashes reported,
only six involved personal injury.
There were a total of 37 crashes reported at the intersection of Hartford Avenue (Route 126) and the I-495
northbound ramps. Of these 37 crashes, 16 crashes are angle collisions between the westbound left turn
and eastbound through movements for vehicles entering the I-495 northbound on-ramp. There are also 13
rear-end collisions, six of which are from the I-495 northbound off-ramp. Additionally, two westbound
rear-end collisions from the adjacent intersection of Hartford Avenue (Route 126) at Stallbrook Market
Place and the Charles River Center are more likely attributed to this intersection due to westbound
queueing issues. The remaining crashes are other angle, sideswipe, and single vehicle accidents. Of the
crashes reported, seven involved personal injury.

At the intersection of Hartford Avenue (Route 126) at Stallbrook Market Place and the Charles River
Complex, there were a total of 35 crashes reported. Of these 35 crashes, two crashes are attributed to the
intersection operations of Hartford Avenue (Route 126) at the adjacent I-495 northbound ramps. Of the
remaining 33 crashes, 18 crashes are rear-end collisions, the majority of which are in the eastbound and
westbound directions on Hartford Avenue (Route 126). There was one accident that involved a pedestrian
where a vehicle attempted to turn right from Hartford Avenue (Route 126) eastbound and struck a
pedestrian in the crosswalk while the pedestrian has the walk light. Another accident involved a bicycle
that attempted to cross Hartford Avenue (Route 126) while vehicular traffic on Hartford Avenue had the
right of way. The bicyclist was obstructed from view by the eastbound left turn queue and proceeded into
the eastbound lanes where they were struck. Both the pedestrian and bicycle accident involved injuries.
The remaining collisions at this intersection are angle collisions and sideswipe collisions.

Road Safety Audit Observations and Potential
Improvements
During the RSA meeting prior to the field visit, a brief introduction of the RSA process and a summary of
traffic volumes and crash information were presented to the audit participants. Following this brief
presentation, the members of the audit team were asked to discuss the existing issues that may affect
safety at the intersections along the Hartford Avenue (Route 126) Corridor between North Main Street
and Stallbrook Market Place. The audit team then visited the site as a group, at which time observations
of various safety concerns and deficiencies were identified and documented. Provided below is a list of
the safety concerns that were identified during the RSA for each intersection and for the corridor as a
whole and the potential enhancements identified during the RSA.
Hartford Avenue (Route 126) Corridor Wide
Safety Issue #1: Vehicle Speeds
Observations:
Vehicle speeds were noted by the RSA team to be a problem at several locations of the
Hartford Avenue (Route 126) corridor between North Main Street and the Stallbrook Market
Place/Charles River Center driveways. At the intersection with North Main Street, vehicles
traveling eastbound from Mendon into the study area were observed to travel at a high rate of
speed. Additionally, vehicles traveling eastbound through the intersection with the I-495
northbound ramps are driving downhill and at a fast rate of speed for the congested corridor
conditions. The posted speed limit on Hartford Avenue (Route 126) is 25 miles per hour to
the west, and 40 miles per hour to the east of Crossroads Plaza, but is only marked by a single
speed limit for the 25 mile per hour speed zone sign in the westbound direction that is
partially covered by vegetation.
Vehicle speed may be a contributing cause for rear-end collisions at each of the study area
intersections and driveways along Hartford Avenue (Route 126). Rear-end collisions along
Hartford Avenue account for approximately 35% of the collisions that occurred corridor
wide.
Enhancements:
• Provide additional speed enforcement at key locations on Hartford Avenue with the use
of temporary radar trailer. Enforcement will help deter drivers from traveling at a high
rate of speed.
• Install additional signage to inform drivers of the speed limit as they enter the study area.
• Ensure that all signage is unobstructed by overgrown vegetation to provide clear direction
to drivers.
• Review clearance intervals for signalized intersections to ensure they are appropriate for
vehicle speeds based on the 85th percentile speed of the roadway.

Safety Issue #2: Traffic Volumes and Congestion
Observations:
High volumes of traffic traveling through this corridor have caused issues with congestion.
The congestion issue is exacerbated during peak shopping times such as Saturdays or holiday
seasons. Based on input from Town officials during the audit, the five traffic signals along
Hartford Avenue (Route 126) were installed in the late 1990’s and are in need of upgrade. In
addition, MassDOT noted that there are coordination issues with the two I-495 ramp signals
due to the overpass. Congestion contributes to driver frustration and aggressive driving which
can be characterized by driving too closely to another vehicle, frequent lane changes (which
were noted by the RSA team to be an issue), and inappropriate gap acceptance for vehicles
exiting abutting driveways. This type of driver behavior can contribute to a variety of
accident types. Additionally, in a review of the accident data it was noted that accidents are
increased on Saturdays and the holiday season when traffic volumes are greatest. There is
also a large volume of heavy vehicles traveling this corridor due to the proximity of two
distribution centers (Dunkin Donuts & Best Buy) located approximately two miles to the
west.
Enhancements:
• Use adaptive signal control to increase capacity with the influx of seasonal and Saturday
traffic.
• Review/revise signal phasing and coordination to optimize travel through the corridor.
• Investigate Hartford Avenue (Route 126) signal coordination issues to optimize travel
through the corridor.
• Upgrade signalized intersections to add capacity.
• Upgrade all protected/permissive signal heads to flashing yellow arrow signal heads to
warn drivers of the permissive left turn.
• Provide the proper amount of dedicated turning lanes and through lanes for each
intersection as dictated by traffic volumes to optimize capacity.
• Design pavement for heavy truck use to reduce issues with pavement caused by heavier
vehicles.
Safety Issue #3: Distracted Driving and Inattention
Observations:
Audit team members discussed distracted driving and inattention as a problem within the
corridor. Several drivers have been observed using cell phones or texting while sitting in
congested traffic. Corridor wide, inattention contributes to approximately 33% of the total
accidents.

