Program Agreement, Assumption of Risk, and General Release
A.
General Information, Program Rules, Field Trips
Student acknowledges that participation in the Community of Scholars program
(“Program”) is voluntary. However, in order to participate in the Program, Student must
agree to the following terms and conditions of this Program Agreement, Assumption of
Risk, and General Release (“Agreement”).
Program Rules: Student agrees to:
1. conduct self in a safe and prudent manner while participating in this Program;
2. follow the directions of faculty, staff, and field trip supervisors;
3. observe all applicable rules of any field trip sites; and
4. observe all applicable University rules and policies, including those referenced in the
Community of Scholars Summer 2016 Program Policies.
Student understands that failure to comply with Program Rules or failure to conduct self in a
responsible manner may result in Student’s immediate removal from the Program at
Student’s own expense and forfeiture of all tuition and expenses paid. Participant
understands that this decision shall be made at the sole discretion of American University
(“University”) without the requirement of going through the resolution process outlined in
the Student Conduct Code.
Field Trips: There may be Program field trips to various sites located in the Washington
D.C. metropolitan area. In some instances, the University will make arrangements for these
trips, including the provision of transportation. Student acknowledges that participation in
field trips involves risks not found on the University campus. The risks for these field trips
and related travel are significant and include without limitation risk of death or serious bodily
injury.
B.
Fitness to Participate and Emergency Medical Treatment
Student represents to American University that there are no health-related reasons or
problems of which Student is aware that preclude or restrict Student from engaging in the
Program. Student hereby authorizes the University to secure necessary emergency medical
treatment in the event of injury or illness while participating in the Program.
C.
Assumption of Risk
Student has read and acknowledges that she/he has been informed about certain risks and
responsibilities involved with this Program, including field trips, and that Student is
knowingly and voluntarily assuming them. I understand that although the University is
sponsoring the Program, the University does not assume responsibility for any loss, injury or
damage to person or property in connection with the Program.
D.
Program Surveys and Evaluations
As part of Student’s participation in American University’s courses and programs, Student
may be asked to complete class or program evaluations or surveys. Participants’ feedback is
essential in the University’s effort to provide quality education experiences. All the
information provided on these surveys will be collected on an anonymous basis and kept
confidential. Students will be given the opportunity to decline to complete any survey or
question they wish.
E.
Likeness & Testimonials
Student hereby grants American University irrevocable permission to use any testimonials
and Student’s image, visual likeness, portrait, or photograph in all forms and media
(including but not limited to publications, websites, catalogues, brochures, books, magazines,
photo exhibits, motion picture films, and/or video) (collectively referred to as “Works”) for
the following purposes:

Teaching;
Admissions applications and recruiting materials;
Professional journals and papers;
Institutional publicity and public relations;
Archival purposes; and
Any other purpose which the University deems fit in the interest of education,
knowledge, research, marketing, advertising, or public relations.
Student agrees that all right, title and interest in and to all such Works and any reproductions
or derivative work thereof shall be the exclusive property of American University. Student
understands that the University may keep or may use the Works and derivative works now
and in the future. Student agrees that the University does not owe Student any
compensation for the acts that Student has consented to in this agreement.
Student hereby releases American University, its officers, directors, agents and employees
from all liability or legal responsibility that may arise from the acts that Student has
authorized or consented to herein.
F.
Authorization to Release Student Information
Student gives permission for the faculty and/or staff of the Program to release the following
information about Student to agencies and other official sites that require it for security
purposes as a condition of our class visit. I understand that I am not required to provide this
information, but that failure to do so may result in either a delay or prevention of my
admission to the site. Information to be shared includes but is not limited to: Full name,
social security number, date of birth, plac e of birth, citizenship, passport number,
and/or home address.
G.
Voluntary or Involuntary Withdrawal or Dismissal
Student understands that all participants are subject to University regulations, Program
guidelines, and the laws of the jurisdictions of the D.C. metropolitan area. If I violate these
rules, if I fail academically, or if I behave in a way that is detrimental to other students or the
Program, the University shall have the right to dismiss me from the Program. The
University's decision will be final and may result in the loss of academic credit and Program
fees.
Student agrees to pay for all costs arising out of my voluntary or involuntary withdrawal
from the Program prior to its completion for whatever reason, including withdrawal caused
by illness or disciplinary action. Student (including Student’s parents, guardians, or legal
representatives) agrees not to assert claims for or hold the University, its trustees, employees,
officers, faculty, agents and co-sponsoring institutions and their agents responsible for any
costs or losses resulting from Student’s early withdrawal from the Program.
H.
General Release
Student understands that participation in Program is strictly voluntary and involves risks.
Student agrees that, in consideration of American University sponsoring Program and
permitting Student to participate, Student (including Student’ parents, guardians, or legal
representatives) agrees to indemnify, release and hold harmless the University, its trustees,
officers, employees, faculty, agents, and affiliates from all liability which may result from
Student’s participation in the Program.
Student and his/her parents, guardians, or legal representatives represent that they have read
and understood all the provisions in this Program Agreement, Assumption of Risk, and
General Release. Student’s parent, guardian or legal representative hereby specifically grants
permission for Student to participate. Parents, guardians, or legal representatives agree to be
legally responsible for the obligations and acts of the Student as described in this Agreement.
Student and his/her parents, guardians, or legal representatives agree to be bound by all
terms of this Agreement, as indicated by their signatures below.
