2015-2016 Academic Year Student Health Insurance Dear Students and Parents: Northampton Community College requires students enrolled in the following programs to carry health insurance: Allied Health Majors International F-1 Visa Students On-Campus Residents These students are automatically enrolled in the college’s insurance plan unless they WAIVE coverage by the waiver deadline. Health insurance coverage will be provided by Companion Life Insurance Company utilizing CIGNA’s national PPO network. Complete plan details and a waiver link will be available online at www.northampton.edu/studentinsurance/2015-16 by June 15, 2015. The annual premium is $2500.00 and will be prorated and billed in two installments in the Fall 2015 and Spring 2016 sessions. The premium increase is due to our high volume of claims during the past two years. Also please note that similar coverage on the State Marketplace/Exchange would cost between $3,600-$5,000 per year. Spouse, domestic partner, and dependent child insurance options are also available at an additional cost. IMPORTANT: This is an Annual Plan Enrollment. Students enrolled in the fall who return to school in the spring will continue to be enrolled and responsible for payment of the full annual premium for coverage effective 8/15/2015 – 8/14/2016 regardless of any changes in their program or residency status. Students enrolled in the college insurance plan in the fall cannot waive coverage in the spring for any reason, including but not limited to a change of major to a non-allied health major, moving out of on-campus housing, change in visa status, reduction in financial aid, and/or the purchase of other insurance. IMPORTANT INFORMATION for Students Who Have Medical Insurance: Students insured by family or private medical insurance may request to waive coverage in the College-sponsored plan as follows: To qualify for a waiver, your policy must: --be issued by a U.S. insurance carrier --have no out of area restrictions --cover mental health and substance abuse --have an annual deductible not to exceed $2500.00 --remain active while you are a student at NCC --International students must have emergency medical transport and repatriation of mortal remains; $1,000,000 combined single limit, and their coverage must be comparable to that of the student plan Waiver requests are only accepted online at www.northampton.edu/studentinsurance/2015-16 Proof of Insurance coverage submitted to the College Health Center will NOT waive the college insurance coverage. Successful submission of the online waiver will return a confirmation that you must retain for your records. The insurance carrier reviews waiver requests and reserves the right to request additional information at any time. Allow 2 weeks for processing of waivers. If approved, a credit equal to the amount of the insurance charge will be posted on the student’s tuition bill. Students are responsible for confirming the waiver credit on their tuition bill and may appeal a missing waiver credit. Students must bring a copy of their confirmation notice to the Bursar’s Office no later than 2 weeks after the waiver deadlines below. Students may go to MyNCC to view their Tuition Bill and other important information 24/7. Waivers that are not approved by the student’s tuition due date will require payment of the insurance charge at that time. Subsequent approval will generate a refund accordingly. Failing to pay a tuition bill in full may not guarantee a student’s registration. Timely submission of the waiver is your responsibility. NCC cannot remove charges or defer payment from the student bill until a waiver is formally approved by the carrier. NO EXCEPTIONS can be made to the waiver policy. Waiver Deadline: August 31, 2015. The waiver will be good for the entire academic year. New students in the Spring 2016 semester must complete the waiver by January 31, 2016 to avoid charges for Spring 2016.