Enhancements:
• Develop a public education campaign related to distracted driving. Police are looking to
obtain a distracted driving grant that will allow for better education of the dangers of
distracted driving and will be advertised in both regular media and social media.
• Provide additional enforcement of distracted driving laws to minimize the number of
distracted drivers.
• Improve capacity of the corridor which will reduce the opportunity for drivers to become
distracted.
Safety Issue #4: Emergency Response
Observations:
Representatives from the local fire and police department in Bellingham were present and
identified issues with emergency response through this corridor. The major issue corridor
wide is that the congestion at the intersections makes emergency response difficult.
Additionally when an emergency response vehicle needs to travel through this corridor, there
are no shoulders for drivers to move into or an area to allow emergency vehicles to pass. This
forces emergency responders to move into opposing lanes to bypass queued traffic.
Emergency responders frequent this corridor to access the Milford Regional Hospital to the
west. The Police Department stated that they received a grant to purchase motorcycles to help
with emergency response in this congested corridor, but have since lost funding to maintain
the motorcycles. In many cases, drivers were forced to run a red light in order to let an
emergency vehicle through. One accident in the corridor is attributed to emergency response
operations.
Enhancements:
• Review emergency response preemption system to determine if it can be activated earlier
to facilitate movement through the corridor when an emergency vehicle is approaching.
• Investigate installation of additional emergency preemption detectors in the middle of the
bridge to help clear the intersections.
• Evaluate the impacts to adding shoulders along Hartford Avenue (Route 126) for vehicles
to pull into when an emergency vehicle is passing though the corridor.
• Investigate future funding to reinstate
motorcycles for police department
response. Safety Issue #5: Access Management
Observations:
Within the Hartford Avenue (Route 126)
study area, there are several commercial
driveways accessing adjacent land uses as

seen in Figure 2. The majority of these land uses abut the eastbound lanes of Hartford Avenue
(Route 126) between North Main Street and the Crossroads Shopping center driveway,
including a Dunkin’ Donuts, a Sunoco gas station, and a Hess gas station. There is also a
Mobil Gas station west of North Main Street that abuts both North Main Street and Hartford
Avenue.
It was observed that there is a “Do Not Enter” sign facing east at the Dunkin’ Donuts
driveway. This may cause confusion among drivers as this driveway is in close proximity to
the Hess driveway. Also, the driveway at the Sunoco gas station also is graded such that
vehicles have to slow dramatically to keep from scraping the driveway with the bottom of
their vehicles. It was also noted that Hartford Avenue eastbound right turning vehicles use the
Mobil Gas Station as a cut through to avoid the signal.
Crashes related to driveway access account for approximately 40% of the accidents that occur
between the Mobil gas station and the Hess gas station, and approximately 17% of accidents
corridor wide. Typically, these crashes are angle collisions that result from exiting vehicles
turning left. Having to cross three lanes of traffic at these driveways exacerbates the issue
and introduces the opportunity for courtesy crashes.
Enhancements:
• Evaluate turn restrictions at the various site driveways to facilitate the flow of traffic and
minimize conflicts on Hartford Avenue (Route 126), including right-in, right-out
alternatives and exit only alternatives using signage and geometry.
• Evaluate the possibility of adding an internal site driveway connecting the Hess Station to
the Crossroads Shopping Center to decrease traffic on Hartford Avenue (Route 126) and
allow Hess patrons to use the Crossroads Shopping Center traffic signal to make safer left
turns.
• Reduce the width of the Dunkin’ Donuts site driveway to help enforce a right-out only
condition.
• Install additional signage exiting the site to Hartford Avenue (Route 126) to enforce
Dunkin’ Donuts turn restriction exiting the site.
• Remove or relocate “Do Not Enter” signage at the Dunkin’ Donuts driveway to an
optimal location to portray a clear message to drivers.
• Improve the grade of the Sunoco gas station driveway to allow for vehicles to enter more
easily.
• Improve operations on Hartford Avenue (Route 126) to reduce cut-through traffic in the
Mobil gas station.
• Consolidate the Mobil gas station driveways to reduce cut-through traffic.

Safety Issue #6: Sun Glare
Observations:
It was noted by audit team members that sun glare, particularly in the afternoon in the
westbound direction, is an issue. Crashes related to visibility due to sun glare account for six
accidents corridor wide, four occurring during the afternoon and two occurring during the
morning.
Enhancements:
• Add back plates to signal heads where needed to help increase visibility by allowing
contrast between the back plate and signal head.
• Replace existing incandescent signal indications with L.E.D traffic signal indications to
improve visibility.
Safety Issue #7: Pavement Conditions and Markings
Observations:
In several areas throughout the study
area, rutted or distressed pavement was
observed. Distressed pavement may be a
contributing factor in rear-end collisions
as vehicles are not able to stop as
efficiently on these worn surfaces.
Additionally, through several segments of
the study area on Hartford Avenue, there
are missing pavement markings to
delineate marked lanes through the

Additionally, there are several offset intersections observed with little direction to the shifted
receiving lanes. This may be a contributing factor in the number of sideswipe collisions
traveling through this segment. The Town is planning a milling and resurfacing project for
the spring of 2015 to address the rutting and pavement markings on the Town owned section
of Hartford Avenue (Route 126). This project was discussed at the RSA meeting.
Enhancements:
• Repave/repair rutted pavement throughout the corridor to provide a better travel surface.
• Replace missing pavement markings to delineate travel lanes on Hartford Avenue
between North Main Street and the Crossroads Shopping Center.
• Add dotted lane extensions through intersections to guide vehicles through shifted lanes.
• Use thermoplastic striping versus paint to increase durability and attention of pavement
markings.

