Guidelines for Nurses Advancing Within VPNPP

(Effective May 4, 2015)

Below are the steps for a Manager and Candidate to take once the decision is made to advance within

VPNPP. Please note: Nurses have to have been in their current position for a minimum of 6 months in order to advance to the RN3/4 or LPN3/4 position

Step 1: Manager/Staff Nurse Discussion

May be initiated by either the Manager or Staff Nurse wishing to advance

Discussion occurs to decide if advancement is desired and appropriate at this time

Candidate completes the “Letter of Intent” document and the Manager signs

Step 2: CREDO Behavior evaluation

Manager completes the “CREDO Behavior Addendum” and provides a copy to the Candidate.

Step 3: Performance Evaluations

Evaluations templates can be found on the “Tools for Leaders” tab on the VPNPP website. Once inside this tab, the link is on the left-hand side of the page,

“Evaluation Tools.” Manager needs to choose the three evaluation templates appropriate for the type of nurse candidate (RN vs. LPN) and their unit (Inpatient,

Outpatient, OR, Procedural or HR/PACU).

Manager completes the Manager Feedback tool and saves.

Candidate completes the Self Reporting Tool, save the document to their computer and emails the document to their Manager. **

Peer Feedback Tools need to be completed by two peers, or by 1 group of peers

(please make a note of all names on the “Evaluator’s Name” line.) Save the document and email back to the Manager. **

Manager will collect all evaluation tools and review for any concerns related to advancement.

** If using a Clinical Workstation, staff will need to print out the evaluation and give to the Manager. Manager will need to scan and save to their computer.

Step 3: Advancement Interview Questions

Candidate completes the Advancement Interview Questions based on his/her work over the past year that is reflective of the 5 pillars.

Step 4: Healthcare Team Endorsement

If the Candidate is advancing to either RN or LPN 4, he/she needs to ask a licensed member of the healthcare team that he/she works closely with to complete the “Healthcare Team Endorsement Form” and submit this along with the on-line application. This form must be completed by someone other than a nurse, unless the nurse is working in an APN role.

Step 5: Application submission

Once all of the steps above are completed, the Candidate submits her/her online application to advance on the VPNPP website.

Step 6: Packet submission

Within 72 hours of submitting the online application, Candidate needs to email their Advancement Interview Questions and Healthcare Team Endorsement (if applicable) to vpnpp@vanderbilt.edu

At the same time, Manager needs to email the CREDO Behavior Addendum and

4 evaluations (Manager, Self and 2 Peers/1 Group) to vpnpp@vanderbilt.edu

Final Steps:

Once the full application has been received at vpnpp@vanderbilt.edu

, the packet will be compiled by the VPNPP Program Coordinator and will have a preliminary review by one of the

VPNPP Clinical Co-Leads of the team.

After the preliminary review, the VPNPP team will then assign the Candidate to an Advocacy

Team for an interview. The members of the Advocacy Team will contact the candidate to set up the interview.

Once the interview has been completed, the Advocacy Team will present the Candidate to the

VPNPP Central Committee for final recommendations.

The Candidate and Manager will be notified of the decision made by the Central Committee. The emailed notification will include the date of advancement and the candidate’s increase in pay information.