Safety Issue #8: Signage
Observations:
It was observed by the audit team that there are several signs throughout the corridor that
create clutter, and several signs that are not placed in an optimal location to convey a clear
message to drivers such as advance warning signage and guide signage. In addition, there
was a general lack of signage for the I-495 interchange to guide motorists as to where the onramps are located. The presence of multiple travel lanes on Hartford Avenue (Route 126)
exacerbates this issue as drivers may not know what lane is appropriate to access their
intended destination, creating last minute lane changes resulting in sideswipe and rear-end
collisions. One item that was specifically mentioned during the RSA meeting was the existing
directional sign on Hartford Avenue eastbound to I-495 northbound currently has a vertical
arrow to continue east, but still leaves motorists guessing which side of the roadway the ramp
can be accessed from. In addition, there is a lack of directional signage for the I-495
southbound on-ramp on Hartford Avenue westbound.
Enhancements:
• Consolidate repetitive regulatory and warning signs to reduce sign clutter.
• Review sign locations for optimal placement.
• Provide overhead guide signage to increase visibility and awareness of the I-495 ramps.
• Provide diagrammatic lane use signage at all approaches to improve driver understanding
and reduce the associated collisions.
• Replace I-495 northbound ramp signage from a vertical arrow to text for the “Second
Right” to further direct drivers to the proper lane placement.
Safety Issue #9: Pedestrian, Bicycle, and Transit Amenities
Observations:
Within the study area corridor, there are
sidewalks located on the northern side of
Hartford Avenue (Route 126) through the
study area that are five feet wide. A sidewalk
is also present on the southerly side of
Hartford Avenue from North Main Street to
the Hess gas station driveway. Through the
study area, there are typically ramps
available; however, some do not comply with
ADA standards.

It was also noted during the RSA field review that the pedestrian signal heads only show the
outlines of the “walk” and “don’t walk” symbols, which are not as visible. In some locations,
the audit team noted that pedestrian signal “don’t walk” indication was burnt out and must be
replaced. In addition, the Town has received complaints that the pedestrian timings are too

short and may not conform to current standards. As described in the crash data, there was one
accident that involved a pedestrian where the pedestrian had the signalized crossing and was
struck by a turning vehicle in a crosswalk at the intersection of Hartford Avenue (Route 126)
at the Charles River Center.
Additionally, there are no bicycle amenities present within the study area. The shoulders on
either side of Hartford Avenue are too narrow to safely accommodate bicycles. To travel this
corridor bicycle traffic would be forced to use the vehicular travel lane, which may cause
additional conflicts and aid driver frustration. There was one accident that involved a bicycle
where a bicycle attempted to cross Hartford Avenue at the Stallbrook Market Place against
the eastbound and westbound through phase and was struck.
It was also noted during the meeting that there are limited transit options in this area. The
Greater Attleboro Regional Transit Authority (GATRA) currently has a MBTA Shuttle
servicing this corridor providing access to the Forge Park commuter rail station with a stop at
the Crossroads Plaza. There is also a shuttle that was provided by the Charles River Center
during the holiday season last year that also serviced the Stallbrook Market Place.
Enhancements:
• Create an ADA compliant walking surface for the length of the corridor including
bringing sidewalk surface, handicap ramps, and crossings to ADA standards.
• Update pedestrian signals to have upgraded signal indications that are more visible.
• Update pedestrian heads to include countdown timers to increase awareness of the
pedestrian phase.
• Evaluate accommodations for bicycles in this area including a five-foot shoulder to travel
on.
• Install “Right Turns Yield to Pedestrians” signage to increase awareness of pedestrians.
Review/revise pedestrian walk and clearance intervals.
• Coordinate with GATRA to determine feasibility of including Stallbrook Plaza or
Charles River Center as a stop on the adjacent shuttle route.
• Coordinate with the Charles River Center to investigate providing a permanent shuttle
route between shopping centers during peak periods on Saturdays and the holiday season.

Hartford Avenue (Route 126) at North Main Street (Route 126)/Cedar Hill Road
Safety Issue #1: Sight Distance
Observations:
Audit participants noted visibility issues for
movements opposing the eastbound through
movement at this intersection. The eastbound
movement comes around a curve into the
intersection and often vehicle speeds were
observed to be higher than appropriate. There is
also obstructing vegetation on the southwestern
corner of this intersection that further limits
visibility. The movements affected by this are the
permissive westbound left turn and the
Poor sight distance looking west on Hartford
northbound right turn that is able to turn right on
Avenue at North Main Street
red. These permissive movements opposing a
higher speed movement with limited visibility
may be a contributing factor in the five angle collisions with the eastbound through
movement.
Enhancements:
• Evaluate the traffic impacts of restricting the westbound left turn movement to a
protected left turn only.
• Implement the flashing yellow arrow for westbound permissive left turns.
• Evaluate the traffic impacts of restricting the northbound right turn movement to no right
turn on red.
• Determine additional mitigation to reduce vehicle speeds for vehicles approaching the
intersection from the west.
• Remove vegetation on the southwestern corner of the intersection to improve sight
distance.
Safety Issue #2: Roadway Geometry
Observations:
The inner westbound through lane turns into an exclusive left turn lane at this intersection,
causing a turn lane trap for vehicles that have been traveling in the left lane through the
Hartford Avenue corridor. This issue may contribute to a number of the sideswipe collisions
and the crash that involved a vehicle backing at this location, as vehicles realize they are not
able to continue through on Hartford Avenue from the left lane. There is also a lane merge
sign on the departure end of the intersection traveling westbound though there is only one
travel lane through this intersection in the westbound direction. In the eastbound direction,
the receiving lanes narrow and are not currently marked, which caused one sideswipe
collision per the crash data.

Safety Issue #3: Traffic Operations
Observations:
Audit participants discussed traffic operations and queuing issues at this intersection. During
peak conditions there are long queues and insufficient stacking room. During the audit, it was
observed that the westbound through arrow signal indication is not as visible as a standard
green ball indication and may contribute to westbound crashes at this location.
Enhancements:
• Review existing timings to optimize traffic signal operations.
• Investigate extending storage bay pavement markings for the eastbound through and
northbound right turn lanes to provide additional queue storage.
• Review signal coordination with adjacent signalized intersections to manage queues on
Hartford Avenue (Route 126).
• Investigate modifying the exclusive northbound left-turn lane to a shared lane to provide
a second lane for right turns.
• Replace the westbound green through arrow signal indication with LED. If the signal
visibility is still an issue, consider replacing the green through arrow signal indication
with a standard green ball and install a No Right Turn (R3-1) sign mounted on the mast
arm to reinforce the one-way circulation on Cedar Hill Road.
Safety Issue #4: Striping
Observations:
Recent construction has resulted in several of the crosswalks being paved over.
Enhancements:
• Restripe crosswalks at the intersection.
Hartford Avenue (Route 126) at Crossroads Plaza/Rawson Road
Safety Issue #1: Sight Distance
Observations:
During the field review, audit participants noted that
the sight distance looking east from Rawson Road is
obstructed by overgrown vegetation. Based on discussions during the audit meeting, the vegetation
is overflowing from the adjacent private property on Sight distance at Rawson Road looking east

Safety Issue #2: Roadway Geometry
Observations:
Audit participants discussed issues with
the westbound left turn into the
Crossroads Shopping Center. Several
drivers have been observed to cut the turn
short, turning into the lanes exiting the
shopping center to Hartford Avenue on the opposite side of the median. Currently at this intersection, the
northbound right turn has two exclusive
right turn lanes that are under a no right
Northbound double right turn lane out of
turn on red condition, which is necessary

with a double turning movement. Audit
participants observed during the field review that the left-most right turn lane is underutilized
leaving most of the right turning traffic in the right-most turn lane. There was one crash
shown in the crash data that was a product of the double right turn lanes where a vehicle in
the left-most lane turned right into the right-most lane and collided with another vehicle.
Enhancements:
• Add a “Keep Right” sign to the Crossroads Shopping Center median.
• Evaluate the traffic impacts of consolidating the northbound right turn movement to one
lane and allowing vehicles to turn right on red to improve operations.
• Provide dotted lane extensions for the northbound right turn to keep vehicles in their
intended lane and prevent sideswipes.
• Consider allowing right turn on red from the right lane only using a “No Turn on Red
Except from Right Lane” sign (R10-11c).
Safety Issue #3: Pedestrian Accommodations
Observations:
There is a marked crosswalk across Rawson Road but no pedestrian signal heads are
provided. As stated earlier, there were several pedestrian signal head issues identified at
this intersection including, blank signal heads and burnt out “flashing don’t walk” signal
indications.

Safety Issue #1: Signal Equipment
Observations:
The eastbound left turn from Hartford
Avenue (Route 126) to Deerfield lane has
an exclusive left turn lane and turn phase.
Since Deerfield Lane is a local residential
roadway that ends in a cul-de-sac, the
traffic volumes in this eastbound left turn
lane are low and often not present every
cycle. The phase, however, is being
called each cycle despite the absence in
vehicles in this lane. During the field
review, audit team members observed the
Vehicle turning from I-495 ramp over
northbound left turning vehicles from the
eastbound left turn lane
I-495 ramp to Hartford Avenue (Route
126) westbound often traveling over this lane when no vehicles are present. Additionally, the
westbound through vehicles have a lane shift to continue through and also tend to travel into
the eastbound left turn lane. Since both of these movements are traveling over the eastbound
left turn detectors when no vehicle is present in the lane, the phase is being called each cycle.
This causes undue delay as the signal is operating inefficiently.
In addition, there is currently a marked crosswalk across Deerfield Lane, but no pedestrian
signal heads are provided, similar to Rawson Road.
Enhancements:
• Review if the eastbound left turn memory is locked.
• Relocate detector loops further back in the eastbound left turn lane from the area that is
being traveled over by other movements.
• Add a minimal delay to the detector to reduce the vehicle calls from movements running
over the detector loops.
• Relocate the stop bars for the eastbound lanes further east to create a tighter turning
radius for the northbound left turn that will keep vehicles from crossing into the left turn
lane.
• Install pedestrian signal heads for the Deerfield Lane crosswalk to provide safe pedestrian
accommodations.

Safety Issue #2: Westbound Left Turn Lane Operations
Observations:
There is currently no westbound left turn lane at the intersection of Hartford Avenue (Route
126) at the I-495 southbound ramps/Deerfield Lane despite high left turn volumes. This
forces the inner westbound through lane to become a defacto left turn lane and causes the
right through lane to queue across the bridge. Through vehicles also travel up the left turn
lane until they reach the left turn lane queue and attempt to move back to the right most lane
creating sideswipes and rear-end collisions.
The signal phasing currently has the westbound left turn run as a lag phase after the
eastbound through movements. Audit members stated if no phase is called from the I-495
southbound off-ramp, the eastbound through phase is called after the lagging westbound left
turn; however, westbound left turning vehicles continue through the intersection, interrupting
the eastbound flow of traffic. There is also a lack of acceptable gaps within the permissive
left turn phase which leads to driver frustration. These issues create angle collisions between
the westbound left turn movement and eastbound through movement at this intersection.
Additionally, in several cases, the driver of the westbound left turning vehicle claims to have
not seen the eastbound through vehicles. This may be attributed to the two eastbound through
lanes where vehicles obstruct one another from view.
In addition, it was stated that some westbound vehicles desiring to turn left may also use
Deerfield Lane as a short cut to access the I-495 southbound on-ramp by turning around in a
residential driveway and using the protected signal phase from Deerfield Street, which can be
attributed to one accident with a vehicle backing up on Deerfield Lane.
Enhancements:
• Add one or two exclusive westbound left turn lane(s) to minimize the congestion issues
surfacing from the left turn lane queues.
• Provide protected only left turn phasing for the westbound left turn to eliminate the
permissive turn.
• Implement a flashing yellow arrow to the westbound left turn to warn drivers of the
permissive turn requirement to yield to oncoming vehicles.
• Review signal phasing to determine if changes can be made to optimize traffic flow at
this location.
• Consider opportunities for westbound drivers to access I-495 southbound without
stopping Hartford Avenue (Route 126) as a long term enhancement.
Safety Issue #3: Pavement Markings
Observations:
On the northbound left turn lanes from the I-495 southbound off-ramp, the pavement
markings separating the two left turn lanes and the lane utilization arrows indicating that the
right-most lane is a left and through lane to Deerfield Avenue and Hartford Avenue (Route
126) are faded.
Additionally, the crosswalk on Deerfield Lane is partially missing due to recent roadway
construction and needs to be restriped.

Safety Issue #4: Yield Control
Observations:
In a review of the crash data, the audit team noted that seven of the 40 crashes at this location
are in the northbound right turn lane from the I-495 southbound off-ramp. This right turn
movement from the off-ramp is under yield control, allowing drivers to proceed through the
intersection when there is an available gap in traffic. It was observed by audit participants
that this yield control is a contributing factor for the number of rear-end collisions at this
location, as drivers are looking left for an available gap and begin to edge into the roadway
thinking that the vehicle ahead of them has merged into traffic when it has not, resulting in a
rear-end collision.
Enhancements:
• Consider installing yield control pavement markings for the off-ramp as a short-term
enhancement.
• Evaluate the impacts of including the northbound right turn as part of the traffic signal
and enforcing a no right turn on red condition.
• Realign angle of I-495 southbound off-ramp right turn lane to be closer to a 90 degree
turn to help drivers maintain view of vehicles in front of them as a long term
enhancement.
Safety Issue #5: Signage
Observations:
The eastbound “Right Lane Must Turn Right” sign was turned at a skewed angle.
Enhancements:
• Replace the “Right Lane Must Turn Right” sign.
• Upgrade exiting signage to diagrammatic lane usage signage to provide clear lane usage
instruction to drivers.
Safety Issue #6: Sight Distance
Observations:
During the field review, audit participants noted that the sight distance looking east from the
Deerfield Lane approach is obstructed by vegetation and a fence on an adjacent property

Hartford Avenue (Route 126) at I-495 Northbound Ramps
Safety Issue #1: Westbound Left Turn Operations
Observations:
The westbound left turn movement was widely discussed by audit participants during the
RSA. As seen in the crash data provided, the approximately 45% of the crashes that occurred
at this intersection were angle collisions between the westbound left turn and the eastbound
through movement. The eastbound inner lane blocks the visibility of the outside lane from the
westbound left turn movement as the eastbound lanes travel around a slight horizontal curve.
Additionally, the vehicles are coming down a slight hill where vehicle speeds may be slightly
higher than expected for the opposing westbound left turn movement. Additionally operations
at this intersection are poor. Vehicles queue to the Stallbrook Plaza intersection and drivers
are required to wait several light cycles to proceed through the intersection, causing driver
frustration and the increased likelihood that an insufficient gap will be accepted. Queues from
the short westbound left turn spill into the adjacent through lane, further diminishing
operations. The westbound left turn movement currently includes protected and permissive
left turns.
Enhancements:
• Replace the existing “Left Turn Yield on Green (Ball)” sign that is faded and broken.
• Implement a flashing yellow arrow to the westbound left turn to warn drivers of the
permissive turn requirement to yield to oncoming vehicles.
• Evaluate the impacts of restricting the westbound left turn movement to allow protected
turns only.
• Extend the length of the westbound left turn lane to increase queue storage.
• Construct a new I-495 northbound on-ramp on the northerly side of Hartford Avenue to
eliminate the left turn movement.
Safety Issue #2: Roadway Geometry
Observations:
The eastbound right turn is channelized to access the I-495 northbound on-ramp; however,
there is no exclusive right turn lane with storage. This makes it difficult for the permissive
westbound left turning drivers to differentiate between right turn and through movements
from the west, which may contribute to the high number of angle collisions at this location.
Additionally, it may be a contributing factor between the eastbound right turn and through
movement crashes documented.

Hartford Avenue (Route 126) at Stallbrook Plaza/Charles River Center
Safety Issue #1: Pedestrian Accommodations
Observations:
This intersection is heavily utilized by pedestrian traffic, based on discussions during the
audit meeting; in particular, many young pedestrians access the adjacent McDonald’s in
Stallbrook Market Place from the movie theater in the Charles River Center. In several cases,
younger pedestrians have been observed cutting through the Pier 1 Imports parking lot and
crossing Hartford Avenue (Route 126) outside of the crosswalk. Additionally, a pedestrian
was struck at this location in a crosswalk. The audit team discussed that the current signal
phasing at this intersection allows a long wait time for pedestrians before the phase is called.
As mentioned there is a large volume of young pedestrian traffic crossing at this location,
particularly at times in the evening and night when traffic volumes are lower on Hartford
Avenue (Route 126). This leads to pedestrians crossing outside of the protected signal phase
when they feel it is safe to cross as they do not want to wait for the pedestrian signal. As
stated earlier, there were several pedestrian signal head issues identified at this intersection
including, blank signal heads, burnt out “flashing don’t walk” signal indications and the
pedestrian signal post in the northeast corner is blocked by vegetation.
Enhancements:
• Install “Right Turns Yield to Pedestrian” signage to further warn drivers to yield to
pedestrians in the crosswalk.
• Install a fence along the Pier 1 Imports property abutting Hartford Avenue (Route 126) to
deter pedestrians from crossing at this location.
• Trim vegetation away from the pedestrian signal in the northeast corner of the
intersection.
• Evaluate signal phasing and timing to allow the signal to be more responsive during off
peak periods and encourage pedestrians to use the protected pedestrian signal phase more
often.
• Upgrade current pedestrian accommodations to current ADA standards.
• Replace pedestrian signal indications that are burnt out or no longer working.
Safety Issue #2: Sight Distance
Observations:
The northbound right turn is obstructed to the left by the adjacent light pole in the median of
the Charles River Center driveway. Additionally, the adjacent signal controller cabinet further
obstructs sight distance at this location to the left. In the westbound direction, the adjacent
retaining wall to the east of the Stallbrook Market Place driveway at the cemetery obstructs
sight distance around the corner where queues extend on Hartford Avenue, which may be a
contributing factor in rear-end collisions at this location. There is currently an existing

warning beacon on Hartford Avenue (Route 126) in the westbound direction but it is not
currently working.
Enhancements:
• Relocate both the existing light pole in center median of Charles River Center and the
signal controller cabinet in the southwest corner of the intersection to improve sight
distance from the Charles River Center driveway.
• Restrict northbound right turn on red from the Charles River Center driveway.
• Install warning signage (W3-3) on Hartford Avenue east of the intersection to warn
oncoming motorists to the potential for queued traffic.
• Repair the existing warning beacon assembly on Hartford Avenue (Route 126).
Safety Issue #3: Roadway Geometry
Observations:
During the RSA, audit team members discussed a crash that recently occurred at this
intersection adjacent to the Pier 1 Imports building that was not shown in the crash data
provided. A vehicle traveling in the eastbound lanes went off the road and crashed near the
gas line of the Pier 1 building. There is a slight horizontal curve in the eastbound direction
which may have contributed to the vehicle running off the road. In addition, during the RSA
trucks were observed having difficulty turning right when exiting the Stallbrook Market Place
driveway. The trucks had to encroach into adjacent lanes and onto the sidewalk/grass
adjacent to the McDonalds.
Enhancements:
• Install a guardrail adjacent to the Pier 1 Imports store in the vicinity of the gas line
connection to protect the building from errant vehicles at this location.
• Increase the curb radii exiting the Stallbrook Market Place driveway to accommodate
larger vehicles.
Safety Issue #4: Signage
Observations:
During the field review, audit participants observed a yield sign for the westbound right turn
into Stallbrook Market Place. This turn is signalized and a yield sign should not be present at
this location. Additionally, it was discussed during the field visit that the “Left Turn Yield on
Green” sign (R10-12) is not in an optimal location, however, this signage is not applicable as
the eastbound and westbound left turn movements always run permissive. Also, the
directional signage on the overhead span wire may not line up correctly with the travel lanes
as it is on a horizontal curve. Two of the sideswipe crashes may relate to the overhead
signage issue.
Enhancements:
• Remove the existing yield sign for the westbound right turn.

Safety Issue #5: Traffic Signal
Observations:
There currently are no protected phases for the left turn movements out of the shopping
center driveways. While the majority of traffic from Stallbrook Plaza is expected to turn right
to access I-495, the majority of traffic exiting the Charles River Center is expected to turn left
to access the highway. There are two receiving lanes on westbound Hartford Avenue
(Route 126), therefore, the southbound right turn may enter the outside line while the
northbound left turn enters the inner lane. Three crashes are attributed to the permissive
condition at the shopping center driveways. During the audit review, it was observed the
southbound green signal indication located on the pedestal pole by the Pier 1 Imports
building was burnt out.
Enhancements:
• Evaluate signal phasing for driveway approaches.
• Replace signal indications that are burnt out or no longer working.
• Provide dotted lane extensions to delineate turning movements from the shopping center
driveways onto Hartford Avenue (Route 126) westbound.
Safety Issue #6: Site Access
Observations:
The intersection is congested due to the large traffic volumes accessing the adjacent shopping
centers. For both Stallbrook Plaza and the Charles River Center, this intersection is the single
point of access, which aides in intersection congestion and capacity problems.
Enhancements:
• Evaluate the potential for additional access points to both shopping centers.
Safety Issue #7: Pavement Markings
Observations:
In the eastbound direction, the two receiving lanes quickly transition into one travel lane,
which is denoted by an existing warning sign. However, the pavement markings for this
transition are missing. Additionally, there seemed to be a general lack of lane utilization
pavement markings on both of the shopping center driveways, which aids to the confusion of
which lane motorists should be in.
Enhancements:
• Restripe eastbound receiving lane pavement markings.
• Install additional lane utilization pavement markings on the shopping center driveways.

I-495 Mainline Ramps
Safety Issue #1: Roadway Geometry
Observations:
It was discussed during the RSA meeting that there have been rollover crashes on the I-495
ramps. There is an advisory speed limit of 25 miles per hour on these ramps as marked by
warning signage. In some case, the warning signage is obstructed or not present.
The Police Chief mentioned that there has been ponding and icing issues along the I-495
northbound off-ramp which has attributed to crashes. This issue may be related to the areas
that are in the shade from adjacent trees and catch basins that are not flush with the roadway.
The acceleration and deceleration lanes for the ramps were noticed by audit members to be
shorter than appropriate and do not provide adequate time to accelerate to I-495 speeds or
slow to the 25 mile per hour ramp advisory speed. Audit team members indicated that vehicle
speeds on I-495 are higher than the posted 65 miles per hour.
Enhancements:
• Evaluate acceleration and deceleration lane length standards and lengthen the lanes, as
appropriate, to accommodate the 85th percentile speeds on I-495.
• Add signage or remove obstructions of existing advisory speed signage on the I-495
ramps.
• Evaluate if the recent I-495 pavement repaving project has addressed the ponding/icing
issue on the northbound off-ramp.
• Evaluate the turning radii of the ramps to ensure they meet current standards.
• Resurface the ramps to provide a higher friction surface for vehicles to travel on.
• Add delineators and chevron signage on the ramp curves.
• Install queue detection on the off-ramps with any signal improvements along Hartford
Avenue (Route 126).

Recommendations
After the site visit, audit participants returned to discuss the safety issues and consider various
improvements. The audit participants were encouraged to consider both short and long-term
improvements for each of the existing safety issues. Each improvement considered has been categorized
as short-term, mid-term, or long-term based on the definitions shown in Table 2. Additionally, a cost
category has been assigned to each improvement based on the parameters set forth in Table 2.

42 causing Vehicle 1 to slow down. Vehicle 2 rear ended Vehicle 1.
Vehicle 1 was exiting the Dunkin Donuts parking lot while Vehicle 2 was
38 entering the lot. Vehicle 2 turned too early and struck Vehicle 1.
Vehicle 2 turning left onto North Main Street struck Vehicle 2 traveling East
21 on Hartford Ave.. Unclear if green arrow was lit determining right of way.
Both vehicles traveling north on North Main Street. Vehicle 2 rear ended
19 Vehicle 1 at the stop light at Hartford Avenue.
Vehicle 2 traveling EB attempted to turn left at 194 Hartford Ave. He did not
51 see Vehicle 1 traveling EB on roadway and struck him.
Vehicle 2, headed East on Hartford Avenue, was following Vehicle 1 too
closely. An uninvolved vehicle in front of Vehicle 1 slowed, causing Vehicle 1
18 to slow, Vehicle 2 failed to slow rear ending Vehicle 1.
Both vehicles heading NB on North Main Street waiting to turn right onto
Hartford Avenue. Light turned green and Vehicle 1 proceeded then suddenly
37 stopped and Vehicle 2 rear ended it.
Vehicle 2 was looking at her cell phone and did not see Vehicle 1 slow down.
48 Vehicle 2 rear ended Vehicle 1.

Vehicle 1 was stopped at the stoplight at the North Main and Hartford Ave

Vehicle 1 stopped in traffic to turn left. Vehicle 2's brakes failed and she

Both vehicles stopped at red light at North Main St. and Hartford Ave.
Vehicle 1 edged out to take a right onto Hartford Ave then stopped for traffic.

Vehicle 1 attempted to go around several vehicles that were stopped due to
an accident on and struck a sign at the Dunkin Donuts.
Vehicles were stopped at North Main Street stop light. Vehicle 2 thought

Vehicle 1 was in the left turn lane for the 495NB ramp and Vehicle 2 was in
the middle travel lane. As the light turned green both vehicles turned left,
Vehicle 2 doing so abruptly, striking the rear back side of Vehicle 1.
Vehicle 1 was traveling EB on Hartford Ave when he was cut off by a black
Volvo causing him to swerve and hit the curb and guardrail.
Both vehicles headed WB on Hartford Ave in heavy traffic. Vehicle 2 rear
ended Vehicle 1.

Vehicle 1, going EB on Hartford Ave, was hit by Vehicle 2 turning left onto I­
495 NB ramp.
Both vehicles at the stop light at the I-495 NB off ramp onto Hartford Ave.
The light turned green and Vehicle 2, who was behind Vehicle 1, accelerated
before Vehicle 1, rear ending it.
Vehicle 2, a Tractor Trailor Unit, rear ended Vehicle 1 at the red light at the
495 NB off ramp to Hartford Ave. No narrative.
Both vehicles traveling WB on Hartford Ave in heavy traffic. Vehicle 2
attempted to change lanes and pass Vehicle 1, but side swiped Vehicle 1 in
doing so.
Vehicle 1 was stopped in the WB lane of Hartford Ave. Vehicle 2 was
adjusting his radio, did not see Vehicle 1, and rear ended him.
Both vehicles were on the 495NB off ramp onto Hartford Ave. Vehicle 1
stopped, began to proceed, and stopped again and was then rear ended by
Vehicle 2.
Vehicle 1, a Tractor Trailor Unit, in the left lane attempted to turn right onto
495 NB ramp and struck Vehicle 1.
Both vehicles headed EB on Hartford Ave, Vehicle 1 in the left lane, Vehicle
2 in the right. Vehicle 2 looked down to release emergency brake and drifted
into the left lane, side swiping Vehicle 1.
Both vehicles headed EB on Hartford Ave, Vehicle 1 in the right lane, Vehicle
2 in the left. Vehicle 1 changed lanes and cut off Vehicle 2. Vehicle 1
jammed on the brakes for traffic and Vehicle 2 rear ended him.
Both vehicles had just merged onto Hartford Ave from 495 NB. Vehicle 2,
behind Vehicle 1, quickly moved into the left lane of traffic. Vehicle 1
attempted to do so shortly after cutting off Vehicle 2.

Both vehicles headed WB on Hartford Ave , approaching the stoplight at the
495 NB ramp. Vehicle 1 stopped for red light and Vehicle 2, due to
inattention, did not stop in time, rear ending Vehicle 1.
All vehicles headed WB approaching stop light. Vehicle 3 rear ended
26 Vehicle 2, propelling him into Vehicle 1.
Both vehicles headed WB on Hartford Ave. Vehicle 2 attempted to turn left
onto the 495NB on ramp from the right lane. Vehicle 1 was in the blind spot
of Vehicle 2. Vehicle 2 struck Vehicle 1.
Vehicle 2 was behind Vehicle 1 stopped at the red light at the 495 NB ramps,
facing WB. Vehicle 2 thought traffic had started moving and accelerated.
Vehicle 1 had not moved and was rear ended.
Vehicle 1 stopped in line at the red light at the 495 NB ramps facing WB.
Vehicle 2 attempted to pass Vehicle 1 on the right to get on the ramp to 495
NB, in doing so he struck Vehicle 1 on the rear passenger side.
Both vehicles were on the 495NB off ramp onto Hartford Ave. Vehicle 1
stopped, began to proceed, and stopped again and was then rear ended by
Vehicle 2.
Vehicle 2, a Tractor Trailor Unit, was taking a left turn onto the 495 NB ramp,
and was given the right of way by a green left arrow. Vehicle 1, a
motorcycle, ran through the red light and skidded out, crashing into the TT.
Both vehicles were stopped at the light on the 495NB off ramp onto Hartford
Ave. Vehicle 2 operator sneezed, causing her foot to come off of the brake
pedal, and rear end Vehicle 1.

the Stallbrook Plaza intersection with two unknown vehicles occupying the
two left only lanes blocking the view of a bicyclist crossing the intersection
from north to south. Vehicle 1, whose light was green, struck the bicyclist.
Vehicle 2 was in the left turn only lane seeking to turn left into the Charles
River Plaza. Vehicle 1 attempted to merge into the left only lane and side
swiped Vehicle 2.
Vehicle 1 was headed EB on Hartford Ave going through the Stallbrook
Plaza intersection when Vehicle 2 attempted to move into the left travel lane

Plaza. Vehicle 2 rear ended Vehicle 1.
Vehicle 2 was stopped in traffic when Vehicle 1 rear ended him.
Vehicle 2, in the left turn only lane from Stallbrook Plaza to Hartford Ave, was
in the wrong lane and backed up, hitting Vehicle 1.
Vehicle 1 stopped at the red light at the Stallbrook Plaza facing W B. Vehicle
2 failed to stop in time and rear ended Vehicle 1.
Vehicle 1 turning left onto Hartford Ave while Vehicle 2 was turning right onto
Hartford Ave from the opposite side of the intersection. The two vehicles
collided when merging.
Vehicles were both on the access road to the Stallbrook Plaza. Vehicle 1
was in Vehicle 2's (TT) turn radius and was hit by Vehicle 2.
Heavy traffic. Both vehicles crossed double yellow to access 495 NB.

a parked car assisting another motorist who lost control of her vehicle.
Both vehicles on the ramp to I-495 NB. Vehicle 1 slowed to yield and Vehicle 2
rear ended her.
Vehicles 1, 2 and 3 were traveling 495 North. Vehicle 1, in the left lane

Vehicle 1 was slowing for a tow truck with light flashing when it was rear ended
by Vehicle 2 who attempted to avoid a vehicle who slammed on his brakes.
Vehicle 1 was attempting to enter 495 South via exit 18 and came to close to
the snow bank which pulled his vehicle off the roadway until it was forced to roll
over.
Vehicle 1 was traveling 495 South when the operator lost control and drifted into
the median, down the slope and collided with a tree.
Vehicle 2 attempted to merge into the left lane on 495 South and collided with
Vehicle 1, forcing it into the median.
A tire bounced out from the median toward Vehicle 1. Vehicle 1 swerved into
the median to avoid any collisions, the tire then collided with Vehicle 2 traveling
in the center lane.
Vehicle 1 traveling 495 South could not stop in time for slowing traffic, so the
operator swerved to the right and collided with a cruiser parked in a shut down
lane, then went off the roadway and striking a tree.
Vehicle 1 traveling 495 North in the center lane looked down at his phone and
drifted into the left lane colliding with Vehicle 2.
Vehicle 1 traveling 495 South struck a deer that entered the roadway.
